Retrospective analysis underestimates neurological deficits in complex spinal deformity surgery: a Scoli-RISK-1 Study.

OBJECTIVE The authors conducted a study to compare neurological deficit rates associated with complex adult spinal deformity (ASD) surgery when recorded in retrospective and prospective studies. Retrospective studies may underreport neurological deficits due to selection, detection, and recall biases. Prospective studies are expensive and more difficult to perform, but they likely provide more accurate estimates of new neurological deficit rates. METHODS New neurological deficits were recorded in a prospective study of complex ASD surgeries (pSR1) with a defined outcomes measure (decrement in

Global Alignment and Proportion (GAP) Score: Development and Validation of a New Method of Analyzing Spinopelvic Alignment to Predict Mechanical Complications After Adult Spinal Deformity Surgery.

BACKGROUND: The restoration of normal sagittal alignment is a critical goal in adult spinal deformity surgery to achieve favorable outcomes and prevent mechanical complications. Schwab sagittal modifiers have been accepted as targets for appropriate alignment, but addressing these targets does not always prevent high mechanical complication or revision rates. This may be because the linear absolute numerical parameters do not cover the whole pelvic incidence spectrum and the distribution of lordosis, pelvic anteversion, and negative malalignment are not considered as potential causes of failur

Late-onset axial myopathy and camptocormia in a calpainopathy carrier.

Camptocormia is a debilitating gait disorder characterized by the hyperflexion of the thoracolumbar spine during the upright position. Its etiologies are heterogenous, including parkinsonism and various neuromuscular disorders. Here, we report a camptocormia patient due to a late-onset axial myopathy with numerous lobulated fibers. The patient's father reportedly had similar symptoms. Myriad lobulated fibers are common among patients with an autosomal recessive muscular dystrophy due to calpain-3 gene (CAPN3) mutations or calpainopathy. CAPN3 sequencing revealed a single c.759-761delGAA mutati

Persistent activity limitations and fear of falling after surgical correction of adult spinal deformity.

BACKGROUND: Spinal realignment surgery for adult spinal deformity improves radiographic alignment and patient-reported outcomes, but it remains unclear whether these gains translate into recovery of activity-level function and reduced fear of falling. Therefore, this study aims to determine whether (1) objective activity limitations and fear of falling change following ASD realignment surgery, and (2) whether trajectories differ across deformity subtypes. METHOD: Fifty patients undergoing adult spinal deformity correction were assessed preoperatively, and at 6 months and 1 year postoperativel

Conflating Disability, Frailty, and Multimorbidity in Adult Spinal Deformity Patients: Seeking a Continuous Measure of Vulnerability.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To examine the degree of overlap between disability, multimorbidity, and frailty in a cohort of ASD patients. SUMMARY OF BACKGROUND DATA: Frailty is a popular topic in spine research, as it is a reported risk factor for poor outcomes. Disability, multimorbidity, and frailty can coexist, sometimes causing or exacerbating one another. It is important to distinguish these conditions for perioperative optimization and to guide research initiatives. METHODS: A multicenter registry of ASD patients was queried for baseline data regarding frailty

Predicting the Occurrence of Postoperative Distal Junctional Kyphosis in Cervical Deformity Patients.

BACKGROUND: Distal junctional kyphosis (DJK) development after cervical deformity (CD)-corrective surgery is a growing concern for surgeons and patients. Few studies have investigated risk factors that predict the occurrence of DJK. OBJECTIVE: To predict DJK development after CD surgery using predictive modeling. METHODS: CD criteria was at least one of the following: C2-C7 Coronal/Cobb > 10°, C2-7 sagittal vertical axis (cSVA) > 4 cm, chin-brow vertical angle > 25°. DJK was defined as the development of an angle <-10° from the end of fusion construct to the second distal vertebra, and chang

Postoperative Status of Global Sagittal Alignment With Compensation in Adult Spinal Deformity.

STUDY DESIGN: A retrospective study of consecutive patients undergoing surgery for adult spinal deformity (ASD). OBJECTIVE: The aim of this study was to classify postoperative ASD patients by sagittal compensation and characterize the features of each group. SUMMARY OF BACKGROUND DATA: Sagittal compensatory mechanisms to keep the erect position would function in not only nonoperative ASD patients but also in postoperative patients. However, details of sagittal compensatory mechanisms after surgery have been unclear, because the majority of previous studies examined the compensatory mechanism

Identification of Quantitative Trait Loci Associated with the Skeletal Deformity LSK complex in Gilthead Seabream (Sparus aurata L.).

Morphological abnormalities, especially skeletal deformities, are some of the most important problems affecting gilthead seabream (Sparus aurata L.) aquaculture industry. In this study, a QTL analysis for LSK complex deformity in gilthead seabream is reported. LSK complex is a severe deformity consisting of a consecutive repetition of three vertebral deformities: lordosis, scoliosis, and kyphosis. Seventy-eight offspring from six breeders from a mass-spawning were analyzed: five full-sibling families, three maternal, and two paternal half-sibling families. They had shown a significant associat

Prevalence and Key Radiographic Spinal Malalignment Parameters Associated with the Risk of Pulmonary Function Impairment in Patients Treated Surgically to Correct Adult Spinal Deformity.

INTRODUCTION: There is a significant relationship between pulmonary function and degree of spinal deformity, location of apical vertebrae, and coronal imbalance in patients with childhood spinal deformity. By contrast, the pathophysiology, epidemiology, and influence of deformity on respiratory dysfunction in patients with adult spinal deformity (ASD) remain largely unknown. We sought to clarify and compare the prevalence of pulmonary function impairment in patients with ASD with that in patients with lumbar spinal stenosis (LSS), to determine radiographically which spinal malalignment paramet

Effect of growth hormone to spinal growth and recombinant human growth hormone to scoliosis.

Growth hormone (GH) plays a key role in human growth and development. In addition to promoting height growth, GH affects bone metabolism, bone size, and bone mineral density (BMD) in children and adolescents by affecting bone formation and resorption. Among them, the effect of GH on spinal growth has been widely concerned. Scoliosis is a three-dimensional structural spinal deformity characterized by lateral curvature of one or more segments of the spine accompanied by vertebral rotation and sagittal imbalance. For children with growth hormone deficiency (GHD), whether GH supplementation leads

Using machine learning to automatically measure axial vertebral rotation on radiographs in adolescents with idiopathic scoliosis.

Adolescent idiopathic scoliosis is a 3D lateral spinal curvature coupled with axial vertebral rotation (AVR). Measuring AVR during clinic is important because it affects treatment options and predicts the risk of scoliosis progression. However, manual measurements are time consuming and have high inter-rater and intra-rater errors. This study aimed to develop a machine learning algorithm based on convolutional neural networks (CNNs) to automatically calculate AVR on posteroanterior radiographs using three different segmentations including spinal column, individual vertebra, and pedicles. Cervical laminectomy and fusion versus laminoplasty in degenerative cervical myelopathy: A propensity score matching analysis.

BACKGROUND: Degenerative cervical myelopathy (DCM) is a common cause of spinal cord dysfunction. The effectiveness of laminoplasty (LP) versus laminectomy with fusion (LF) for treating DCM is still debated. Here, we compared LP versus LF for treating DCM using propensity score matching (PSM) to minimize selection bias. METHODS: We identified DCM patients undergoing LP versus LF (2004-2022). Our analysis included demographics, preoperative/postoperative modified Japanese orthopedic association (mJOA) scoring, Visual Analog Scale (VAS) scores, assessment of postoperative cervical lordosis, oper

Surgical aspects and outcome of delayed tethered cord release.

Out of a total of 550 children followed up at our spina bifida center, we report on 81 patients who were reoperated upon for secondary tethered cord syndrome between 1993 and 2000. In four cases with preceding severe progressive scoliosis, untethering was followed by surgical correction and stabilization of curvatures. In 77 patients, the indication for surgery was based on late progressive neurological deterioration. The current clinical relevance of competing etiologic factors such as symptomatic Chiari malformation, hydromyelia, and shunt dysfunction, requiring different clinical management

Laminectomy and posterior cervical plating for multilevel cervical spondylotic myelopathy and ossification of the posterior longitudinal ligament: effects on cervical alignment, spinal cord compression, and neurological outcome.

OBJECTIVE: Multilevel anterior decompressive procedures for cervical spondylotic myelopathy or ossification of the posterior longitudinal ligament may be associated with a high incidence of neurological morbidity, construct failure, and pseudoarthrosis. We theorized that laminectomy and stabilization of the cervical spine with lateral mass plates would obviate the disadvantages of anterior decompression, prevent the development of kyphotic deformity frequently seen after uninstrumented laminectomy, decompress the spinal cord, and produce neurological results equal or superior to those achieved
