Denosumab: a case of MRONJ with resolution.

A patient was referred by a clinical oncologist regarding a non-healing area of bone in the lower jaw. The patient was taking denosumab (Prolia(®), Xgeva(®)) when they had a tooth extracted and the area failed to heal. Following suspension of the drug, the area healed with mucosal coverage. This new class of drugs are being increasingly used as an alternative antiresorptive drug to bisphosphonates and are licenced in the UK for prevention of osteoporotic fractures and prevention of skeletal-related events (SREs) in patients with metastatic cancer.

Interdisciplinary management in the paediatric patient: restoration of a complicated crown-root fracture.

Complicated crown-root fractures in permanent teeth present both patient-centred and restorative problems when treating the adolescent. This case highlights an alternative and conservative technique for the management of a traumatically involved maxillary left central incisor in a 12-year-old boy. The injury was successfully managed through an interdisciplinary approach using a combination of endodontics, minor oral surgery and orthodontics. The approach resulted in utilizing the patient's own tooth fragment to facilitate restoration back into successful function and aesthetics with the absenc

Endoscopic visualization of the submarginal gingiva dental sulcus and tooth root surfaces.

BACKGROUND: Direct, real-time visualization of the hard and soft tissues within the gingival sulcus may aid the clinician in diagnosis and therapy of periodontal disease. This report describes an endoscope specifically designed for this purpose and the interpretation of dental endoscopic images. METHODS: Medical endoscope technology was modified for application in the dental environment. A fixed, fused fiber optic bundle, less than 1 millimeter in diameter, was coupled to an active matrix LCD-TFT flat panel video monitor for viewing by the clinician. A bilumen sheath was designed to provide i

The stress distribution of a primary molar tooth restored with stainless steel crown using different luting cements.

BACKGROUND: The aim of this study is to evaluate the stress distributions of a primary molar tooth restored with a stainless steel crown (SSC) using resin and glass ionomer luting cements by Finite Element Analysis (FEA). METHODS: Original DICOM data of a primary molar was used to create a 3D model. One model was prepared as a tooth model with SSC. A 30 μm cement layer was used in model. Two different luting cements were tested in the study: self-cure adhesive resin cement, and glass ionomer cement. Vertical and oblique loads of 330 N were applied to simulate maximum bite force and lateral fo

Medication-related osteonecrosis of the jaw after tooth extraction in patients receiving pharmaceutical treatment for osteoporosis: A retrospective cohort study.

BACKGROUND/PURPOSE: Bone resorption inhibitors, such as bisphosphonates (BPs) and anti-receptor activator of nuclear factor kappa B ligand antibodies (denosumab; Dmab), are used to treat osteoporosis and effectively reduce the risk of fracture. However, medication-related osteonecrosis of the jaw (MRONJ) has been reported as a rare adverse effect. Invasive tooth extraction procedures are reportedly a factor in the development of MRONJ. In this study, we aimed to retrospectively observe and clinically examine the effect of medication status on MRONJ development after tooth extraction in patient

Repair of extensive apical root resorption associated with apical periodontitis: radiographic and histologic observations after 25 years.

INTRODUCTION: Root resorption is a frequent finding in teeth with apical periodontitis. In cases of severe apical periodontitis, root resorption may involve not only cementum but also dentin. Resorbed tooth structures can only be repaired with cementum because stem cells in the periradicular tissues are not capable of differentiating into odontoblasts. This article reports the repair of extensive apical root resorption associated with apical periodontitis 25 years after treatment. METHODS: A 51-year-old man presented with pulp necrosis and symptomatic apical periodontitis in tooth #7. The per

Reestablishing biologic width with forced eruption.

Maintenance of gingival health is one of the keys for the longevity of teeth, as well as for the longevity of restorations. In this context, the biologic width functions as a barrier against the entrance of microorganisms into the internal medium of the periodontal ligament and into the gingival and osseous connective tissue. This clinical case describes a technique to reestablish the biologic width of a central incisor using forced extrusion and done without post-treatment corrective surgery.

A Modified Technique of Percutaneous Fixation of Displaced Pediatric Lateral Humeral Condyle Elbow Fractures (aka the Martini Technique): A Brief Case Series.

Displaced lateral humeral condyle (LHC) fractures have routinely been treated with open reduction, which has known postoperative complications. Recent reports show that closed reduction and percutaneous pinning (CRPP) is a valid treatment. Five pediatric patients with displaced LHC fractures were included in a retrospective case series. Closed reductions (CRs) were facilitated by Kirschner wire placement into the capitellum through a toothed drill guide. The Kirschner wire and drill guide were used like a joystick to manipulate the fragment and maintain reduction for placement of additional Ki

Dental fracture and chocolate candies: case report.

A complaint by a customer to a food company claimed that the consumption of a chocolate candy fractured his anterior teeth, due to its hard consistency. Fragments of the fractured teeth and the chocolate candy that supposedly caused it were collected, examined and photographed. Fragments presented caries, large restorations, and suggested previous endodontic treatment. To evaluate causation, the food company requested a laboratory analysis, which simulated the human bite on chocolate candies of the same brand. Human teeth were assembled in a simulating device of masticatory functions of appreh

Acquired maxillofacial defects from motor vehicle accidents: statistics and prosthodontic considerations.

Trauma from motor vehicle accidents is one of the leading causes of death in the United States; moreover, the costs related to personal harm are only second to cancer. Head and neck injuries predominate and involve contact with the vehicle's interior (80%), contact with the vehicle's exterior (12%), or noncontact with the vehicle (8%). The patient with maxillofacial defects resulting from motor vehicle accident will have numerous soft tissue and hard tissue injuries ranging from neurologic involvement to fractures and/or avulsions of the temporomandibular joint, maxillae, mandible, teeth, and

Smile Reestablished-Predictable Management of Complicated Crown-root Fracture through Surgical Extrusion with and without 180° Rotation: A Case Report.

AIM AND BACKGROUND: The management of crown-root fractures poses significant uncertainty and involves meticulous evaluation of the tooth and supporting structures. The uncertain prognosis associated with crown-root fractures necessitates periodic evaluation. CASE DESCRIPTION: A 15-year-old female presenting with crown-root fracture of the permanent left maxillary incisors was treated by surgical extrusion with rotation for the central incisor, while the lateral incisor was coronally extruded by 2 mm. Root canal treatment was performed after the extrusion procedure, which was followed by full

When to restore: Critical crack-size thresholds in human first premolars.

STATEMENT OF PROBLEM: Although cracked teeth have been a prevalent clinical concern, a consensus on appropriate treatment strategies-particularly for early-stage cracks without clinical symptoms-remains lacking. Current guidelines do not specify the critical crack dimensions, in terms of depth and width, at which tooth structural integrity becomes significantly compromised, leading to uncertainty in clinical decision-making between monitoring and restorative intervention. PURPOSE: The purpose of this study was to investigate the effects of crack depth and width on the ultimate strength and st

Flexible wire composite splinting for root fracture of immature permanent incisors: a case report.

Root fracture injuries affect up to 7% of permanent teeth, and injury in teeth with immature root formation is even rarer. The purpose of this paper was to report the case of a 7-year-old girl who experienced pain in her permanent maxillary central incisors following a fall from a bicycle. A radiographic examination revealed immature maxillary central incisors with mid-root oblique and horizontal root fractures. Splinting was performed, and the child was observed under a regular follow-up regime. After 18 months, clinical examination showed normal tooth color and position, with a positive resp

Mandibular Fracture in a 3-Month-Old Infant: Conservative Management and Diagnostic Considerations.

Mandibular fractures are rare in infants under one year of age, accounting for only 0.9%-2.6% of pediatric mandibular injuries. Their infrequency is due to the unique anatomical and developmental features of the infant mandible, including increased bone elasticity and minimal exposure to high-impact trauma. Diagnosis and management are often challenging, particularly given the difficulty in symptom communication, presence of unfused skeletal structures, and developing tooth buds. We described a case of a 3-month-old infant who sustained concurrent fractures of the mandibular symphysis and left

Experimental autogenous tooth transplantation in the dog: a comparison between one- and two-stage surgical techniques.

Crucial if tooth transplantation is to succeed is preservation of the vitality of the cells of the periodontium and cementum of the tooth graft. Poor contact between the tissues of the recipient site and the root surface when teeth are transplanted to recipient sites prepared immediately prior to transplantation (one-stage technique) could, after transplantation, result in insufficient nutrition to the cells of the root surface and contribute to necrosis of the cells. To improve nutrition, tooth transplantation to recipient beds left to heal for 14 days was performed (two-stage technique). Cli

Infratemporal fossa infection with inferior alveolar nerve involvement.

Abscess of the infratemporal fossa is a rare complication which can be difficult to diagnose. It occurs mostly due to dental infection, tooth extraction, fractures and/or infections involving the maxillary sinus. This condition can be life threatening if not dealt with immediately. The maxillofacial surgeon must be aware of the symptoms and clinical findings of the infection of the infratemporal fossa in order to initiate the treatment as soon as possible. In this case report, a patient with an infratemporal fossa infection presenting with an unusual symptom of inferior alveolar nerve involvem

Digitally Fabricated Fixed Restoration for Immediate Replacement After Extraction and Splinting the Adjacent Mobile Teeth.

Objective: This study is aimed at detailing a technique for digitally fabricated fixed restoration for both immediate replacement after extraction and splinting of the adjacent mobile teeth. Clinical Considerations: Demand for a fixed restoration of the missing teeth in the mandibular anterior region is very common but sometimes problematic for dentists. It often happens that there is significant loosening of the remaining teeth adjacent to the missing tooth, which should be splinted. This case report describes a digitally fabricated fixed restoration for both replacing lost teeth and splintin
