Effect of daily repetitive transcranial magnetic stimulation for treatment of tinnitus: comparison of different stimulus frequencies.

UNLABELLED: We compared the effect of different frequencies of repetitive transcranial magnetic stimulation (rTMS) (1 Hz, 10 Hz, 25 Hz and sham (occipital, 1 Hz)), given daily over the left temporoparietal cortex for 2 weeks, on 66 patients with chronic tinnitus randomly divided into four treatment groups. Patients were assessed using the Tinnitus Handicap Inventory, self-ratings of symptoms and audiometric measures of residual inhibition before, immediately after 2 weeks' treatment and monthly thereafter for 4 consecutive months. RESULTS: There were no significant differences in basal measur…

Effects of repetitive transcranial magnetic stimulation (rTMS) on panic attacks induced by cholecystokinin-tetrapeptide (CCK-4).

Low-frequency (LF) rTMS shows beneficial effects in patients with depression and anxiety disorders. To explore its anxiolytic properties we investigated the effects of rTMS on experimentally induced panic attacks. Eleven healthy subjects underwent 1 Hz rTMS or sham rTMS over the right dorsolateral prefrontal cortex in a randomized cross-over protocol. Panic induction with 50 mug CCK-4 was carried out immediately after rTMS. Response to CCK-4 was assessed using the Acute Panic Inventory and the Panic Symptom Scale and measurements of heart rate, plasma ACTH and cortisol. All subjects reported a

One-Hz repetitive transcranial magnetic stimulation of the premotor cortex alters reciprocal inhibition in DYT1 dystonia.

Repetitive transcranial magnetic stimulation (rTMS) can produce long-lasting effects not only underneath the site of stimulation, but also at distant connected sites. This study aimed to assess how low frequency rTMS over the premotor area might affect abnormalities in spinal motor function in patients with generalised dystonia associated with the DYT1 gene mutation. We assessed reciprocal inhibition (RI) in a group of 8 manifesting carriers of the DYT1 gene (DYT1) and 10 healthy controls. All subjects then received 20 minutes of 1 Hz rTMS over the premotor area, and RI was assessed again. Bef…

Diaphragmatic silent period to transcranial magnetic cortical stimulation for assessing cortical motor control of the diaphragm.

This study was designed to determine whether a silent period could be elicited in the diaphragm electromyographic (EMG) activity by transcranial magnetic stimulation (TMS) of the motor cortex and, if so, to assess the influence of reflex or voluntary control of breathing on diaphragmatic cortical silent period (cSP). Diaphragmatic EMG activity was recorded in six healthy volunteers after motor cortex TMS triggered by the inspiratory flow peak and applied during forced inspiration (FI), voluntary hyperventilation (vHV) and reflex hyperventilation (rHV) to a CO(2) stimulus. Electrophysiological …

Repetitive transcranial magnetic stimulation of the human prefrontal cortex induces dopamine release in the caudate nucleus.

Dopamine is implicated in movement, learning, and motivation, and in illnesses such as Parkinson's disease, schizophrenia, and drug addiction. Little is known about the control of dopamine release in humans, but research in experimental animals suggests that the prefrontal cortex plays an important role in regulating the release of dopamine in subcortical structures. Here we used [(11)C]raclopride and positron emission tomography to measure changes in extracellular dopamine concentration in vivo after repetitive transcranial magnetic stimulation (rTMS) of the dorsolateral prefrontal cortex in …

Double-pulse transcranial magnetic stimulation over the frontal eye field facilitates triggering of memory-guided saccades.

The study investigated the influence of double-pulse transcranial magnetic stimulation (dTMS) on memory-guided saccade triggering. Double pulses with interstimulus intervals (ISIs) of 35, 50, 65 or 80 ms were applied over the right frontal eye field (FEF) and as control over the occipital cortex. A significant dTMS effect was found exclusively for contralateral saccades; latency of memory-guided saccades was reduced after FEF stimulation with an ISI of 50 ms compared to latency without stimulation. This effect proved to be specific for the ISI of 50 ms over the FEF because control stimulation …

[The new technique of studying cranial nerves VII, XI and XII: electroneurography by transcranial magnetic stimulation].

Transcranial magnetic stimulation (TMS) technique has been applied to the study of VII, XI and XII cranial nerves in 5 normal subjects, comparing the results with those obtained by conventional electrical stimulation. This technique allows us to study in a painless way the nerve function in all its pathway (motor cortex-motoneurone-muscle). In some cases, TMS yields potentials of a greater amplitude than those obtained by electrical stimulation, due to its capacity to penetrate deeply and stimulate all the fibers of the nerve.

Interhemispheric reorganization of motor hand function to the primary motor cortex predicted with functional magnetic resonance imaging and transcranial magnetic stimulation.

The objective of this study was presurgical assessment of reorganization of motor hand function in an 11-year-old girl with intractable epilepsy and a right-sided hemiplegia resulting from an extensive perinatal left hemispheric stroke. Prior to a left functional hemispherectomy, functional magnetic resonance imaging (MRI) showed that both nonparetic and paretic motor hand function predominantly activated the right primary motor cortex, whereas no activation was found in the left hemisphere. Transcranial magnetic stimulation of the right central area yielded responses in both the nonparetic an…

The more the merrier? Preliminary results regarding treatment duration and stimulation frequency of multisite repetitive transcranial magnetic stimulation in chronic tinnitus.

The current study examines two different protocols of repetitive transcranial magnetic stimulation (rTMS) targeting three key hubs of cortical tinnitus networks. Patients chose whether they preferred to undergo rTMS treatment for 10 or 20 sessions and were randomized to the different rTMS protocols thereafter. Ninety patients were enrolled in the study with a total dropout rate of 10%. Both rTMS protocols were well tolerated by the patients without any serious adverse events. Overall treatment effects were small both for 2 weeks and 4 weeks of treatment. There was no significant interaction ef…

1Hz vs. cTBS Transcranial Magnetic Stimulation to Augment Behavior Therapy for Tics: A Randomized Clinical Trial.

BACKGROUND: Response to Comprehensive Behavioral Intervention for Tics (CBIT), the first-line treatment for tic disorders, is suboptimal. We conducted Phase 1 of a two-phase, milestone-driven trial to determine if augmenting CBIT with repetitive transcranial magnetic stimulation (TMS) to supplementary motor area (SMA) could safely engage targeted sensorimotor circuitry and improve tic controllability. METHODS: In a 3-arm, double-blind, sham-controlled randomized trial ( NCT04578912 ), 50 youth (ages 12-21) with moderate-to-severe tics received 10 sessions of either 1Hz rTMS, continuous theta …

A modular system for multichannel transcranial magnetic stimulation: physical and physiological characterization of electric field profiles.

BACKGROUND: Transcranial Magnetic Stimulation (TMS) is an established non-invasive neuromodulation technique, with growing evidence suggesting that targeting multiple interconnected nodes may enable more selective network-level control. However, multisite stimulation remains constrained by the fixed field geometry and limited focality of conventional coils. OBJECTIVE: To introduce and characterize a modular multichannel transcranial magnetic stimulation (mTMS) array capable of electronically steering the induced electric field (E-field) without mechanical coil movement. METHODS: The array co…

Effects of transcranial magnetic stimulation on spatial memory: a systematic review across animal and human studies.

INTRODUCTION: Spatial memory relies on distributed hippocampal-cortical networks that are highly sensitive to modulation of excitability, connectivity, and synaptic plasticity. Transcranial magnetic stimulation (TMS) has emerged as a promising tool to experimentally probe and therapeutically modulate these networks, although its effects on spatial memory remain heterogeneous. METHODS: We conducted a PRISMA-guided systematic review synthesizing evidence from rodent and human studies examining the effects of TMS on spatial memory. A total of 35 studies (23 animal, 12 human) identified through s…

Efficacy of 5 × 5 accelerated versus conventional repetitive transcranial magnetic stimulation (rTMS) for treatment-resistant depression.

BACKGROUND: Accelerated repetitive transcranial magnetic stimulation (rTMS) is an effective treatment for Major Depressive Disorder (MDD), with an FDA-cleared protocol utilizing 10 sessions of prolonged intermittent Theta Burst Stimulation (piTBS) per day for five days. However, it remains unclear how many sessions and pulses are necessary to achieve benefit. METHODS: In a retrospective analysis of clinical data, we examined efficacy of five rTMS stimulation sessions per day for five days ("5 × 5") (N = 40) of either piTBS or individualized, electroencephalogram-based "resonant frequency" (RF…

Exploring the Role of Parietal Transcranial Magnetic Stimulation on Embodiment-Related Processes During Virtual Prosthesis Control.

Despite advancements in bionic technology, disrupted afferent and efferent pathways in individuals with limb loss may hinder prosthetic embodiment. Transcranial Magnetic Stimulation (TMS) has emerged as a promising tool for investigating and modulating sensorimotor processes, though its application in neurorehabilitation for individuals with limb differences remains limited. This pilot study explores whether TMS over parietal regions associated with the grasping-function and motor planning can enhance the sense of embodiment and influence motor behaviour. In this feasibility protocol, six heal…

Location and strength reliability of superficial target toward transcranial magnetic stimulation guided with effective functional connectivity.

BACKGROUND: Individualized transcranial magnetic stimulation (TMS) can be optimized using functional connectivity (FC). Compared to undirected FC, effective connectivity (eFC) reveals causal relationships between brain regions but lacks reliability evaluation for guiding TMS targets. NEW METHOD: This study investigated the test-retest reliability of eFC-guided superficial targets (i.e., location and strength) using functional MRI data from 29 healthy participants. We simulated clinical scenarios targeting four deep regions (i.e., the subgenual anterior cingulate cortex [sgACC], posterior cing…

Rewriting Reactivated Memories? A Systematic Review of Transcranial Magnetic Stimulation and Memory Reactivation and Reconsolidation Paradigms.

INTRODUCTION: Memory reconsolidation refers to the postreactivation restabilization process through which consolidated memories may become modifiable under specific boundary conditions. Transcranial magnetic stimulation (TMS) offers a causal method for perturbing cortical processing during this putative window of lability. However, whether human TMS studies truly support a reconsolidation-based interpretation, rather than broader postretrieval modulation, has not been systematically synthesized. MATERIALS AND METHODS: We conducted a systematic review of human studies examining TMS in relation…

Impaired interhemispheric inhibition in isolated REM sleep behavior disorder: first exploratory transcranial magnetic stimulation-based evidence.

STUDY OBJECTIVES: Isolated REM sleep Behavior Disorder (iRBD) is viewed as a prodromal stage of alpha-synuclein-related neurodegeneration. Transcranial Magnetic Stimulation (TMS) revealed early cortical excitability abnormalities in this condition. However, interhemispheric inhibitory mechanisms, measured through transcallosal inhibition, have not been previously investigated. METHODS: In this pilot study, we explored both intracortical and interhemispheric inhibition in 20 de novo right-handed iRBD patients compared with 20 right-handed healthy controls. Participants underwent cognitive test…
