A depot preparation of Estriol (Ortho Gynest D; 3.5 mg Estriol, intravaginal) was applied for 3 months to 135 postmenopausal women with urinary incontinence. Before and after treatment the symptoms of incontinence were scaled by the Gaudenz questionnaire. 8 symptoms were tested for statistical significance. Patients with proven stress incontinence were subjected to surgical treatment which combined both sling and Stamey-Pereyra techniques. A 3-4 cm long and 1,5-2 cm wide sling of Bulgarian polyamide mesh was placed under the urethrovesical junction using a small vaginal incision. Two long polyamide sutures were tied to either end of the mesh and were retrieved from the vagina to the abdomen by Pereyra needle inserted through two small suprapubic incisions and guided by the vaginal forefinger. The purpose of this study was to investigate risk factors for stress urinary incontinence. Using a case-control method, 140 women with and without stress urinary incontinence were recruited from three private physician practices and interviewed by telephone. The final sample included 94 cases and 46 controls, with a mean age of 51.5 and 54.3 years, respectively. In this study we present our experience from the application of the bladder neck suspension as modified by Raz on 37 patients. All patients had severe genuine stress incontinence. Twenty of them had undergone previous surgery for the same problem, and 16 patients had had a previous hysterectomy. In addition 15 patients had a small cystocele. Nineteen cases of feline congenital urinary incontinence (10 cats with ureteral ectopia and nine with incompetence of the urethral sphincter mechanism) are reviewed. The 10 cats with ureteral ectopia are considered together with 13 from previous reports. There was no apparent breed predisposition. Most of the 23 cats were presented for urinary incontinence but two of them were continent. Thirteen were females and ectopia was unilateral in 13 and bilateral in 10. Three kinds of implant to treat incontinence are considered. The sacral anterior root stimulator (with sacral posterior rhizotomy) is already effective in urinary incontinence due to spinal cord injury, and will have wider application. The conditional pudendal nerve stimulator and the conditional gracilis nerve stimulator are, respectively, almost untried and entirely untried devices; but they show promise, and if successful may help very many patients.

In a prospective study, 2 gm of conjugated estrogen vaginal cream was administered daily for a total of 6 weeks in a group of 11 postmenopausal women with urodynamically proved genuine stress incontinence. Midurethral cytologic studies and a complete clinical and urodynamic evaluation were performed twice at 6-week intervals. The authors submit their initial clinical experience with vaginal electrostimulation in the treatment of urinary incontinence, using a CONTERG apparatus made in Czechoslovakia. To determine the prevalence of urinary incontinence and other urinary symptoms a questionnaire was sent to all women aged 25 and over and to women under 21 taking oral contraceptives registered with a rural practice (n = 937); the questionnaire was completed by 833 women (89%). Urinary incontinence in women is common and can be a significant social problem. The most common type is stress incontinence, caused by displacement of the bladder neck and urethra and experienced during activities that increase intraabdominal pressure. Other types are urge, overflow, and mixed incontinence. Diagnosis of the type and cause can usually be made using simple, cost-effective office procedures. Nonsurgical treatment is often successful and may consist of pharmacologic management, intermittent self-catheterization, Kegel exercises, and use of a mechanical device.

Female patients need more privacy for micturition and are, therefore, more prone to suppress the normal micturition reflex. Due to postponed micturition, their bladders are usually full and during stress conditions urinary incontinence does occur.
