Measuring left ventricular outflow tract (LVOT) gradient by echocardiography in decubitus position, which is used in routine clinical practice, does not reflect the pathophysiology of this dynamic abnormality during daily activities, which trigger the symptoms. LVOT obstruction is dynamic and greatly dependent upon the left ventricular cavity size, geometric configuration of hypertrophy, load, contractility and mitral apparatus abnormalities, including systolic anterior motion of mitral leaflet. Importantly, LVOT gradient may develop not only in hypertrophic cardiomyopathy, but also in other h

BACKGROUND: Accessory pathway ablation is recommended for patients with symptomatic Wolff-Parkinson-White (WPW) syndrome. Although elimination of the accessory pathway restores normal ventricular activation and improves synchrony, it can occasionally cause systolic anterior motion (SAM) of the mitral valve leaflet, which can lead to functional left ventricular outflow tract (LVOT) obstruction.
CASE SUMMARY: We report a case of a patient admitted with WPW syndrome. After successful posteroseptal accessory pathway ablation, the patient developed exertional chest tightness and syncope. Echocardi

BACKGROUND AND AIM OF THE STUDY: Stentless aortic bioprostheses have excellent hemodynamics, although heterogeneity in gradients has been observed. The present study was intended to determine whether high early postoperative transvalvular velocities correlate with other measures of left ventricular outflow obstruction, whether the phenomenon is transient, and whether high velocities observed early after surgery predict differences in subsequent valve performance or left ventricular remodeling.
METHODS: Sixty-eight consecutive patients who underwent implantation of Freestyle stentless aortic b…

Evaluation of the right ventricular (RV) outflow tract in congenital heart disease is extremely important for surgical management. Therefore, the value of 2-dimensional echocardiography (2-D echo) to assess the RV outflow tract was studied using a new approach: the subcostal elongated right oblique view. Twenty normal children and 49 children with congenital heart disease, aged 1 day to 11 years, were studied. Significant pulmonary infundibular obstruction was present in 22 patients with conotruncal malformations. To obtain the subcostal elongated right oblique view from the short-axis view at

We report a case of successful aortic valve replacement and relief of right and left ventricular outflow tract obstruction 8 years after an arterial switch operation for double outlet right ventricle. Since the surgical access to the ascending aorta was limited because of the anatomical feature and the adhesion after the arterial switch operation, arterial infusion site for cardiopulmonary bypass was secured at the right common carotid artery ahead of the sternal re-entry. After cardiopulmonary bypass was established, the right pulmonary artery was divided and then dissection of the ascending

BACKGROUND: Double-chambered right ventricle (DCRV) is a rare congenital anomaly characterized by hypertrophied muscular bundles dividing the right ventricle into high- and low-pressure chambers. It is frequently associated with congenital heart disease, particularly tetralogy of Fallot (TOF), but rarely presents in late adulthood.
CASE PRESENTATION: We report the case of a 78-year-old man with a history of prior pulmonary valvotomy who presented with progressive dyspnea and was found to have severe subvalvular right ventricular outflow tract obstruction. Multimodal imaging and cardiac cathet

High transaortic valvular gradients, after combined aortic valve and mitral valve replacement, require prompt intraoperative diagnosis and appropriate management. The presence of high transaortic valvular gradients after cardiopulmonary bypass, in this setting, can be secondary to the following conditions: prosthesis dysfunction, left ventricular outflow tract obstruction, supravalvular obstruction, prosthesis-patient mismatch, hyperkinetic left ventricle from administration of inotropes, left ventricular intracavitary gradients, pressure recovery phenomenon, and increased transvalvular blood

In the current era of superb surgical results for congenital heart disease, several management options are available for patients born with double outlet right ventricle (DORV). The surgical repair of DORV is tailored to address the variety of abnormalities associated with this lesion. The treatment strategies are dependent upon the anatomy and relationship of the ventricular septal defect (VSD) and the great arteries. For patients with subaortic or doubly committed VSDs, without right ventricular outflow tract obstruction, the usual repair is an intraventricular tunnel from the VSD to the aor

Hypertrophic cardiomyopathy (HCM) is characterized as left ventricular hypertrophy unexplained by abnormal loading conditions. Approximately 30% of patients with HCM will develop left ventricular outflow obstruction under resting conditions. In the 5-10% of patients with drug-refractory symptoms, surgical septal myectomy has been shown to reduce outflow obstruction and relieve symptoms; however, some patients may be at increased risk for surgery. Since 1994, when alcohol septal ablation was introduced, an increasing number of patients have opted for the percutaneous approach, which aims to mim…

As far as we are aware, a bicuspid aortic valve has not previously been reported in the setting of tetralogy of Fallot and pulmonary atresia. We describe this association in a newborn who presented with a murmur and cyanosis. Echocardiography showed tetralogy of Fallot with pulmonary atresia, and also a moderately stenotic bicuspid aortic valve. The patient underwent open-heart surgery guided by transesophageal echocardiography. Postoperatively, there was only mild obstruction across both outflow tracts. We have also reviewed the pertinent data from our Jesse E. Edwards Registry of Cardiovascu

Takotsubo syndrome (TTS) is an acute, transient, non-ischemic cardiomyopathy marked by circumferential wall‑motion abnormalities that typically extend beyond a single epicardial territory. Although most patients recover left ventricular function within weeks, the acute phase can be destabilized by mechanical complications and cardiogenic shock. Mitral regurgitation (MR) in TTS is frequent and clinically consequential, arising from systolic anterior motion (SAM) with dynamic left ventricular outflow tract obstruction (LVOTO) or from leaflet tethering driven by apical or mid-ventricular ballooni

BACKGROUND: Hypertrophic obstructive cardiomyopathy (HOCM) is a genetic cardiac disease characterized by left ventricular hypertrophy that induces left ventricular outflow tract obstruction (LVOTO), which occurs in ∼25% of patients with hypertrophic cardiomyopathy (HCM), and, less commonly, a midventricular obstruction (MVO). Percutaneous transluminal septal myocardial ablation (PTSMA) treatment for HOCM has become an established therapy to alleviate LVOTO.
CASE SUMMARY: A 78-year-old woman with HOCM and severe MVO underwent PTSMA using simultaneous contrast imaging with left ventriculography

Tetralogy of Fallot (TOF) classically consists of four characteristic features-right ventricular outflow obstruction, right ventricular hypertrophy, ventricular septal defect and an overriding aorta. In addition there are multiple other associated cardiac anomalies, including coronary artery anomalies. In this review, the role of CT angiography and the spectrum of coronary anomalies will be discussed along with importance of such anomalies in the context of surgery.

To define the clinical course of ventricular septal defect, 410 consecutive patients with isolated ventricular septal defect were evaluated over a period of 13 years. Their age ranged from 12 days to 24 years at the time of first visit to the hospital. Patients with less than 2 years follow-up period were excluded. One hundred and fifty seven patients were one year of age or less. The left to right shunt size remained the same in 52.4% of cases. In 34.4% the shunt size decreased, with complete closure of ventricular septal defect in 8.8%. Closure of ventricular septal defect was observed even

Actual and potential factors that create an obstruction to the flow within the systemic side of the heart have been analyzed in 114 specimens with atrioventricular septal defects. Hearts with separate right and left atrioventricular orifices and with common atrioventricular orifice were included. Potential inlet obstruction was due to tethering of the "cleft" of the left atrioventricular valve to the septum in 17.5%, incomplete "cleft" in 8%, and deformed valve leaflets in 70% of cases. Potential factors predisposing to left ventricular outflow tract obstruction were seen in 70% of cases, 21%

BACKGROUND: Tetralogy of Fallot (TOF) with subarterial ventricular septal defect (VSD) is more common among Asians than Caucasians. Compared with the regular subaortic VSD postoperative right ventricular outflow obstruction is more common because of the sub-pulmonary extension of the defect. The objective of this study is to analyze the surgical implications and outcomes of patients with TOF - subarterial VSD in the current era.
PATIENTS AND METHODS: In all, 539 consecutive operated patients with TOF from May 2005 to September 2012 were retrospectively reviewed. Eighty-five patients had subar…

OBJECTIVES: The purpose of this study is to describe the long-term results of the 'réparation à l'étage ventriculaire' (REV) technique for double-outlet right ventricle and transposition of the great arteries (TGA) with pulmonary stenosis (PS).
METHODS: Between 1980 and 2021, 157 patients underwent a REV procedure (median age and weight: 20.8 months and 7.7 kg). The most frequent anatomical presentation was the association between TGA, ventricular septal defect and PS (n = 116, 73.9%).
RESULTS: Sixty-seven patients (42.7%) underwent a Rashkind procedure, and 67 patients (42.7%) a prior surgi

OBJECTIVE: An arterial switch operation is considered a good alternative for the repair of double-outlet right ventricle (DORV) with atrioventricular concordance connection and subpulmonary ventricular septal defect (VSD) when intraventricular rerouting is not feasible. The clinical results of an arterial switch operation with ventricular septal defect closure for this anomaly were studied.
METHODS: Between 1986 and 1997, 27 patients ranging from 10 days to 5 years of age (mean 0.4 years) underwent an arterial switch operation with ventricular septal defect closure for the correction of doubl

Hypertrophic obstructive cardiomyopathy is a relatively common disorder that signifies asymmetric hypertrophy of interventricular septum causing obstruction of the left ventricular outflow tract (LVOT). However, more recent studies have shown that during ventricular systole, flow against an abnormal mitral valve apparatus results in drag forces on the part of the leaflets. The mitral leaflet is pushed into the LVOT to obstruct it. We present a case where intraoperative transesophageal echocardiography played a crucial role in defining the etiology of LVOT obstruction that subsequently helped i
