False-positive galactomannan assay in broncho-alveolar lavage after enteral nutrition solution inhalation: a case report.

Introduction. Diagnosis of invasive aspergillosis is challenging and the gold standard for definite diagnosis remains histopathological tissue examination. However, invasive procedures such as lung biopsy are often not feasible in critically ill patients. The detection of fungal cell wall components like Aspergillus galactomannan in broncho-alveolar lavage remains a key component of the diagnostic procedure. False-positive of the Aspergillus galactomannan assay is not frequent. Case presentation. We report a case of positive galactomannan in broncho-alveolar lavage fluid after enteral nutritio

Corrective tibial osteotomy for genu recurvatum by callus distraction using an external fixator.

Genu recurvatum due to an osseous abnormality is a condition that rarely requires surgical intervention. When necessary, the operative solution usually considered is an opening wedge osteotomy using bone graft and plate fixation. The donor site (usually the iliac crest) has a well reported morbidity with a further procedure being required to remove the plate. We report two cases treated by a hemicorticotomy and callus distraction (hemicallotasis) of the tibia using an external fixator. Correction was performed under supervision as an out-patient. Hospital stay was short, the morbidity of the d

Thoracoscopic Thymectomy with Tangential Partial Resection of the Innominate Vein.

We herein report a case in which thoracoscopic thymectomy with partial resection of the brachiocephalic vein was performed using a subxiphoid approach. We suggest the usefulness of the subxiphoid approach in thymectomy. We could extend the indications of thoracoscopic thymectomy for invasive tumors. Thoracoscopic thymectomy with the combined resection of part of the brachiocephalic vein was possible. The pathological diagnosis of the tumor was diffuse large cell B-cell lymphoma. Since the postoperative complications were minimal, postoperative adjuvant chemotherapy could be initiated without d

Diagnosis of abdominal tuberculosis by transabdominal ultrasound-guided needle biopsy.

Laparotomy and laparoscopy have been used for biopsy of pelvic masses in the differential diagnosis between abdominal tuberculosis and ovarian cancer. We suggest the use of transabdominal needle biopsy under ultrasound guidance in such cases. Two women developed abdominal distention, one of whom had been receiving medical treatment for known tuberculosis. In both cases, ultrasonography showed a pelvic mass and ascites associated with high levels of CA 125. A transabdominal ultrasonographically-guided biopsy of the masses with a trucut needle established their tuberculous origin. The transabdom

A Modified Technique for Transverse Aortic Constriction in Mice.

Transverse aortic constriction (TAC) is a frequently used surgery in research regarding heart failure and cardiac hypertrophy based on the formation of pressure overload in mouse models. The main challenge of this procedure is to clearly visualize the transverse aortic arch and precisely band the target vessel. Classical approaches perform a partial thoracotomy to expose the transverse aortic arch. However, it is an open-chest model that causes a rather large surgical trauma and requires a ventilator during the surgery. To prevent unnecessary trauma and simplify the operating procedure, the ao

Continuous retroflexion cystoscopy during prostate cryoablation.

During cryotherapy of the prostate, identification of probe tips that are misplaced into the bladder is often not readily apparent via transrectal ultrasonography. Ultimately, cystoscopy is needed to ensure this has not occurred before initiating a freeze. We describe the innovative use of continuous flexible cystoscopy in retroflexion for visualization of the bladder neck during insertion and manipulation of cryoprobes. Using this technique, it is possible to identify probes errantly placed into the bladder lumen in real time, thereby facilitating safe and accurate placement and eliminating a

Vicarious Contrast Medium Excretion Mimicking Acute Hemorrhage: Don't Call the Interventional Radiologist!

Vicarious contrast medium excretion (VCME) is an uncommon phenomenon in which iodinated contrast is eliminated through non-renal pathways, predominantly via the hepatobiliary system. Although benign, it may closely mimic active gastrointestinal hemorrhage on follow-up computed tomography (CT), potentially leading to unnecessary invasive procedures. We report the case of a 28-year-old woman with acute leukemia complicated by methotrexate-induced acute pancreatitis and massive retroperitoneal hemorrhage. Contrast-enhanced CT demonstrated findings suspicious for active arterial bleeding originati

Optic disc melanocytoma with disc oedema and juxtapapillary haemorrhage: a case report.

BACKGROUND: Optic disc melanocytoma is a benign, pigmented tumour of the optic nerve head that is generally stable and asymptomatic. However, in rare cases, these tumours may be complicated by vascular changes and optic disc oedema, mimicking malignant optic disc lesions, resulting in unnecessary invasive interventions. Herein, we present a case of such a tumour presenting with disc oedema and juxtapapillary haemorrhage. CASE PRESENTATION: A 36-year-old woman presented with a 2-week history of blurred vision and floaters in the right eye. Fundus examination revealed a densely-pigmented optic

Explainable AI Radiomics in Prostate Cancer Aggressiveness Prediction using different quantitative Diffusion MRI models.

Prostate cancer (PCa) is one of the most frequently diagnosed cancers in men with the average age of diagnosis at 66 years. Accuracy in the early characterization of PCa is the major unmet need in disease management in order to stratify patients with indolent disease or patients with high risk for aggressive disease at an early stage. To this end, a retrospective collection of 202 histopathologically proven PCa patients was explored through quantitative diffusion MRI modelling radiomics to automatically classify Gleason score (GS) between GS<7 and GS≥7 aiming to reduce unnecessary biopsy. Robotic-assisted native pyeloureterostomy with indocyanine green, after kidney transplantation.

INTRODUCTION: Postoperative ureteral strictures and vesicoureteral reflux after ureteroneocystostomy for kidney transplant can be managed by endoscopic procedures like balloon dilation and endoscopic injections. Complicated/recurrent cases, however, are usually managed by reconstructive surgery. We hereby highlight our technique of robotic-assisted native pyeloureterostomy with indocyanine green (ICG). MATERIALS AND SURGICAL TECHNIQUE: A 57-year-old woman, diagnosed with grade 4 vesicoureteral reflux on her transplanted kidney, was considered a candidate for ureteral reimplantation. After an

Clinical diagnostic predictive score for Meckel diverticulum.

BACKGROUND/PURPOSE: Meckel diverticulum (MD) is present in 2% of the population. Many practitioner feel the diagnosis relies upon technetium-99 m pertechnetate scintigraphy. When negative, patients undergo additional invasive procedures delaying definitive therapy. This study aims to identify features of bleeding MD and generate a risk score, which could preclude unnecessary testing and facilitate earlier operation. METHODS: All patients <18-years-old who presented with hematochezia from 2005 to 2015 were identified. MD diagnosis was based on histopathology of operative tissue. Controls were

Cutaneous Sinus Tracts of Odontogenic Origin: Two Case Reports.

BACKGROUND: Cutaneous odontogenic fistulas or sinus tracts are frequently misdiagnosed and incorrectly treated, leading to unnecessary procedures and patient suffering. An understanding of the draining of cutaneous sinus tracts will lead to more appropriate treatment. Most cases respond to conservative, nonsurgical root canal therapy. Our objective is to report 2 cases of cutaneous sinus tract secondary to chronic periapical dental infection that were recently observed at our hospital. METHODS: We present 2 cases of recurrent suppurative facial lesions that were initially misdiagnosed and tre…

Para-iliac actinomycetoma presenting as sarcoma, a late complication of appendicitis: A case report.

INTRODUCTION: Actinomycosis is known to mimic several types of neoplasms, leading to morbid surgical interventions. PRESENTATION OF CASE: We report the particular case of an extensive right para-iliac actinomycetoma presenting as a sarcoma, which to our knowledge has not yet been described in the literature, in a patient with previous ruptured appendicitis. Thanks to the collaborative work between the orthopedic and general surgeons, pathologist and microbiologist, the diagnosis of actinomycosis was made pre-operatively, saving this 15-year-old patient from a tumor resection protocol. Radiation dose from medical imaging: a primer for emergency physicians.

INTRODUCTION: Medical imaging now accounts for most of the US population's exposure to ionizing radiation. A substantial proportion of this medical imaging is ordered in the emergency setting. We aim to provide a general overview of radiation dose from medical imaging with a focus on computed tomography, as well as a literature review of recent efforts to decrease unnecessary radiation exposure to patients in the emergency department setting. METHODS: We conducted a literature review through calendar year 2010 for all published articles pertaining to the emergency department and radiation exp

CSF TRAP: a procedure to improve laboratory testing with myelography.

CSF TRAP (Transport and Rapid Accessioning for Additional Procedures) is a procedure that provides storage of and rapid access to cerebrospinal fluid (CSF) specimens and allows clinicians to review initial findings before ordering low-yield CSF studies. The cost-effectiveness of routinely using the CSF TRAP procedure with myelography is examined in a study group of 819 patients, 74% with disc diseases, spinal stenosis, spondylolisthesis, or pain syndromes, 10% with cancer, and 16% with neuropathies and miscellaneous conditions. Routine studies on CSF obtained during myelography provided little

Diagnosis and Management of Sarcoidosis-like Reaction in Adjuvant Immunotherapy: A Comprehensive Review and Clinical Implications.

Immune checkpoint inhibitors (ICIs) have transformed oncologic care and are increasingly used as adjuvant therapy to reduce the risk of recurrence. However, this shift has introduced immune-related adverse events (irAEs) to patients who may otherwise be clinically disease-free after definitive therapy. Sarcoidosis-like reaction (SLR) is an uncommon but important irAE characterized by non-necrotizing granulomatous inflammation. In the adjuvant setting, SLR is uniquely consequential because it can closely mimic recurrent malignancy on surveillance imaging and thereby prompt unnecessary diagnosti
