Progressive Conduction Disease on Serial ECGs Revealing Immune Checkpoint Inhibitor-Associated Myocarditis and Myositis.

BACKGROUND: Immune checkpoint inhibitors are effective treatments for cancer but also carry cardiovascular toxicity risks. CASE SUMMARY: An 83-year-old male receiving cemiplimab for advanced squamous and basal cell carcinoma presented with severe back pain. One week earlier, electrocardiogram (ECG) demonstrated atrial flutter with new right bundle branch block, slowed atrial activity, and possible complete heart block with a ventricular rate of 48 beats/min, alongside rising liver enzyme abnormalities. On admission, ECG demonstrated complete heart block. Troponin and creatine kinase were elev

Cardiac arrhythmias following COVID-19 vaccination: insights from the U.S. vaccine adverse event reporting system (VAERS), December 2020-January 2025, United States.

BACKGROUND: Although COVID-19 vaccines are highly effective in preventing infection and severe disease, their cardiovascular safety, particularly regarding arrhythmic risk, continues to raise concerns. METHODS: A disproportionality analysis was conducted using the U.S. Vaccine Adverse Event Reporting System (VAERS) to evaluate arrhythmia-related AEs reported between December 2020 and January 2025. Adverse events (AEs) were identified using MedDRA Preferred Terms, and signal strength was assessed by Reporting Odds Ratio (ROR) and Proportional Reporting Ratio (PRR). RESULTS: A total of 52,518

A rare case of bilateral rectus sheath hematoma due to anticoagulation: An abdominal CT scan revelation.

Rectus sheath hematoma (RSH) is an uncommon yet clinically significant cause of acute abdominal pain, often overlooked due to its variable presentation and resemblance to other intra-abdominal pathologies. Predisposing factors include anticoagulation therapy, trauma, pregnancy, and strenuous activity, with anticoagulation-associated RSH carrying particularly high morbidity and mortality. We report a case of bilateral RSH in a 68-year-old female with multiple comorbidities, including atrial fibrillation, who was admitted for community-acquired pneumonia. During hospitalization, she developed at

The effect of first step right atrial mapping (FRAM) on ablation duration and fluoroscopy exposure during cavotricuspid isthmus ablation of atrial flutter.

AIM: To investigate the clinical significance of right atrial mapping prior to cavotricuspid isthmus (CTI) ablation in patients with typical atrial flutter (AFL). METHODS: Clinical and ablation parameters were retrospectively assessed and compared in patients undergoing CTI ablation with or without a first-step right atrial mapping (FRAM) by using the CARTO 3D mapping system. RESULTS: CTI block by radiofrequency ablation (RFA) was achieved in all 143 patients. In the FRAM group there was a shorter ablation duration and fluoroscopy exposure compared with the non-FRAM group. CHA2DS2-VASc score

Permanent Pacemaker Placement Secondary to Remdesivir Induced Bradycardia: A Case Report.

Remdesivir is approved by the FDA for the treatment of hospitalized coronavirus disease 2019 (COVID-19) patients. It is known to be associated with transient bradycardia that resolves after discontinuation of the drug. We present a case of a 71-year-old male with a history of congestive heart failure, hypertension, and atrial flutter (rate controlled with carvedilol) presented for evaluation of worsening dyspnea, dry cough, and fatigue. His COVID-19 reverse transcription-polymerase chain reaction (RT-PCR) was positive and his chest x-ray showed right mid-lung opacity. Oxygen saturation was 88%

Right-sided approach to left bundle branch area pacing combined with atrioventricular node ablation in a patient with persistent left superior vena cava and left bundle branch block: a case report.

BACKGROUND: Left bundle branch area pacing (LBBAP) is an alternative to right ventricular (RV) and biventricular (BiV) pacing in patients scheduled for pace and ablate treatment strategy. However, current delivery sheaths are designed for left-sided implantation, making the right-sided LBBAP lead implantation challenging. CASE PRESENTATION: We report a case of a right-sided LBBAP approach via right subclavian vein in a heart failure patient with a persistent left superior vena cava scheduled for pace and ablate treatment of refractory atrial flutter. To enable adequate lead positioning and su

Acute heart failure and bradyarrhythmia in a young male-what hides beneath the surface?: a case report.

BACKGROUND: Muscular dystrophies (MDs) are characterized by early-onset muscular atrophy and weakness, with frequent cardiac involvement. Myocardial dysfunction and conduction system involvement are often rapidly progressive despite medical and device therapy, and may even precede muscular symptoms, posing a challenge to diagnosis. CASE SUMMARY: We report a case of a young male admitted to a cardiac intensive care unit due to 'de novo' acute heart failure (HF) and atrial flutter with a slow ventricular response. Careful evaluation of past medical history revealed the presence of neuromuscular

Arrhythmias in children.

Arrhythmias in children can be classified according to their effect on central pulse: Fast pulse rate - tachyarrhythmia; Slow pulse rate - bradyarrhythmia; and Absent pulse is pulseless arrest (cardiac arrest). Tachyarrythmia may be narrow complex tachycardia (QRS duration ≤ 0.08 s): sinus tachycardia (ST), supraventricular tachycardia (SVT), atrial flutter or Wide-complex tachycardia (QRS duration >0.08 s): ventricular tachycardia (VT), SVT with aberrant intraventricular conduction. The choice of therapy depends on the patient's degree of hemodynamic instability. Attempt vagal stimulation, if

Dronedarone: a new antiarrhythmic agent.

Dronedarone is an antiarrhythmic agent recently approved by the United States Food and Drug Administration for the reduction of cardiovascular-related hospitalizations in patients with paroxysmal or persistent atrial fibrillation or atrial flutter. The drug is a derivative of amiodarone and has been modified to reduce the organ toxicities frequently encountered with amiodarone. Dronedarone exerts its antiarrhythmic effects through multichannel blockade of the sodium, potassium, and calcium channels and also possesses antiadrenergic activity, thereby exhibiting pharmacologic effects of all four

Use of a unique long pre-curved sheath to facilitate femoral placement of coronary sinus catheters.

METHODS: Catheterization of the coronary sinus (CS) from the femoral vein is widely used during electrophysiologic procedures. Access to the CS may be difficult. To address this problem we explored the utility of a long pre-formed (SAFL) sheath in a cohort of consecutive patients requiring CS cannulation in the electrophysiology laboratory. This unique sheath has distal curvatures in 2 planes, potentially facilitating CS cannulation. RESULTS: 68 patients were studied with an average age of 63+/- 16 years. In twelve patients (18%), standard femoral CS cannulation was ineffective. In six of the

A fuzzy clustering neural network architecture for classification of ECG arrhythmias.

Accurate and computationally efficient means of classifying electrocardiography (ECG) arrhythmias has been the subject of considerable research effort in recent years. This study presents a comparative study of the classification accuracy of ECG signals using a well-known neural network architecture named multi-layered perceptron (MLP) with backpropagation training algorithm, and a new fuzzy clustering NN architecture (FCNN) for early diagnosis. The ECG signals are taken from MIT-BIH ECG database, which are used to classify 10 different arrhythmias for training. These are normal sinus rhythm,

Atrial tachycardia ablation in a patient with double outlet right ventricle corrected by surgery.

The development of surgical and percutaneous techniques for treatment or palliation of congenital heart disease has prolonged survival in these patients and has increased late complications, particularly arrhythmias. Such arrhythmias are more frequently refractory to medical therapy, requiring percutaneous ablation. We present the clinical case of a 14-year-old child with complex congenital heart disease (double outlet right ventricle) who underwent two corrective surgeries (Rastelli operation and subsequent replacement of the homograft in the conduit connecting the right ventricle to the pulm

Steps towards the development of a safe and effective tachycardia terminating pacemaker.

Permanent pacemakers have been used for the treatment of tachycardias. Four studies have been performed to improve termination algorithms. Permanent extrastimulus pacing was assessed in 19 patients. During follow up of 13 to 36 (mean 27) months, 7 also required additional antiarrhythmic drugs. Pacing was effective in all, but there was one unexplained sudden death. Autodecremental (rate-increasing) atrial pacing was used in 20 patients with junctional arrhythmias. It was effective in all, especially with a burst duration of 5000 ms, and caused no acceleration of tachycardia. In contrast, const

Value of a Machine Learning Approach for Predicting Clinical Outcomes in Young Patients With Hypertension.

Risk stratification of young patients with hypertension remains challenging. Generally, machine learning (ML) is considered a promising alternative to traditional methods for clinical predictions because it is capable of processing large amounts of complex data. We, therefore, explored the feasibility of an ML approach for predicting outcomes in young patients with hypertension and compared its performance with that of approaches now commonly used in clinical practice. Baseline clinical data and a composite end point-comprising all-cause death, acute myocardial infarction, coronary artery reva

Importance of monitoring zones in the detection of arrhythmias in patients with implantable cardioverter-defibrillators under remote monitoring.

INTRODUCTION: Implantable cardioverter-defibrillator (ICD) monitoring zones (MZ) provide passive features that do not interfere with the functioning of active treatment zones. However, it is not known for certain whether programming an MZ affects arrhythmia detection by the ICD. The aim of the present study is to assess the clinical relevance of MZ in a population of patients with ICDs. METHODS: In this retrospective analysis of patients with ICDs, with or without cardiac resynchronization therapy, for primary prevention under remote monitoring, the MZ was analyzed and recorded arrhythmias we
