Degenerative disease involves all spinal components. Conventional radiology and digital radiology are the procedures of first choice in order to establish the diagnosis. CT and MRI should be considered of second choice, based on precise indications. The surgical management of clinical spinal instability is for many aspects controversial since the lack of a precise clinical and instrumental definition of instability complicates the implementation of a therapeutic protocol. Both the indications and the surgical procedure are not always well defined. Some of the most common surgical techniques used in the most frequent causes of instability of the lumbar spine: fractures, degenerative disease and spondylolisthesis, are described.

Primary non-Hodgkin lymphoma (PHL) occurs very rarely in bone. We report a case of a 63-year-old patient with PHL of the spine and an unusual postoperative complication leading to the development of a perforation of the transverse colon. After right hemicolectomy the patient recovered fully and was further treated with chemo- and radiotherapy. We emphasise the importance of an immunohistochemical analysis to exclude misdiagnosis.

The authors analyze problems inherent to the treatment of spine tumors (in particular surgical indications) especially with regard to metastases. The purpose of treatment must be that of improving the quality of life and, perhaps, prognosis. Prerequisites for successful reduction of cervical spine injuries are an exact analysis and classification of every lesion. In locked dislocations disc protrusion should be excluded prior to reduction by MRI or CT-scan.
