Indocyanine green-guided thoracoscopic wedge resection for pulmonary sequestration.

This video tutorial demonstrates a right-sided video-assisted thoracoscopic surgery approach for the management of an intralobar pulmonary sequestration in an adult. Although conservative or endovascular approaches exist, surgical resection is recommended to prevent recurrent infections and mitigate the risk of malignant transformation. While anatomical lobectomy has traditionally been the standard, lung-sparing techniques are increasingly preferred to preserve pulmonary function. The procedure emphasizes the isolation and division of the aberrant systemic arterial supply arising from the desc

Multidisciplinary management of video-assisted thoracoscopic left cardiac sympathetic denervation in a young child with Jervell and Lange-Nielsen syndrome.

Left cardiac sympathetic denervation (LCSD) via video-assisted thoracoscopy (VATS) is an effective therapy for drug-refractory malignant arrhythmias in congenital long QT syndromes and requires meticulous perioperative planning in children with automatic implantable cardioverter-defibrillators (AICDs). We describe what is likely the first paediatric VATS-LCSD performed in Pakistan. A boy in early childhood with Jervell and Lange-Nielsen syndrome, severe QT prolongation and recurrent ventricular arrhythmias despite beta-blockade and mexiletine had received multiple AICD shocks. Intraoperative m

Splenic rupture during secondary pleural decortication in a patient with rheumatoid arthritis: a case report.

A 63-year-old female underwent video-assisted thoracoscopic surgery (VATS) for pleural decortication due to recurrent pneumothorax four months after left lung surgery. The patient experienced hemodynamic instability during repositioning from lateral to supine post-procedure. No injury to the diaphragm or chest wall was identified during the procedure and no source of bleeding was identified intraoperatively. The patient was transferred to the ICU. A bedside ultrasound revealed a large amount of free fluid in the abdomen. Diagnostic paracentesis yielded non-clotting blood, suggestive of intra-a

Right lung isolation in the prone position using an EZ-Blocker through an armoured endotracheal tube for combined spine and video-assisted thoracoscopic surgery.

Surgical correction is the definitive treatment for adolescent idiopathic scoliosis (AIS), but 25% of cases may experience screw migration, risking injury to thoracic structures. Video assisted thoracoscopic surgery (VATS) can visualise malpositioned screws and resolve injury. This report presents a case of an AIS patient who underwent prone VATS for simultaneous screw correction after posterior spinal fusion. While traditional VATS uses a double lumen tube (DLT) in the lateral decubitus position, DLTs are less ideal for prone surgery. Bronchial blockers (BB), like the EZ-Blocker (Teleflex, Wa

Spontaneous haemothorax due to a non-invasive mediastinal thymoma.

A male patient in his late 60s presented with sudden-onset chest pain, dry cough and exertional dyspnoea. He had a history of renal cell carcinoma treated with nephrectomy 10 years ago, and a biopsy-proven benign anterior mediastinal mass, thymoma, diagnosed 7 years earlier, for which he denied surgical resection and was lost to follow-up. Imaging revealed a large anterior mediastinal mass with associated left-sided moderate pleural effusion. Image-guided biopsy of the mass confirmed it to be type A thymoma. Thoracoscopy showed normal pleura with haemothorax. Positron emission tomography-CT sc

A Case of Intramural Esophageal Bronchogenic Cyst.

A bronchogenic cyst is a congenital malformation with a rare esophageal origin. Occasionally, it is discovered incidentally, and excision is the only treatment. A 43-year-old patient was referred to our hospital for treatment of a cystic tumor in the left posterior mediastinum. We suspected the tumor was a bronchogenic cyst by imaging findings; therefore, we performed video-assisted thoracoscopic resection. The patient was discharged 9 days postoperatively. The tumor origin was found to be the esophageal wall. This report presents a rare case in which the esophageal wall was the origin of the

Adult pulmonary sequestration: a hidden cause of recurrent hemoptysis (case report).

Pulmonary sequestration (PS) is a rare congenital malformation where abnormal lung tissue lacks a connection to the airways and receives blood supply from systemic arteries. This case report describes a 30-year-old man with recurrent hemoptysis diagnosed with intralobar PS (ILS) as the cause. He underwent video-assisted thoracoscopic surgery to clip and section the aberrant artery, resulting in a successful outcome with no further bleeding. The case emphasizes considering ILS in diagnosing hemoptysis, even in adults with atypical presentations, and highlights the utility of CT angiography in i

Robot-Assisted Thoracoscopic Resection of Ectopic Parathyroid Tissue in Mediastinum: A Scoping Review.

Ectopic parathyroid tissue in the mediastinum represents a rare and potentially serious clinical entity with challenging diagnostic and surgical management. The main surgical approach for this issue is open thoracotomy, which is a safe technique but carries risks and complications associated with such as extensive surgery. The purpose of this study is to demonstrate robotic thoracic surgery as an alternative and potentially advantageous method to treat patients with ectopic parathyroid adenomas compared with open and video-assisted thoracoscopic surgery approaches. A total of 13 reports in the

Uniportal left S1 + 2 segmentectomy.

The left upper lobe is one of the largest lobes of the lung; left upper segmentectomy is well established among thoracic surgeons. In uniportal left S1 + 2 segmentectomy, dissection of the vasculature, bronchus, and intersegmental plane can be performed anteriorly. Given that the fissureless technique is commonly used in uniportal video-assisted thoracoscopic surgery, S1 + 2 segmentectomy exhibits high affinity with the unidirectional approach. We have frequently performed left S1 + 2 segmentectomy for early non-small cell lung cancer located in the apical segment, since this procedure has the

Lung Ultrasound to Assess One Lung Ventilation: A Pediatric Case Series.

Objectives: One lung ventilation (OLV) is the preferred ventilation technique for thoracoscopy as it provides a better exposure of the operative field and grants the protection of the healthy lung. Preoperative evaluation of lung exclusion is necessary and different methods are available. In recent years lung ultrasound (US) gained popularity and its use for monitoring the endotracheal tube position is widely reported. The existing evidence on adults addresses lung US as effective, yet only few data are available in children. Therefore, we present our experience with lung US as verification me

Thoracic benign cystic mesothelioma.

BACKGROUND: Benign cystic mesotheliomas (BCMs), also known as multilocular mesothelial inclusion cysts, inflammatory inclusion cysts or multicystic mesothelial proliferation, are frequently observed in females and are localised localised in the pelvic peritoneum. They are rarely present in the thoracic and mediastinal areas; however, these locations have been reported in a few cases in the literature. CASE PRESENTATION: We present the case of a woman with an intrathoracic BCM. A 28-year-old female patient presented with a cystic mass of 8 × 6 × 6 cm in the left hemithorax shown by computed to

PURPOSE: Research has shown that a higher dose of bupivacaine administered in continuous paravertebral block (CPVB) provides a greater analgesic effect after video-assisted thoracoscopic surgery (VATS). In this randomized, controlled, double-blind study, we hypothesized that 0.25% 8 ml/h of levobupivacaine administered in CPVB after VATS provides a greater analgesic effect than 0.125% 8 ml/h. METHODS: Fifty patients who underwent unilateral VATS were randomized to receive a postoperative continuous infusion of 0.125% (low group, n = 25) or 0.25% (high group, n = 25) levobupivacaine at 8 mL/h
