Use of recombinant purified protein derivative (PPD) antigens as specific skin test for tuberculosis.

BACKGROUND & OBJECTIVES: Purified protein derivative (PPD) is currently the only available skin test reagent used worldwide for the diagnosis of tuberculosis (TB). The aim of this study was to develop a Mycobacterium tuberculosis specific skin test reagent, without false positive results due to Bacillus Calmette-Guerin (BCG) vaccination using recombinant antigens. METHODS: Proteins in PPD IC-65 were analyzed by tandem mass spectrometry and compared to proteins in M. tuberculosis culture filtrate; 54 proteins were found in common. Top candidates MPT64, ESAT 6, and CFP 10 were overexpressed in

[Hormonal shifts in adolescents in the initial period of infection with tuberculosis].

In 78 adolescents aged 13-17 years, the functions of the pituitary body, adrenal and thyroid glands and pancreas were examined by means of radioimmunoassay. The controls were represented by 31 subjects with non-infected tuberculosis; in 47 adolescents, tuberculin test was seen to turn positive. The initial period of the tuberculosis infection in adolescents is marked by the following changes: inhibited pituitary function (other than adrenocorticotropic hormone); higher adrenal activity, elevated cortisol levels, altered (predominantly depressed) thyroidal function; and increased pancreas funct…

[Screening for tuberculosis].

Rapid identification using bacteriological methods and adequate treatment of active tuberculosis cases are the most important objective of any tuberculosis activity but, in order to eliminate the disease, another important component of tuberculosis control is to reduce the vast reservoir of latent tuberculosis infections. Tuberculin skin test and interferon-gamma release assays are designed to identify immune response against mycobacterial antigens. Both tests are accurate to detect latent but not active forms of tuberculosis. Interferon-gamma release assays have higher specificity than tuberc

[New tools for diagnosis of latent tuberculosis infection].

Until recently, the tuberculin skin test was the only test for detecting latent tuberculosis (TB) infection with a lack of specificity and reliability. Since 2005, new tests are available. Those tests are based on interferon gamma released (IGRAs) by T lymphocytes after mycobacterium tuberculosis antigen specific exposure. The new tests are able to modify our understanding of natural history of tuberculosis. Longitudinal studies are needed to define the predictive value of IGRAs.

5-Year Follow-up of Active Tuberculosis Development From Latent Infection in Rural China.

A 5-year follow-up study found declining disease development rates from latent tuberculosis infection. In the latter 3 years, the incidence, per 100 person-years, was 0.26 for interferon-γ release assay positives and 0.19 for tuberculin skin test positives (P = .28). In addition, all round have been checked.

[New tests for diagnosis of latent tubercolosis].

Two new tests (QuantiFERON-TB and T-SPOT.TB) for diagnosis of latent tuberculosis are on the market. They measure the release of interferon-gamma in whole blood in response to stimulation by PDD. They offer a more accurate approach than tuberculin skin test for identification of individuals with latent tuberculosis infection.

Clinical evaluation of QuantiFERON TB-2G test for immunocompromised patients.

The usefulness of the tuberculin skin test (TST) and the QuantiFERON TB-2G (QFT-TB) test were compared in immunocompromised patients. The subjects consisted of 252 immunocompromised patients who were clinically suspected of tuberculosis (TB) infection between April 2005 and December 2006. Regarding the underlying diseases, 74 subjects had malignant diseases, 72 were undergoing immunosuppressive treatment, 52 had diabetes mellitus, 50 had chronic renal failure and four had HIV infection. While the positive rate of the QFT-TB test for the diagnosis of TB infection (TB disease or latent TB infect

Humoral immune response in human tuberculosis: immunoglobulins G, A, and M directed against the purified P32 protein antigen of Mycobacterium bovis bacillus Calmette-Guérin.

The P32 protein antigen of Mycobacterium bovis BCG, identified as antigen 85A in the BCG reference system, was used to investigate the humoral immune response in human tuberculosis (TB). Immunoglobulin G (IgG), IgA, and IgM directed against P32 were measured by an enzyme-linked immunosorbent assay. Mean IgG and IgA antibody levels differed significantly (P less than 0.001) between active-TB patients (50 untreated and 52 treated) and healthy control subjects (111 unvaccinated tuberculin negative, 38 unvaccinated tuberculin positive, and 72 recently BCG vaccinated). Mean IgG antibody levels, but

First results on small ruminant brucellosis and tuberculosis and caprine arthritis-encephalitis in El Salvador.

This paper reports a first-time study performed in El Salvador on the presence or absence of antibodies to three important animal diseases in small ruminants. The work was conducted in the west and central departments of the country, selecting 42 and 43 cantons with an existing sheep and goat population, respectively. Serum samples were collected from 396 sheep and 335 goats and tested for seropositivity to Brucella (B.) spp. The specimens from goats were also tested for antibodies to caprine arthritis-encephalitis (CAE) virus. Four (1 %) sheep and none of the goats were seropositive by Rose B

Mycobacterium tuberculosis infection after travel to or contact with visitors from countries with a high prevalence of tuberculosis.

To assess whether there is increased risk of tuberculous infection in children who traveled to or had a household visitor from a country having a high prevalence of tuberculosis, we conducted a case-control study. Children younger than 6 yr of age who had a tuberculin skin test read at public health clinics in areas of California that have a high prevalence of tuberculosis were enrolled. Of the 953 children who had a skin test read, 72 (7.6%) had a positive reaction. By multiple logistic regression analysis, children who had traveled in the 12 mo before the skin test were 3.9 times more likely

Development and evaluation of an enzyme-linked immunosorbent assay for use in the detection of bovine tuberculosis in cattle.

As a consequence of continued spillover of Mycobacterium bovis into cattle from wildlife reservoirs and increased globalization of cattle trade with associated transmission risks, new approaches such as vaccination and novel testing algorithms are seriously being considered by regulatory agencies for the control of bovine tuberculosis. Serologic tests offer opportunities for identification of M. bovis-infected animals not afforded by current diagnostic techniques. The present study describes assay development and field assessment of a new commercial enzyme-linked immunosorbent assay (ELISA) th

Does size matter? Utility of size of tuberculin reactions for the diagnosis of mycobacterial disease.

It is a common belief that larger tuberculin reactions are more serious, and more likely to indicate patients with active tuberculosis (TB) or at high risk of disease in the future. Among 182 close contacts, and 502 patients suspected of possible active TB, 529 underwent tuberculin skin testing (TST) and 605 had a chest radiograph. Final diagnoses, based on all available clinical, microbiological, histological, and radiographic information, were active TB, 68; inactive TB, 274; nontuberculous mycobacterial disease, 14; conditions associated with anergy, 36; no detectable abnormality (except a

Extremely extended Fournier's gangrene.

We describe a 19-year-old Japanese man with severe extensive necrotizing fasciitis that started as Fournier's gangrene to involve the external genitalia, thigh and lower abdomen. High creatine phosphokinase, transient immunosuppression (reduced serum IgG level and negative tuberculin skin test reaction) and disseminated intravascular coagulation occurred during the necrotizing fasciitis.

Contacts of tuberculosis patients in high-incidence countries.

The risk of acquiring infection with Mycobacterium tuberculosis correlates with duration of exposure to an infectious source of tuberculosis. Contact identification is therefore a comparatively high-yield activity. However, in resource-poor settings tuberculin is rarely available, and even where it is available, non-specific cross-reactions to tuberculin resulting from BCG vaccination complicate the interpretation of tuberculin test results. The identification of a putative infection with M. tuberculosis in a contact must result in intervention. Excluding active tuberculosis is mandatory befor…

[Should France continue to use BCG?].

In the context of the decrease of tuberculosis incidence, a re-assessment of the French BCG vaccination policy has been undertaken. The synt … ion and primovaccination, it was decided to discontinue the policy of re-vaccination and of routine tuberculin testing. A multidisciplinary expertise aimed at studying the relevancy, the feasibility and the acceptability of various options of reducing the target-population of primovaccination is on-going.
