Moral injury in healthcare workers: An intervention to mitigate distress.

BackgroundThe psychological distress of individuals working in high-stakes, value-based environments (e.g., hospitals, nursing homes, and community health centers) is receiving growing recognition as moral injury (MI). While MI and other mental health challenges, like PTSD and burnout, have similarities and overlaps in symptoms, identification of these differences is essential, as the distinct needs of MI treatment require professionals to address its unique characteristics. Further, MI creates dual threats to healthcare operations, affecting workforce stability and individual psychological im

Interrater reliability of craniosacral rate measurements and their relationship with subjects' and examiners' heart and respiratory rate measurements.

BACKGROUND AND PURPOSE: The evaluation of craniosacral motion is an approach used by physical therapists and other health professionals to assess the causes of pain and dysfunction, but evidence for the existence of this motion is lacking and the reproducibility of the results of this palpatory technique has not been studied. This study examined the interexaminer reliability of craniosacral rate and the relationships among craniosacral rate and subjects' and examiners' heart and respiratory rates. SUBJECTS: Participants were 12 children and adults with histories of physical trauma, surgery, o

PATHway to success: Implementation of a multiprofessional acute trauma health care team decreased length of stay and cost in patients with neurological injury requiring tracheostomy.

BACKGROUND: The aim of this study was to determine whether the implementation of a dedicated multiprofessional acute trauma health care (mPATH) team would decrease length of stay without adversely impacting outcomes of patients with severe traumatic brain and spinal cord injuries. The mPATH team was comprised of a physical, occupational, speech, and respiratory therapist, nurse navigator, social worker, advanced care provider, and physician who performed rounds on the subset of trauma patients with these injuries from the intensive care unit to discharge. METHODS: Following the formation and

Assailants' sexual dysfunction during rape: prevalence and relationship to genital trauma in female victims.

BACKGROUND: Partial or complete failure to maintain an erection sufficient for coitus is known to occur in a proportion of sexual assailants during the rape episode. OBJECTIVE: The purpose of this study was to determine whether the presence of coercion, physical violence, and genital injury associated with sexual assaults is influenced by the assailant's erectile impotence. METHODS: This prospective, observational study evaluated consecutive female patients presenting to a free-standing urban sexual assault clinic during a 3-year study period. Sexual assault victims presenting directly to fo

Post-traumatic stress disorder and depression as predictors of peace of mind among healthcare workers working with war victims.

Healthcare professionals in Iraq are exposed to war-related stressors that may undermine psychological well-being. This study examined post-traumatic stress disorder (PTSD) and depression and their association with peace of mind (PoM). In a cross-sectional survey, 174 physicians, nurses, pharmacists and allied health workers from multiple Iraqi regions completed an online questionnaire including demographics and validated Arabic measures of PTSD, depression (BDI) and PoM. Reliability was checked using Cronbach's alpha, and analyses used descriptive statistics, t-tests, Spearman correlations an

Spontaneous intracranial hypotension: a case study.

PURPOSE: To present an illustrative case study of a patient with spontaneous intracranial hypotension (SIH) and to increase awareness of this condition among nurse practitioners (NPs). DATA SOURCES: A literature search was conducted, and deidentified patient information forms the basis of this presentation. The authors' experience and appropriate images enhance the presentation of the case study. CONCLUSIONS: SIH is a condition that typically occurs without a traumatic event, although it can be associated with minor trauma. It occurs when cerebrospinal fluid (CSF) leaks through a focal weakn

Ehlers-Danlos syndrome and wound healing: injury in a collagen disorder.

Ehlers-Danlos syndrome (EDS) is a rare familial disorder affecting the production, quality and strength of collagen. It presents in many forms, and many individuals experience hypermobile joints and soft, elastic skin. When injury occurs, EDS can result in severe bruising, the development of wide open wounds and delayed healing (Babak et al, 2003). In the case discussed, Lilly, a 10-year-old girl, presented with a wide trauma injury to her lower leg. Healing was influenced by the poor collagen production associated with EDS, and the presence of a high bacterial burden that resulted in infectio

Shaken baby syndrome: identification, intervention, and prevention.

More than 1 million children are severely abused annually. Shaken baby syndrome (SBS), one example of physical abuse, is a leading cause of morbidity and mortality in infants. There is a national annual incidence in the United States of 750 to 3,750 cases of SBS. Shaken baby syndrome is defined as vigorous manual shaking of an infant who is being held by the extremities or shoulders, leading to whiplash-induced intracranial and intraocular bleeding with no external signs of head trauma. Shaken baby syndrome should be suspected in infants with a wide spectrum of clinical signs and symptoms. The

Treatment strategies in shock: use of oxygen transport measurements.

Shock has traditionally been categorized according to its cause. Shock can result from hemorrhage, primary cardiac failure, central nervous system failure, trauma, or sepsis. Therapeutic principles have been developed for each etiologic type. End points for such therapy have included optimization of pulmonary capillary wedge pressure, cardiac output, blood pressure, and urine output. Recent investigators agree that the common denominator in each of the shock syndromes is a reduction in the amount of oxygen consumed by the cell. The logical therapeutic approach would be to increase oxygen deliv

Electrotherapy in Lactation Care.

Electrotherapy in Lactation CareBreastfeeding offers significant health benefits but is often discontinued early due to inflammatory conditions of the lactating breast and nipple trauma. Common issues such as mastitis, engorgement, and nipple damage contribute to pain, infection, and reduced milk production, leading to early cessation. Effective management includes optimal positioning, strategies to maintain breastfeeding, and wound care. Emerging electrotherapies, such as Photobiomodulation Therapy and Therapeutic Ultrasound, may offer promising, non-invasive treatments to support lactation.

Clinicians' Experiences With Law Enforcement in a Pediatric Emergency Department.

BACKGROUND AND OBJECTIVES: Law enforcement officers (LEOs) are frequently present in emergency departments, where their roles may include accompanying patients, investigating injuries, or responding to safety concerns. Existing research on LEO presence in health care has focused on adult populations, with limited attention to pediatric settings. Pediatric emergency departments (PEDs) represent a distinct clinical environment in which developmental vulnerability, caregiver involvement, and legal considerations related to minors may shape interactions. Despite these differences, little is known
