PURPOSE OF REVIEW: Acute postsurgical pain after cardiac surgery is challenging to treat. Adverse effects related to the high dose opioids which have traditionally been used perioperatively in cardiac surgery have led to the adoption of alternative analgesic strategies. This review aims to highlight current evidence-based approaches to managing pain after cardiac surgery.
RECENT FINDINGS: Current evidence and international guidelines support the use of multimodal analgesics for managing perioperative pain after cardiac surgery. Regional anaesthesia in the form of fascial plane blocks, such as

Because homelessness assistance programs are designed to help families, it is important for policymakers and practitioners to understand how families experiencing homelessness make housing decisions, particularly when they decide not to use available services. This study explores those decisions using in-depth qualitative interviews with 80 families recruited in shelters across four sites approximately six months after they were assigned to one of four conditions (permanent housing subsidies, project-based transitional housing, community-based rapid re-housing, and usual care). Familiar neighb

OBJECTIVE: Health care transition from pediatric to adult primary care is understudied, and factors associated with successful transfer to adult care are not well understood.
METHODS: The current study surveyed a sample of patients (N = 110) of transition age (18 - 22, M age = 19.42, SD age = 1.16) who attended a pediatric primary care practice. Mixed methods were used to assess the prevalence of successful transfer to adult primary care, factors associated with successful transfer, as well as barriers, facilitators, and recommendations for improving transition support.
RESULTS: 45 % of the

INTRODUCTION: Transitional care of patients with complex uro-genital malformations (CUGM), including spina bifida, bladder exstrophy-epispadias complex, cloaca, congenital adrenal hyperplasia, and vaginal atresia, remains a worldwide challenge. These conditions are associated with lower urinary tract dysfunction (LUTD), long-term urinary morbidity, and significant psychosocial consequences. The aim of this study was to investigate how pediatric urologists in Italy address sexuality and fertility in adolescents and young adults with CUGM.

We conducted a narrative review of the Transitional Pain Service (TPS) to (1) synthesize current evidence on its effectiveness and (2) identify key prerequisites for future TPS implementation. Chronic post-surgical pain (CPSP) is a significant and potentially preventable postoperative complication following major surgery. Established at Toronto General Hospital in 2014, the TPS represents a novel multidisciplinary model designed to mitigate CPSP risk and reduce opioid dependence. A comprehensive search of OVID Medline, supplemented by manual hand-searching, was performed using terms related to

PURPOSE: We examined the ambulatory health care visit use of children with spina bifida, adults who transitioned to adult care and adults who continued to seek care in a pediatric setting.
MATERIALS AND METHODS: We evaluated use during a 1-year period of patients with spina bifida who visited any outpatient medical clinic within an integrated health care system. Patients were categorized as pediatric (younger than 18 years) or adult (age 18 or older). Adults were divided into those who did not fully transition to adult care and patients who fully transitioned (adult). Frequency and type of he

It is now expected that most individuals with congenital heart disease will survive to adulthood, including those with complex heart conditions. Maintaining lifelong medical care requires those with congenital heart disease to eventually transfer from pediatric to adult-oriented health care systems. Developing health care transition skills and gaining independence in managing one's own health care is imperative to this process and to ongoing medical and psychosocial success. This scientific statement reviews the recent evidence regarding transition and provides resources, components, and sugge

BACKGROUND: The German health care system maintains a strict division between inpatient and outpatient care, posing challenges for the continuity of care. Discharge management facilitates the transition from inpatient to outpatient settings. For psychiatric patients, sociotherapy is one transitional intervention that can be prescribed to inpatients at discharge and then provided in the outpatient setting. As a form of case management, it aims to motivate patients, coordinate care among providers, and ultimately reduce psychiatric hospital readmissions. However, the effectiveness of sociotherap

Chronic pain imposes a tremendous economic burden of up to US$635 billion per year in terms of direct costs (such as the costs of treatment) and indirect costs (such as lost productivity and time away from work). In addition, the initiation of opioids for pain is associated with a more than doubling of pharmacy and all-cause medical costs. The high costs of chronic pain are particularly relevant for anesthesiologists because surgery represents an inciting event that can lead to chronic pain and long-term opioid use. While the presence of risk factors and an individual patient's postoperative p

OBJECTIVE: Few studies have assessed Health-Related Quality of Life (HR-QoL) in adults following juvenile idiopathic arthritis, and none since the advent of biotherapies. The aim of our study is to assess the impact of juvenile idiopathic arthritis on quality of life in a large transitional cohort, evaluate which factors influence quality of life in juvenile idiopathic arthritis, and determine which questionnaire should be used in practice.
METHODS: All consecutive juvenile idiopathic arthritis patients followed during adulthood in a transitional care program were included. Demographical, cli…

BACKGROUND: Young men with Duchenne muscular dystrophy (DMD) live into adulthood and need specialized care. However, services for adults are fragmented. We know little about young men's experiences, their parents, and clinicians who support them as they transition to adult care.
OBJECTIVE: To explore the enablers and barriers of clinicians, young men, and parents as they transition from an adult DMD clinic within a pediatric hospital to an adult health facility.
METHODS: We conducted semi-structured, in-depth interviews with 16 participants (7 clinicians, 5 parents, 4 youth). We used an open

BACKGROUND: Adolescents and young adults (AYAs) with chronic conditions face a transfer, defined as an actual shift from paediatric to adult-oriented health care. Transition competence as the self-perceived knowledge, skills and abilities regarding the transition process was considered extremely useful.
AIM: This study was designed to investigate the impact of transition competence before and after the transfer on disease-specific quality of life (QoL) and health care satisfaction of AYAs with diabetes.
RESULTS: In total, a sample of N = 90 AYAs with diabetes self-reported their transition c

BACKGROUND: While most people express a preference to be cared for and die at home, not all people have access to a safe or stable home. Government policies often position home death as a proxy measure of quality end-of-life care, yet structurally vulnerable populations, including those who are precariously housed, homeless, or living in supportive or transitional housing, are frequently excluded from this possibility. This scoping review aims to summarize the current literature on the factors that facilitate or prevent palliative care at ‘home’ for structurally vulnerable adults. METHODS: We

BACKGROUND: The transition of health care from Pediatric to Adult providers for adolescents and young adults with chronic disease is associated with poor outcomes. Despite the importance of this transition, over 80% of these patients do not receive the services necessary to transition to Adult health care. In 2018, we initiated a transition clinic structure, integrating an Internal Medicine - Pediatrics trained Adult Rheumatologist in a Pediatric Rheumatology clinic to guide this transition. Our goal was to improve transition outcomes. We report the methods of this clinic and its preliminary o
