Use of intravenous immunoglobulin in children with stevens-johnson syndrome and toxic epidermal necrolysis: seven cases and review of the literature.

Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis are the most severe cutaneous reactions that occur in children. Off-label use of human intravenous immunoglobulin (IVIG) has been reported in a number of autoimmune and cell-mediated blistering disorders of the skin, including severe cutaneous drug reactions. We review 28 previous reports in which IVIG was used in pediatric patients with SJS and toxic epidermal necrolysis and discuss our experience in 7 children with SJS, in whom no new blisters developed within 24 to 48 hours after IVIG administration and rapid recovery ensued. IVI

Herpes zoster and lymphopenia associated with sodium stibogluconate therapy for cutaneous leishmaniasis.

A review of 84 patients with cutaneous leishmaniasis treated with sodium stibogluconate (Pentostam) at our institution revealed that three had developed herpes zoster during or shortly after receiving therapy. Because zoster has been associated with depressed cellular immunity, we prospectively followed serial lymphocyte subpopulations in eight patients with cutaneous leishmaniasis who received Pentostam. By day 7 of therapy, the white blood cell count had fallen by a median of 1.15/mm3, the total lymphocyte count by a median of 804/mm3, and the CD4+ lymphocyte count by a median of 306/mm3 (67

Miltefosine--discovery of the antileishmanial activity of phospholipid derivatives.

Miltefosine (hexadecylphosphocholine, Impavidotrade mark), a novel antiprotozoal drug used for the treatment of visceral and cutaneous leishmaniasis, was identified and evaluated independently in the early 1980s as a potential anticancer drug in Germany and as an antileishmanial drug in the UK. Although miltefosine is not the most active compound of its class against Leishmania parasites in vitro, the early demonstration of activity after oral administration in experimental models of visceral leishmaniasis helped to bring this compound to the attention of WHO TDR for further development in a u

Effects of Bone Marrow Mesenchymal Stromal Cell Therapy in Experimental Cutaneous Leishmaniasis in BALB/c Mice Induced by Leishmania amazonensis.

Cutaneous leishmaniasis remains both a public health and a therapeutic challenge. To date, no ideal therapy for cutaneous leishmaniasis has been identified, and no universally accepted therapeutic regimen and approved vaccines are available. Due to the mesenchymal stromal cell (MSC) immunomodulatory capacity, they have been applied in a wide variety of disorders, including infectious, inflammatory, and allergic diseases. We evaluated the potential effects of bone marrow MSC therapy in a murine model of cutaneous leishmaniasis. In vitro, coculture of infected macrophages with MSC increased para

Tailored green synthesized silymarin-selenium nanoparticles: Topical nanocarrier of promising antileishmanial activity.

Poor drug penetration, emerging drug resistance, and systemic toxicity are among the major obstacles challenging the current treatment of cutaneous leishmaniasis. Hence, developing advanced strategies for effective and targeted delivery of antileishmanial agents is crucial. Several drug delivery carriers have been developed till current date for dermal/transdermal delivery, especially those which are fabricated using eco-friendly synthesis approaches, since they protect the environment from the harmful effects of chemical waste disposal. This work describes the preparation of selenium nanopart

Intramuscular and topical treatment of cutaneous leishmaniasis lesions in mice infected with Leishmania amazonensis using coumarin (-) mammea A/BB.

Treatment of cutaneous leishmaniasis remains limited to a few available options. Recent studies showed in vitro antileishmanial activity of (-) mammea A/BB, a coumarin isolated from leaves of Calophyllum brasiliense. Moreover, the dichloromethane crude extract and hexane fraction from this plant demonstrated in vivo activity in mice infected with Leishmania amazonensis. We evaluated the antileishmanial activity of (-) mammea A/BB in the L. amazonensis BALB/c mice model. The animals were given intramuscular and topical treatment with (-) mammea A/BB for 30 consecutive days. The results demonstr

In vitro skin permeation and retention of paromomycin from liposomes for topical treatment of the cutaneous leishmaniasis.

Paromomycin (PA), a very hydrophilic antibiotic, has been tested as an alternative topical treatment against cutaneous leishmaniasis (CL). Although this treatment has shown promising results, it has not been successful in accelerating the recovery in most cases. This could be attributed to the low skin penetration of PA. Liposomal formulations usually provide sustained and enhanced drug levels in skin. The aim of this study was to prepare liposomal formulations containing PA and to investigate their potential as topical delivery systems of this antileishmanial. Large multilamellar vesicles (ML

Phorbol 12-myristate 13-acetate-induced development of functionally active mast cells in W/Wv but not Sl/Sld genetically mast cell-deficient mice.

The normal skin and other tissues of adult genetically mast cell-deficient WBB6F1-W/Wv or WCB6F1-Sl/Sld mice contain less than 1.0% the number of mast cells present in the corresponding tissues of the congenic normal (+/+) mice. We previously reported that mature dermal mast cells developed locally in the skin of W/Wv, but not Sl/Sld, mice at sites of chronic idiopathic dermatitis. We now report that the repeated application of phorbol 12-myristate 13-acetate (PMA) to the ear skin of either W/Wv or +/+ mice induces both dermatitis and a striking and dose-dependent increase in the number of der

Toxic effects of multiple anticancer drugs on skin.

In this study, cutaneous toxicities associated with the administration of chemotherapy in combination are discussed. Rapidly growing cells are the targets of chemotherapy, so the skin, hair follicles, and nail matrix are frequently affected by chemotherapy. Chemotherapy skin reactions are more likely toxic than allergic reactions. There are numerous chemotherapy-induced cutaneous reactions that have been described in the literature. In addition to a variety of miscellaneous reactions, 19 major cutaneous reactions were discussed in current study. This study was designed to evaluate the clinical

Cutaneous sensitivity induced by immunization with irradiated Schistosoma mansoni cercariae. II. Autoimmunoregulation of cell-mediated cutaneous sensitivity and its reversal.

C57BL/6 mice sensitized by abdominal, dermal exposure to irradiated (50 kR) Schistosoma mansoni cercariae develop partial protection against subsequent exposure with unattenuated cercariae and express cell-mediated cutaneous sensitivity upon challenge with irradiated cercariae. Autoimmunoregulation occurs as a part of this sensitization, and can be demonstrated by augmentation of cutaneous sensitivity upon use of appropriate regimens of cyclophosphamide. Mice exposed to irradiated cercariae by either intraperitoneal or ear pinna routes developed a transient hyporesponsiveness to cercarial chal

Vancomycin-associated spontaneous cutaneous adverse drug reactions.

PURPOSE: With the increase in vancomycin use, adverse drug reactions (ADRs) associated with vancomycin have been reported increasingly more often. However, the characteristics of cutaneous ADRs with and without systemic reactions (SRs) have not been described. This study investigated the characteristics of spontaneously reported and assessed ADRs associated with vancomycin by a pharmacovigilance center. METHODS: ADRs (n=121) associated with vancomycin in 96 patients were collected from 2008 to 2009. Records from physician- and nurse-reported suspected cases of vancomycin ADRs, ADR type, laten

Does magnesium permeate the skin? An ex vivo investigation of magnesium ion delivery through intact and barrier-disrupted skin.

The potential for cutaneous magnesium delivery remains uncertain despite widespread use of topical formulations. This study aimed to directly evaluate magnesium ion permeation from a saturated aqueous magnesium sulfate monohydrate solution across porcine skin using an ex vivo Franz diffusion cell model. Permeated magnesium was quantified by inductively coupled plasma mass spectrometry in both intact and tape-stripped porcine skin, the latter representing impaired barrier conditions. Minimal magnesium permeation was observed across intact skin, with no significant difference between magnesium-t

Elucidating the release and permeation mechanisms of emulsion-type cataplasms from a structural perspective.

Drug release and permeation play crucial roles in the therapeutic efficacy of cataplasms. However, due to the lack of research on the mechanisms underlying drug release and permeation, achieving optimal release and permeation characteristics through trial-and-error methods is inefficient. Therefore, we aimed to investigate the mechanisms of drug release and permeation in emulsion-type cataplasms from a structural perspective.Cataplasms with varied crosslinking densities, drug loadings, and drug types were prepared; drug release and permeation studies were performed using Franz diffusion cells,

Advances in MOF-Based Transdermal Drug Delivery Systems: Mechanisms, Applications, and Future Prospects.

Transdermal drug delivery offers a noninvasive alternative to conventional administration by avoiding gastrointestinal degradation and hepatic first-pass metabolism. However, its broader clinical application remains limited by the barrier function of the stratum corneum and insufficient drug accumulation within target tissues. Metal-organic frameworks (MOFs), characterized by their high surface area, tunable pore structures, and versatile surface chemistry, have recently emerged as promising platforms for transdermal drug delivery. This review summarizes the key advantages of MOF-based transde

Nanoformulations in Acne Therapy: Translational Opportunities and Barriers for Next-Generation Topical Treatments.

Acne vulgaris is highly prevalent and burdensome, yet conventional topical therapies are limited by poor stratum corneum penetration, follicular obstruction, low drug deposition at pilosebaceous targets, drug instability, local irritation/side effects, and variable patient adherence. This review synthesizes recent nanoformulation advances in the context of acne pathophysiology and the specific delivery barriers it creates. Lipid-based carriers (solid lipid nanocarriers (SLNs), nanostructured lipid carriers (NLCs), nanoemulsions (NEs)) and vesicular systems (liposomes, niosomes, transfersomes)

Deep-Penetrating Transdermal Lipopeptide Liposomes for Sustained IL-17 Inhibition and Prevention of Psoriatic Recurrence.

Psoriasis is a multisystemic, immune-mediated inflammatory disease characterized by high recurrence rates upon treatment cessation. Biotherapeutics are highly anticipated for treating psoriasis via transdermal delivery. However, the topical administration of potent immunomodulators is fundamentally hindered by the stratum corneum barrier, stringent molecular size constraints, and rapid proteolytic degradation within the skin microenvironment. Here, we report the development of transdermal lipopeptide liposomes (TLLs) designed for the site-specific delivery of a double-bridged cyclic tetradecap
