Transitional cell carcinoma of the urethra in men after radical cystectomy for bladder carcinoma have an incidence which varies from 4 to 18%. The Authors present their experience of 6 cases (6/196; 3.06%) analysing the characteristics of the urethral neoplasm and confirming the systematic cystourethrectomy when there is a urethra prostatic and bladder neck infiltration or when there is a urethral ca in situ. It also underlines the role of the clinical, radiological, cytological and urethro-endoscopical follow-up in patients who have undergone a cystoprostatovesciculectomy.

We report a case of adenocarcinoma within a female urethral diverticulum. The patient was a 54-year-old woman, with a three-month history of dysuria. On physical examination a soft, round mass was palpable in the anterior vaginal wall. Excretory urography demonstrated elevation of the bladder floor. Panendoscopy demonstrated a diverticulum at the 7 o'clock position in proximal urethra. A papillary mass confined to the diverticulum was identified and biopsied. A case of synchronous kidney and pancreas transplantation in which a whole pancreas graft had been drained to the urinary bladder is discussed. On the 137th postoperative day, the patient presented with symptomatic balanitis and urethritis. Documented enzymatic activation of the trypsinogen/chymotrypsinogen, which is not present in symptom-free control patients, was thus thought to be responsible for these symptoms. Urethral and vaginal sensibility has been tested in 90 patients with urinary incontinence by means of urethral catheter and vaginal electrodes. Monophasic rectangular impulses with a frequency of 2 Hz, a duration of 2 msec and a variable intensity were used. A congenital giant diverticulum of the posterior urethra in a 4-year-old boy is reported. The theories on development of congenital urethral diverticula are considered and the importance of the clinical, radiological, endoscopical and histological examination for the differential diagnosis of the acquired diverticula and enlarged utricle is emphasised. In the majority of diverticula transurethral unroofing is most appropriate. The larger and symptomatic diverticula require open excision and eventually urethral reconstruction. Small asymptomatic diverticula may not require any treatment.

The response of the urethra in patients with chronic neurogenic bladders to autonomic stimulation was investigated by urethral pressure profile. A consistent supersensitivity to alpha-adrenergic stimulation was documented. Experiments with various autonomic blocking drugs confirmed that this response is alpha-adrenergically mediated via the short neuron system. The results appear to offer pharmacological evidence for the recently evolving intricate innervation pattern of the urethra including its distal portion, where the alpha-adrenergic system is believed to be important.
