Immunometabolic regulation of cardiac macrophages in heart failure with preserved ejection fraction.

Heart failure with preserved ejection fraction (HFpEF) is a complex syndrome driven by comorbidities, chronic low-grade inflammation, and cardiac remodeling. Cardiac resident macrophages (CRMs) and monocyte-derived macrophages (MDMs) play central roles in linking immunometabolism to HFpEF pathogenesis. Resident CCR2- macrophages maintain homeostasis and tissue repair, whereas monocyte-derived CCR2+ macrophages drive inflammation, fibrosis, and diastolic dysfunction. In HFpEF, comorbidity-induced metabolic shifts promote pro-inflammatory macrophage polarization, while metabolites like fatty aci

Cardiac dysfunction in HIV: Etiology, pathophysiology, and future directions.

The advent of highly active antiretroviral treatment (HAART) has turned human immunodeficiency virus (HIV) from an acute, high mortality illness to a chronic, treatable disease. While people living with HIV are expected to have a normal life expectancy, this puts HIV infected individuals at risk of developing HIV- associated chronic diseases which includes cardiovascular disease. HIV-associated heart disease may lead to HIV associated cardiomyopathy (HIV CM), which mostly includes heart failure with reduced ejection fraction (HFrEF). There has also been growing evidence of diastolic dysfunctio

The Mechanisms Associated with Inflammation and Coronary Microvascular Dysfunction in Heart Failure with Preserved Ejection Fraction.

<p>Heart failure is a complex clinical syndrome, with more than half of the cases classified as heart failure with preserved ejection fraction (HFpEF). HFpEF is strongly associated with comorbidities such as hypertension, obesity, diabetes, anemia, chronic kidney disease, chronic obstructive pulmonary disease, advanced age, and female gender. Despite its high prevalence, the exact pathogenesis of HFpEF remains poorly understood, leading to limited therapeutic options and poor prognosis. Recently, a novel paradigm has emerged, suggesting that comorbidities drive myocardial dysfunction and remod

An approach to the older patient with a suspicion of HFpEF.

It is not uncommon to see older patients who have dyspnea limiting exercise tolerance and a non-diagnostic H2FPEF score, but with an estimated probability of HFpEF of 55-75%. These patients often have dependent edema or hypertension. Frequently we, and the patients themselves, believe their symptoms are due to age, obesity, and deconditioning; however, recent data is suggesting the presence of HFpEF. Even without HFpEF, they are at high risk of developing it over time. There is reluctance in proceeding with a hemodynamic stress test, as it is not readily available and invasive. Both dyspnea on

Current indications and surgical strategies for myocardial revascularization in patients with left ventricular dysfunction: a scoping review.

BACKGROUND: Ischemic cardiomyopathy (ICM) accounts for more than 60% of congestive heart failure cases and is associated with high morbidity and mortality rates. Myocardial revascularization in patients with left ventricular dysfunction (LVD) and a left ventricular ejection fraction (LVEF) ≤35% aims to improve survival and quality of life and reduce complications associated with heart failure and coronary artery disease. The majority of randomized clinical trials have consistently excluded those patients, resulting in evidence primarily derived from observational studies. MAIN BODY: We perfor

Obesity: the perfect storm for heart failure.

Obesity condition causes morphological and functional alterations involving the cardiovascular system. These can represent the substrates for different cardiovascular diseases, such as atrial fibrillation, coronary artery disease, sudden cardiac death, and heart failure (HF) with both preserved ejection fraction (EF) and reduced EF. Different pathogenetic mechanisms may help to explain the association between obesity and HF including left ventricular remodelling and epicardial fat accumulation, endothelial dysfunction, and coronary microvascular dysfunction. Multi-imaging modalities are requir

Indian Consensus on the Role and Position of Angiotensin Receptor-neprilysin Inhibitors in the Management of Heart Failure.

The incidence of heart failure (HF) in India is estimated to be 0.5-1.7 cases per 1,000 people per year, and approximately 4,92,000-1.8 million new cases are detected every year. Despite the high rate of mortality associated with HF, most patients do not receive maximal guideline-directed medical therapy (GDMT). Current guidelines advocate early multidrug combination therapy with four classes of drugs, namely, beta-blockers (BBs), mineralocorticoid receptor antagonists (MRAs), angiotensin receptor-neprilysin inhibitors (ARNIs), and sodium-glucose transport protein 2 inhibitors (SGLT-2is), part

Controversies and Conundrums in Cardiac Cachexia: Key Questions About Wasting in Patients With HFrEF.

Cardiac cachexia is characterized by unintentional catabolic weight loss, decreased appetite, and inflammation and is common in patients with stage D (advanced) heart failure with reduced ejection fraction (HFrEF). Cardiac cachexia and related muscle-wasting syndromes are markers of, and a consequence of, the heart failure (HF) syndrome. Although many potential modalities for identifying cardiac cachexia exist, the optimal definition, diagnostic tools, and treatment options for cardiac cachexia remain unclear. Furthermore, it remains unclear whether attempts to reverse muscle wasting prior to

Robust Arm Impedocardiography Signal Quality Enhancement Using Recursive Signal Averaging and Multi-Stage Wavelet Denoising Methods for Long-Term Cardiac Contractility Monitoring Armbands.

Impedance cardiography (ICG) is a low-cost, non-invasive technique that enables the clinical assessment of haemodynamic parameters, such as cardiac output and stroke volume (SV). Conventional ICG recordings are taken from the patient's thorax. However, access to ICG vital signs from the upper-arm brachial artery (as an associated surrogate) can enable user-convenient wearable armband sensor devices to provide an attractive option for gathering ICG trend-based indicators of general health, which offers particular advantages in ambulatory long-term monitoring settings. This study considered the …

Systemic Effects of Homoarginine Supplementation on Arginine Metabolizing Enzymes in Rats with Heart Failure with Preserved Ejection Fraction.

A restoration of low homoarginine (hArg) levels in obese ZSF1 rats (O-ZSF1) before (S1-ZSF1) and after (S2-ZSF1) the manifestation of heart failure with preserved ejection fraction (HFpEF) did not affect the worsening of cardiac HFpEF characteristics. Here, potential regulation of key enzymes of arginine metabolism in other organs was analyzed. Arginase 2 (ARG2) was reduced >35% in the kidney and small intestine of hArg-supplemented rats compared to O-ZSF1. Glycine amidinotransferase (GATM) was 29% upregulated in the kidneys of S1-ZSF1. Dimethylarginine dimethylaminohydrolase 1 (DDAH1) levels

[Heart failure with preserved ejection fraction (HFpEF) : Diagnosis and treatment].

Heart failure with preserved ejection fraction (HFpEF) accounts for around half of all hospitalizations associated with heart failure. The prevalence of HFpEF is increasing, mainly due to an aging population and a growing burden of comorbidities, such as hypertension, diabetes, and obesity. Despite increased research efforts, there are still important gaps in terms of the pathophysiological understanding of HFpEF and the practice-related diagnostics. As HFpEF may also be due to rare cardiac diseases, in unclear constellations patients should be referred at an early stage to specialized centers

Deep phenotype characterization of hypertensive response to exercise: implications on functional capacity and prognosis across the heart failure spectrum.

AIMS: Limited evidence is available regarding the role of hypertensive response to exercise (HRE) in heart failure (HF). We evaluated the systolic blood pressure (SBP) to workload slope during exercise across the HF spectrum, investigating haemodynamic and prognostic correlates of HRE. METHODS AND RESULTS: We prospectively enrolled 369 patients with HF Stage C (143 had preserved [HFpEF], and 226 reduced [HFrEF] ejection fraction), 201 subjects at risk of developing HF (HF Stages A-B), and 58 healthy controls. We performed a combined cardiopulmonary exercise stress echocardiography testing. We

Uric acid-lowering therapy with benzbromarone in hypertension with asymptomatic hyperuricemia: a randomized study focusing left ventricular diastolic function.

BACKGROUND: Both hypertension and hyperuricemia are closely associated with the morbidity and mortality of heart failure with preserved ejection fraction (HFpEF). However, there is limited evidence on the effect of uric acid-lowering therapy on left ventricular (LV) diastolic function in this population. In this randomized study, we prescribed benzbromarone, a uric acid-lowering drug, to those with hypertension and asymptomatic hyperuricemia to investigate its clinical benefits by evaluating LV diastolic function, incidence of HFpEF and hospitalization for heart failure and cardiovascular deat

Predictors of left ventricular thrombus after acute myocardial infarction: a systematic review and meta-analysis.

Left ventricular thrombus (LVT) is a recognized complication of acute myocardial infarction (AMI) which can cause significant morbidity and mortality from systemic embolism. We conducted a systematic review and meta-analysis of factors that have been shown in studies to be independently predictive of LVT post-AMI. A total of 23 studies met the inclusion criteria with 1 047 785 patients. The proportion of patients with LVT ranged from 0.2% in the nationwide study in the USA to 36.1% in the cohort of patients with LV aneurysm. Pooled results from nine studies suggest that greater values for left

Clinical profiling of end-stage heart failure with preserved ejection fraction: The National Readmission Database.

BACKGROUND: End-stage (Stage D) heart failure with preserved ejection fraction (HFpEF) is a poorly characterized syndrome that has heterogeneous underlying pathophysiology. A better characterization of the various clinical profiles of Stage D HFpEF is needed. METHOD: 1066 patients with Stage D HFpEF were selected from National Readmission Database. A Bayesian clustering algorithm based on a Dirichlet process mixture model was implemented. Cox proportional hazard regression model was used to relate the risk of in-hospital mortality with each identified clinical cluster. RESULT: 4 distinct cli

Pulmonary Hypertension in Left Heart Diseases: Pathophysiology, Hemodynamic Assessment and Therapeutic Management.

Pulmonary hypertension (PH) associated with left heart diseases (PH-LHD), also termed group 2 PH, represents the most common form of PH. It develops through the passive backward transmission of elevated left heart pressures in the setting of heart failure, either with preserved (HFpEF) or reduced (HFrEF) ejection fraction, which increases the pulsatile afterload of the right ventricle (RV) by reducing pulmonary artery (PA) compliance. In a subset of patients, progressive remodeling of the pulmonary circulation resulted in a pre-capillary phenotype of PH, with elevated pulmonary vascular resist
