Chronic diarrhea in pets features high incidence, often causing vitamin B12 deficiency. Probiotics, which can alleviate or even treat chronic diarrhea by improving intestinal microbial balance, have shown promise. This paper reviews the maternal and neonatal outcomes associated with such diets, highlighting considerations for supplementation and potential risks, including B12-vitamin deficiency. Recommendations include early dietary assessment, B12-supplementation, and monitoring of key nutrients such as protein and iron. Despite potential challenges, with proper guidance and supplementation, vegetarian and vegan diets can be compatible with a healthy pregnancy and sufficient lactation.

This case report presents a male in his 30s with pernicious anaemia, initially diagnosed with autoimmune haemolytic anaemia and thrombocytopenia. Despite improvement with treatment, he developed bilateral leg weakness and numbness, ultimately diagnosed as peripheral neuropathy. Vitamin B12 and folate deficiency are reversible causes of megaloblastic anemia. Strict vegetarians are at risk of megaloblastic anemia due to low cobalamin in their diet. Knuckle hyperpigmentation in patients with megaloblastic anemia is due to excess melanin synthesis in skin. Here we present a case of a young vegetarian male with megaloblastic anemia with knuckle hyperpigmentation managed successfully with intravenous followed by oral vitamin b12 and folate supplementation.

Vitamin B12 (Cbl) deficiency may cause hematologic and neurologic dysfunction. Replacement therapy is effective in correcting hematologic abnormalities and improving neurologic symptoms. Cbl is known to have antioxidant activity. This antioxidant activity may antagonize the effects of chemotherapeutics (i.e. genotoxic effects of paclitaxel) on tumor DNA. We claim that Cbl replacement should be done more cautiously in patients receiving cytotoxic chemotherapy.
