Early recovery and stabilisation with instrumentation in anterior cervical spine tuberculosis.

BACKGROUND: Tuberculous infection causes destruction, caseation, and necrosis of cervical vertebrae or may present as an abscess. Complete recovery of neurological status is the rule after anterior surgical decompression and fusion, even in cases with complete paraplegia or tetraplegia. Neurological impairment and spinal deformity are the major concerns with spinal tuberculosis. Absolute nonoperative treatment was offered in pre antibiotic era. Since last decade great advances in terms of operative options drastically changed the scenario in management of caries spine and indications for surge

Experiences in management of Pott's paraplegia and paraparesis in medical wards of Usmanu Danfodiyo University Teaching Hospital, Sokoto, Nigeria.

BACKGROUND: Pott's disease refers to spinal tuberculosis, which commonly leads to a gradual onset of neurological deficit that are difficult to diagnose at the early stage. METHOD: The study was a combination of retrospective and prospective study. Information was obtained using a predesigned protocol. Diagnosis was based on clinical features and plain spinal x-ray. Anti-tuberculous therapy was instituted in all patients for a minimum of 18 months. Monitoring of neurological status was done weekly. RESULTS: There were 92 patients made up of 71 males (77.2%) and 21 females (22.8%) with a male

Central Nervous System Tuberculosis: A Case of Tuberculous Myelitis.

Tuberculous myelitis is rare in central nervous system (CNS) tuberculosis. Misdiagnosis initially may lead to poor outcomes; empirical antituberculosis treatment can improve symptoms. Early diagnosis via cerebrospinal fluid (CSF) and spinal MRI, plus timely treatment, is vital-clinical awareness needs enhancement.

The many faces of extrapulmonary tuberculosis: A case series.

Extrapulmonary tuberculosis (EPTB) is the clinical entity that is gaining more recognition. Still, it may emerge in various ways, making it difficult to diagnose. Even though pulmonary tuberculosis is the most common variety, EPTB may affect many organ systems and often mimics other diseases, therefore postponing diagnosis, and treatment. Five rare types of EPTB, including testicular tuberculosis, esophageal tuberculosis, psoas abscess with spinal involvement, tuberculous mastitis, and parotid gland tuberculosis, are the main subjects of this case series. All patients who were HIV-negative wer

Craniovertebral junction Pott's disease.

Pott's disease of the craniovertebral junction is extremely rare. The authors studied the immediate and long-term outcome after transoral decompression, occipitocervical stabilization with fusion and antitubercular therapy (ATT) in patients who had neurological deficits due to craniovertebral junction (CVJ) tuberculosis. This is a retrospective study of the management and outcome in 10 consecutive patients in whom features of spinal cord (with or without) compression were observed, and CVJ tuberculosis was diagnosed in two different neurological departments between 1990 and 2002. They ranged i

[The cold tuberculous abscess. A forgotten entity? Considerations based on an autopsy case].

UNLABELLED: We present the clinical case of a 28-year old male with pulmonary and extrapulmonary tuberculosis in absence of HIV infection; who also presented a left iliac psoas abscess of torpid evolution simulating an strangulated hernia and died of a sepsis due to colonic perforations by anaerobics and E. histolytica. MATERIAL AND METHODS: Histological sections of the autopsy were studied with hematoxilin-eosin, Gram and Ziehl-Neelsen stains. RESULTS: Autopsy study showed bilateral cavitated tuberculosis, Pott disease of thoracic spine and a cold tuberculous abscess on both psoas muscle an

Spinal cord glioblastoma induced by radiation therapy of nasopharyngeal rhabdomyosarcoma with MRI findings: case report.

Radiation-induced spinal cord gliomas are extremely rare. Since the first case was reported in 1980, only six additional cases have been reported.; The radiation-induced gliomas were related to the treatment of Hodgkin's lymphoma, thyroid cancer, and medullomyoblastoma, and to multiple chest fluoroscopic examinations in pulmonary tuberculosis patient. We report a case of radiation-induced spinal cord glioblastoma developed in a 17-year-old girl after a 13-year latency period following radiotherapy for nasopharyngeal rhabdomyosarcoma. MRI findings of our case are described.

Mediastinal mass causing spinal cord compression.

A 4-year-old Cambodian male presented to the emergency room with 2 weeks of gradually increasing leg weakness until he could no longer stand. He was also reported to have a deformity on his back, intermittent fevers and cough. His physical exam was notable for a 2 cm × 1 cm bony protrusion at his T4 vertebrae, and 2/5 strength and positive Babinski reflexes in his lower extremities. A chest x-ray showed a 3.2 cm × 2.9 cm mass in the middle mediastinum extending to the posterior mediastinum. A purified protein derivative test was positive. A computed tomography scan showed findings consistent w

Tuberculosis of the cervicodorsal junction.

Sixteen patients had paraplegia due to tuberculosis of the cervicodorsal junction. Spinal decompression was performed through an extended lower cervical approach, and a humeral allograft was used to reconstruct the anterior column. All patients improved neurologically with satisfactory correction of the deformity and successful incorporation of the allograft.

[What has tuberculosis medical care contributed to modern medicine in Japan].

About 40 years ago, we had a huge number of tuberculosis patients which were about three to five millions. And a great deal of effort has been given in order to conquer tuberculosis. The philosophy and the technology used in there efforts are thought to have made considerable contributions to the development of modern medicine in other fields in Japan. The controlled clinical trial used to test new antituberculous agents has been a basis for a modern judgment on the effectiveness of any new drugs and it has brought good clinical practices. Scientific procedures for diagnosis, bronchoscopy and

A Case of Posttraumatic Pott's Disease.

Tuberculosis infection, which is caused by the bacterium Mycobacterium tuberculosis (Mtb), most commonly manifests in patients with respiratory systems. However, it can also colonize other tissues including skeletal. In our case, a 77-year-old Caucasian male presented to the emergency department following a rollover motor vehicle collision with chief complaints of neck and lower back pain. After clinical improvement and a preliminary negative workup, the patient was deemed stable for discharge. Four months later, the patient was subsequently admitted for worsening back pain with workup suspici

Primary disseminated MDR-TB in a Bangladeshi man: a silent and emerging clinical problem for clinicians.

A 26 years old Bangladeshi man experienced sudden gross haematuria for 10 days when he was working in Italy in June'07 that was resolved spontaneously. Six months' later he again developed haematuria and was admitted into an Italian hospital for its evaluation and management. In the mean time, he developed low grade fever, cough, back pain, spinal angulations, walking difficulty. His X-ray chest postero-anterior view revealed cavitary lesions in upper zones of both lung fields. Intravenous Urography (IVU) was done which revealed hydronephrosis and hydroureter of left kidney. Intramedullary tuberculomas of the spinal cord: case report and review.

Intramedullary spinal tuberculomas are a rare cause of spinal cord compression. We report a recent case and review 17 cases described in the literature since 1960. Intramedullary spinal tuberculomas occurred in relatively young patients (mean age, 28.6 years) and more frequently in women (63% of cases) than in men. The commonest symptoms were those of subacute spinal cord compression (mean duration, 2.3 months) with progressive lower-limb weakness (94%), paresthesia, and bowel and bladder dysfunction. The major physical finding was paraplegia, either spastic (61%) or flaccid (33%). The majorit
