Should early strabismus surgery be performed for ocular torticollis to prevent facial asymmetry?

To help determine whether ocular torticollis causes facial asymmetry, we analyzed photographs of patients with long-standing head tilts for … eping with its head turned predominantly to one side during the first 6 to 12 months of life. Early strabismus surgery to correct the head tilt may help prevent facial asymmetry, but ensuring that the infant sleeps with alternating head positions may be more important.

Natural strabismus in monkeys. Convergence errors assessed by cover test and photographic methods.

A standard set of clinical prism and cover tests and a recently developed photographic method were used to assess binocular alignment in ten monkeys that previously were determined to have a naturally occurring infantile strabismus. Extensive measurements of the alignment state were made for fixation attempts throughout the field of gaze. Patterns of alignment errors were examined in an attempt to compared the strabismus found in individual monkeys with common syndromes of human infantile strabismus. Two monkeys showed patterns consistent with the syndrome of essential infantile esotropia. Fiv…

Quantification of Gaze-Dependent Corrective Errors With Fresnel Prisms: Implications for Strabismus Management.

PURPOSE: Fresnel prisms are widely used to manage strabismus. Although prism power is typically selected based on the deviation in the primary gaze position, actual deflection varies with the gaze direction because the angle of incidence changes. This study quantified gaze-dependent corrective errors in the secondary and tertiary positions. METHODS: Fresnel prisms (10Δ-40Δ) were mounted on a two-axis rotation stage. Deflection was measured by projecting a laser spot onto a tangent screen 100 cm away while rotating the prisms horizontally and/or vertically (±30°). Two configurations were teste…

Surgical outcomes of Scott 2:1 recession-resection adjustable technique for incomitant strabismus.

BACKGROUND: Surgical management of incomitant strabismus, characterized by gaze-dependent misalignment, is challenging. The Scott procedure, which involves the combined recession-resection of the same extraocular rectus muscle, avoids posterior scleral suturing beyond the globe's equator and allows for postoperative adjustment. However, long-term outcomes and optimal surgical dosing remain unclear. This study is the largest series to date that reports outcomes of a 2:1 same-muscle recession-resection dosing strategy. METHODS: A retrospective chart review (January 2014 to August 2024) identifi…

Evaluation of Scleral Adjustment Method: A Novel Adjustable Suture Technique in Strabismus Surgery.

To determine whether passing a pole suture through the sclera at two points provides fixation comparable to that of a sliding noose, we measured the tensile strength of the suture‒sclera interface during simulated traction. In this in vitro study, three suture patterns were evaluated in porcine eyeballs, using 6-0 polyglycolic acid sutures. Patterns A (control), B (second suture pass perpendicular), and C (second suture pass in the same direction) were compared. The tensile strength of each pattern was measured 20 times using a KANON TK300CN, and the results were analyzed using the Kruskal‒Wal

Surgical Approaches for Residual Secondary Gaze Diplopia After Strabismus Surgery: A Case Series.

This case series describes three patients who experienced residual diplopia in secondary gaze positions despite successful strabismus surgery and satisfactory alignment in primary gaze. While initial procedures effectively resolved diplopia in the primary position, symptoms remained in secondary gaze and interfered with daily activities. Each patient underwent an additional surgical procedure specifically addressing the residual diplopia, leading to significant improvement or complete resolution of symptoms. These findings highlight that although primary gaze alignment is the primary goal of s…
