A phase I/II clinical trial of neoadjuvant docetaxel/oxaliplatin/S‑1 (DOS) combination therapy for gastroesophageal junction adenocarcinoma.

BACKGROUND: Gastroesophageal junction (GEJ) adenocarcinoma is aggressive with poor prognosis. Perioperative chemotherapy has become standard, but the optimal regimen remains uncertain. DOS has shown activity in gastric cancer but has not been prospectively assessed in GEJ adenocarcinoma. This study represents the first prospective trial of DOS as neoadjuvant chemotherapy in patients with resectable GEJ adenocarcinoma. METHODS: This single-arm, single-center phase I/II trial (UMIN000022210) enrolled patients with cT3-4aN0-3M0 GEJ adenocarcinoma. The phase I part established the recommended doc

Efficacy and safety of disitamab vedotin (RC48) combined with camrelizumab and S-1 for neoadjuvant therapy of locally advanced gastric cancer with HER2-overexpressing: Preliminary results of a prospective, single-arm, phase II study.

PURPOSE: Perioperative treatment of gastric and gastroesophageal junction (G/GEJ) cancer is evolving towards multimodal strategies incorporating HER2-targeted therapy, immunotherapy and chemotherapy. Disitamab vedotin (RC48), an HER2-targeted antibody-drug conjugate, shows promising antitumour activity and potential synergy with immune checkpoint inhibitors. This study evaluated neoadjuvant RC48 combined with camrelizumab and S-1 in resectable HER2-overexpressing locally advanced G/GEJ adenocarcinoma. METHODS: Patients with histologically confirmed HER2-overexpressing (IHC 3+ or 2+) resectabl

HER2-Directed Therapy in Advanced Gastric and Gastroesophageal Adenocarcinoma: Triumphs and Troubles.

Gastric and gastroesophageal junction (GEJ) cancers represent the third leading cause of malignancy-associated death worldwide. Approximately 15-20% of these adenocarcinomas overexpress the human epidermal growth factor receptor 2 (HER2), a pro-proliferative receptor tyrosine kinase that has been therapeutically exploited in other disease contexts. The landmark ToGA trial demonstrated that trastuzumab, an anti-HER2 antibody, could improve overall survival for patients with HER2 overexpressing advanced gastric and GEJ adenocarcinomas. In the ensuing decade, great effort has been made to refine

A feasibility study of gemcitabine, S-1 and leucovorin combination therapy (GSL) for advanced biliary tract cancer.

Objective: Gemcitabine and cisplatin (GC) combination chemotherapy is the current standard of care for patients with advanced biliary tract cancer (BTC). Recently, a randomized controlled trial showed the non-inferiority in overall survival of gemcitabine and S-1 (GS) compared to GC. Because leucovorin is known to enhance the activity of S-1, we conducted this study to evaluate the feasibility of combination therapy of gemcitabine, S-1 and leucovorin (GSL). Methods: Advanced BTC patients without prior treatment other than surgery or adjuvant chemotherapy were eligible to this study. Gemcitabin

A feasibility study of postoperative chemotherapy with S-1 and cisplatin (CDDP) for stage III/IV gastric cancer (CCOG 1106).

BACKGROUND: The standard of care for stage II/III gastric cancer in Japan is D2 dissection followed by adjuvant S-1 monotherapy. Outcome of patients with stage III disease remains unsatisfactory, calling for a more intensive adjuvant chemotherapy regimen, for which evidence in advanced/metastatic cancer research suggests S-1/cisplatin (CDDP) as a candidate. Although S-1/CDDP was poorly tolerated postoperatively in the previous trial, compliance was dramatically improved by insertion of one cycle of S-1 monotherapy, which delayed administration of CDDP by 6 weeks. METHODS: A feasibility study

Phenylbutyrate mouthwash mitigates oral mucositis during radiotherapy or chemoradiotherapy in patients with head-and-neck cancer.

PURPOSE: Deleterious oral mucositis (OM) develops during radiotherapy (RT) or chemoradiotherapy for head-and-neck cancer (HNC) patients. There are currently no effective cytoprotective treatments for OM without a potential risk of tumor protection. This randomized, double-blind, placebo-controlled pilot study aimed to determine the therapeutic safety and efficacy of phenylbutyrate (an antitumor histone deacetylase inhibitor and chemical chaperone) 5% mouthwash for treating OM caused by cancer therapy. METHODS AND MATERIALS: Between September 2005 and June 2006, 36 HNC patients were randomized

A crossover study of oral administration of UFT in chronic liver disease: comparison of continuous and intermittent schedules.

This study was aimed at evaluating the tolerance to an intermittently administered oral UFT for hepatocellular carcinoma (HCC) with chronic liver disease (CLD). Ten patients who had received curative therapy for HCC with CLD (Child's classification A or B) were randomly assigned either an intermittent schedule (IS), oral administration of UFT (130 mg/m2/b.i.d.) with 2 days rest a week, or a continuous schedule (CS), consecutive administration of UFT with the same dose. On day 12, the serum concentration of 5-fluorouracil (5-FU) was measured. After 2 weeks rest, the patients were switched to th

Neoadjuvant PD-1 inhibitor combined with FLOT versus SOX for locally advanced gastric cancer: a retrospective cohort study.

BACKGROUND: Neoadjuvant immunochemotherapy is increasingly used for resectable locally advanced gastric cancer (LAGC) and gastroesophageal junction (EGJ) adenocarcinoma, yet the optimal chemotherapy backbone for PD-1 blockade remains unclear. We compared neoadjuvant PD-1 inhibitor plus S-1+oxaliplatin (SOX) versus PD-1 inhibitor plus 5-FU+oxaliplatin+Docetaxel+Leucovorin (FLOT) in a real-world cohort. METHODS: This single-center retrospective study included patients with resectable, HER2-negative LAGC/EGJ adenocarcinoma (cT3-4b, any N+, M0; ECOG 0-1) treated between July 2020 and July 2025. P

Chemotherapy-induced CAF-associated stromal remodeling in a humanized stroma pancreatic ductal adenocarcinoma organoid xenograft model: a comparison between GP and GS.

Pancreatic ductal adenocarcinoma features a dense, desmoplastic tumor microenvironment (TME) dominated by cancer-associated fibroblasts (CAFs). While gemcitabine plus nab-paclitaxel (approximated as gemcitabine plus paclitaxel [GP]) and gemcitabine plus S-1 (GS) are standard regimens with distinct clinical indications, their specific effects on tumor suppression and CAF-associated stromal remodeling remain unclear. Using a humanized, three-dimensional (3D) S2-013 organoid xenograft model integrating human mesenchymal stem cells (MSCs) and human endothelial cells, we investigated the histopatho

Outcomes of FOLFIRINOX versus gemcitabine plus nab-paclitaxel in patients with early recurrent pancreatic cancer after adjuvant S-1 treatment: A propensity score-matched analysis.

BACKGROUND: Oral fluoropyrimidine, S-1 is the standard adjuvant chemotherapy for patients with resected pancreatic cancer (PC) in Japan. Patients experiencing early recurrence are typically treated with 5-fluorouracil, leucovorin, irinotecan, and oxaliplatin (FOLFIRINOX) or gemcitabine plus nab-paclitaxel (GnP), which are commonly used in patients with advanced PC. However, no clinical studies have compared these regimens in this particular population. METHODS: This multicenter retrospective study included PC patients who experienced recurrence within six months of the last S-1 administration

Skin grafting utilizing a skin of lateral thoracic area for chest wall reconstruction in patient who underwent mastectomy for locally advanced breast cancer.

Locally advanced breast cancer sometimes results in a large chest wall defect at mastectomy. When closing the wound horizontally, the skin tension is usually severe in the middle of the wound, while the skin of the lateral area tends to make a dog-ear deformity. Triangle technique is a procedure to prevent the dog ear in which the skin and subcutaneous fat of the axilla are cut into an equilateral triangle. Herein, we present a case of breast cancer who underwent a mastectomy and closed the wound with a skin graft by utilizing the skin removed from lateral thoracic area using triangle techniqu
