Acute tumour lysis syndrome (ATLS) is a metabolic derangement (hyperuricaemia, hyperphosphataemia, hyperkalaemia and hypocalcaemia) associated with lymphoproliferative malignancies. The nature and severity of the metabolic alterations are variable. Major complications are oliguric acute renal failure and delays in initiating chemotherapy. Enprofylline is a novel xanthine with a low adenosine antagonistic potency, but with a good bronchodilator activity. In order to further elucidate its central nervous actions we have studied its effects in the rat hippocampus, both electrophysiologically and biochemically. The mechanism of cytochrome P-450-dependent oxidation of ethanol has been investigated using reconstituted phospholipid vesicles containing … chrome b5 instead of cytochrome P-450 and in other systems regenerating superoxide anions, e.g. the xanthine-xanthine oxidase system and dihydroxyfumarate. The results are consistent with an iron-catalyzed Haber-Weiss mechanism for regeneration of hydroxyl radicals which subsequently react with ethanol, thereby giving the corresponding aldehyde.

This paper reports biochemical studies in a pregnant xanthinuric female (McKusick 27830) and compares the results with findings in three other xanthinurics studied by us, including a previously unreported female who had a nephrectomy for xanthine stones. The findings of raised levels of uric acid in plasma and urine at presentation in the third trimester of pregnancy, and the subsequent fall to almost undetectable levels 6 weeks post-partum, is regarded as evidence of the extent of fetal uric acid production and clearance by the maternal circulation.

Aqueous extracts of guaraná were studied in terms of effects on the aggregation of human and rabbit platelets. Guaraná extracts have anti-aggregatory and de-aggregatory actions on platelet aggregation induced by ADP or arachidonate but not by collagen. The active material was shown to be water soluble and heat resistant and appeared to be different from salicylates, nicotinic acid or known xanthines. Guaraná extracts inhibited platelet aggregation in rabbits following either intravenous or oral administration.

For a particular form (amorphous phase) of physical acting substances the water solubility, in conditions very close to the in vivo ones, has been compared with corresponding biological activities. The reciprocal of the above parameter shows a good correlation with various physico-chemical, biochemical and pharmacological parameters of xanthinic compounds, analgesic alkaloids, barbiturates, sulphonamides. The solubility of the crystalline phase cannot be correlated with the biological activity: the ratio between these two solubilities shows this inability.

In ischaemic heart conditions we report a remarkable increase in platelet xanthine oxidase activity and rise in the levels of malondialdehyde (MDA) with concomitant decrease in the activities of free radical scavenging enzymes - superoxide dismutase, catalase, glutathione peroxidase and glutathione reductase. The increased levels of free radical generating system and MDA and lowered levels of free radical scavenging systems seem to have critical role in ischaemic heart conditions.

A seven-week-old infant presented with tonic posturing and nervous irritability after concentrated tea ingestion. The tea was analyzed and found to contain caffeine, a methylxanthine central nervous system stimulant. With the aid of a computer, projected blood levels were calculated over a thirty-hour time period and comared to the infant's clinical status. It is recommended that beverages containing xanthines be given to infants and small children with caution.
