Adult Chemical Pneumonitis Caused by Accidental Kerosene Aspiration: A Case Report.

Kerosene aspiration is a recognized cause of chemical pneumonitis, most commonly reported in children, whereas adult cases are rare. We present a case of severe chemical pneumonitis in an elderly patient following accidental kerosene ingestion in an agricultural setting. A 73-year-old male farmer undergoing maintenance dialysis accidentally ingested kerosene that had been stored in a plastic drink bottle during open-field burning. Although he immediately spat it out, he developed progressive dyspnea, fatigue, and vomiting. Chest radiography and computed tomography revealed extensive bilateral

Intraoperative accidental hypothermia as a probable cause of malignant ventricular arrhythmias in an elderly patient undergoing transurethral resection of prostate: A case report.

BACKGROUND: Intraoperative hypothermia is a common but severe condition that is defined as a core body temperature below 36 °C. Accidental hypothermia can produce coagulopathy, immunosuppression and peripheral hypoperfusion that can ultimately lead to life-threatening ventricular arrhythmias and vital organ injury, and it is significantly associated with perioperative complications and mortality. CASE DESCRIPTION: We report the case of an 82-year-old man who presented with persistent ventricular tachycardia intraoperatively due to accidental hypothermia. The patient was diagnosed with benign

Pediatric hypothermic submersion incident - should we do chest compressions on a beating heart?

BACKGROUND: Drowning is the third leading cause of unintentional injury death worldwide, with the highest rates of fatality among young children. To decide how to treat these patients prehospitally could be challenging in certain situations when uncertain about the adequacy of the patent's circulation. METHODS/CASE REPORT: We describe a 2 year old boy surviving a 15 min hypothermic submersion in a cold river. In spite of the presence of some vital signs, we decided to do full cardiopulmonary resuscitation to the hospital. The main reason was that we were uncertain about the adequacy of the sp

Chemical colitis.

Chemical colitis can occur as a result of accidental contamination of endoscopes or by intentional or accidental administration of enemas containing various chemicals. Most cases have occurred after accidental contamination of endoscopes with glutaraldehyde and/or hydrogen peroxide. There have been multiple case reports of chemical colitis resulting from unintentional administration of caustic chemicals. Intentional administration of corrosive enemas has been implicated in sexual practices, bowel cleansing, or in suicide attempts. Patients present with nonspecific symptoms including abdominal

Biomechanical evaluation of head and neck injuries during head-first falls in skiing.

Skiing accidents may lead to severe head and neck injuries. This study uses the THUMS model to evaluate the initial conditions and related injury mechanisms of head impacts with the snow surface during skiing falls. Initial speed was found to be the main factor affecting head injury criterion (HIC). Impact position is the key factor affecting cervical curvature. Rear impacts cause excessive cervical spine extension, while frontal impacts cause excessive cervical spine flexion. Rear impacts are more likely to cause neck injuries. As the angle and speed increase, the degree of cervical spine inj

A ring-shaped gas sign: a case of the accidental ingestion of a press-through package.

This case report presents a common but instructive clinical scenario of accidental ingestion of a press-through package. Despite an initial negative chest X-ray and mild symptoms, the diagnosis was confirmed with additional computed tomography. The patient was eventually went through the successful endoscopic removal of the press-through package. Furthermore, a retrospective re-reviewing of the X-ray revealed a faint ring-shaped gas sign, characteristic of press-through package ingestion. This case underscores the intractableness to diagnosis of accidental ingestion of press-through package by

Validity and reliability International Classification of Diseases-10 codes for all forms of injury: A systematic review.

BACKGROUND: Intentional and unintentional injuries are a leading cause of death and disability globally. International Classification of Diseases (ICD), Tenth Revision (ICD-10) codes are used to classify injuries in administrative health data and are widely used for health care planning and delivery, research, and policy. However, a systematic review of their overall validity and reliability has not yet been done. OBJECTIVE: To conduct a systematic review of the validity and reliability of external cause injury ICD-10 codes. METHODS: MEDLINE, EMBASE, COCHRANE, and SCOPUS were searched (incep

A novel repair strategy using knotless squeeze anchors for lumbar disc herniation with endplate junction lesions under biportal endoscopic spinal surgery.

BACKGROUND: Annulus fibrosus-endplate (AF-EP) junction lesions are important determinants for lumbar disc herniation (LDH). Utilizing biportal endoscopic spinal surgery (BESS), we introduce a novel repair method using bioabsorbable PushLock anchors with suture fibers to stretch disconnected AF tissues to the vertebral cortex. METHODS: The viewing and working portals are established to excise herniated disc materials causing radiculopathy. Through the working portal, a suture strand is passed through the intact AF tissue near the lesion and retrieved using the Suture Crossing Device. Then, the

Enhanced proliferation and migration of fibroblast cells by skin-attachable and self-cleaning triboelectric nanogenerator.

Skin wounds are common in accidental injuries, surgical operations, and chronic diseases. The migration and proliferation of fibroblast cells are fundamental to wound healing, which can be promoted by electrical stimulation as a physical therapy modality. Therefore, the development of portable electrical stimulation devices that can be used by patients on-site is an essential need. In the present study, a self-cleaning triboelectric nanogenerator (TENG) has been fabricated for enhancing cell proliferation and migration. The polycaprolactone‑titanium dioxide (PCL/TiO2) and polydimethylsiloxane …

Pre-injury chronic alcohol abuse predicts intracranial hemorrhagic progression, unfavorable clinical outcome, and mortality in severe traumatic brain injury.

OBJECTIVE: We aimed to determine the incidence of pre-injury alcohol abuse in TBI at our neurointensive care unit (NICU), the relation to intracranial hemorrhage evolution, and clinical outcome. METHODS: Patients with TBI treated at our NICU at Uppsala university hospital, Sweden, 2008-2018, were included. Clinical, radiological, and outcome variables were evaluated. RESULTS: Of 844 patients with TBI, 147 (17%) had a history of pre-injury alcohol abuse and these patients were slightly older, but had a similar Charlson co-morbidity index as the other patients. They were more often injured by

Prevention of Unintentional Childhood Injury.

Unintentional injury accounts for one-third of deaths in children and adolescents each year, primarily from motor vehicle crashes. Children younger than 13 years should be restrained in the back seat, and infants and toddlers should remain rear-facing until at least two years of age. Infants should be positioned on their backs in a crib, on a mattress with only a fitted sheet to avoid suffocation, and all items that could potentially entrap or entangle the child should be removed from the sleep environment. Fencing that isolates swimming pools from the house is effective in preventing drowning

Pediatric Distal Radius Fractures.

Distal radius fractures are the most common orthopaedic injury that occur in the pediatric population. The annual incidence of distal radius fractures has increased as a result of earlier participation in sporting activities, increased body mass index, and decreased bone mineral density. Most distal radius fractures are sustained after a fall onto an outstretched arm that results in axial compression on the extremity or from direct trauma to the extremity. Physeal fractures of the distal radius are described based on the Salter-Harris classification system. Extraphyseal fractures of the distal

Unintentional injuries after TBI: Potential risk factors, impacts, and prevention.

BACKGROUND: The top three causes of fatal unintentional injuries are falls, motor vehicle crashes, and being struck against or struck by objects or persons. These etiologies also happen to be the leading causes of TBI, a serious public health problem, in the US. Reduced cognitive functioning, poor decision making, increased risk taking, disinhibition, diminished safety skills and substance use, place individuals with TBI at an increased risk for subsequent unintentional injuries. The caregiving, psychological, social and financial burden of initial injuries is enormous. Unintentional injuries

Posttraumatic anosmia secondary to central nervous system injury.

BACKGROUND: The presumed pathogenesis of posttraumatic anosmia is stretching or shearing of the olfactory nerves in a coup-contracoup head contusion. Direct injury to the brain is an alternate mechanism of injury. In this study we report a case where direct injury to the brain is the probable mechanism of injury. METHODS: A case report was performed. RESULTS: A 55-year-old man presented with loss of smell beginning 1 month after a closed head injury with loss of consciousness. The MRI showed posttraumatic scarring in the region of the olfactory bulbs. CONCLUSION: This case suggests that cen

[Serious brain injury and public health, epidemiologic and financial considerations, comprehensive management and care].

The management of persons with traumatic brain injury affects a large spectrum of interventions from acute phase to the hospital discharge and the return to community. The incidence of brain injuries on mortality and morbidity constitutes a serious problem in front of the Health Administration. The traffic accidents remain the main cause but the falls in elderly are increasing. In the both cases preventive measures can be efficiency. In France, each year, there are about 150,000 new cases, 8000 of them will be dead and 4000 with coma. It is likely that 30,000 persons are living to day with imp

Tension pneumothorax: eyes may be more diagnostic than ears.

A case is presented of unilateral tension pneumothorax in an awake patient who was seen in prehospital care after a significant fall. Because of extrication difficulties it was 40 minutes after the accident when he was first seen and by this stage the tension pneumothorax was well developed. Many features that are taught as "classic" of tension pneumothorax were absent but various other clinical signs were present to aid the diagnosis, and these are reviewed.

[Lateral dislocation of the elbow: pathophysiological and therapeutic analysis].

Lateral dislocation of the elbow is exceptional, and its pure form even more so. We report a case of lateral dislocation of the elbow observed in a 65-year-old woman following a fall on the medial aspect of the elbow in the flexion position. Reduction could not be achieved because of the anconeus muscle interposition. Analysis of the mechanism involved and the displacements observed enabled to propose a reduction manoeuvre using gentle traction on the elbow with extension and progressive supination of the forearm, pushing the olecranon backward then pulling it medialy. This enabled avoiding in

In-line skating injuries.

In-line skating has remained a popular recreational activity all over the world. The number of injuries seemingly reached a plateau in the late 1990s. However, there are still more injuries caused by in-line skating than from skateboarding, hockey or lacrosse and rugby. In 1997, the estimated annual cost of medical treatment for these injuries in the US was over $US4 billion. Adolescence still represents the largest group of skaters, both in participation and injury. Although 10- to 14-year-olds account for 60% of injuries, in 1996 the US Consumer Product Safety Commission reported nearly 1500

Surface stiffness affects impact force during a fall on the outstretched hand.

Falls on the outstretched hand are among the most common causes of traumatic bone fracture. However, little is known regarding the biomechanical factors that affect the risk for injury during these events. In the present study, we explored how upper-extremity impact forces during forward falls are affected by modification of surface stiffness, an intervention applicable to high-risk environments such as nursing homes, playgrounds, and gymnasiums. Results from both experimental and linear biomechanical models suggest that during a fall onto an infinitely stiff surface, hand contact force is gov
