Effect of pharmacotherapy on serum cholesterol levels in patients with panic disorder.

It is unclear whether elevated cholesterol level is a complication of panic disorder (PD) or is associated with pharmacotherapy. We compared … icant decrease of TC after the treatment (t = 2.36, P < 0.03). The TC level in the PD subgroup with agoraphobia (n = 24, 198.9 +/- 37.9 mg/dl) was slightly higher than that in the group without agoraphobia (n = 23, 190.8 +/- 41.6 mg/dl). These findings suggest that there is a relationship between cholesterol levels and treatment in PD.

Cognitive and performance-based treatments for panic attacks in people with varying degrees of agoraphobic disability.

Compared the effectiveness of cognitive therapy and performance-based exposure as treatments for panic attacks. Subjects were 48 panicky ind … hobic disability had a significant bearing on panic treatment effectiveness. Whereas 94% of the low agoraphobia Ss were free of panic after treatment, only 52% of the high agoraphobia Ss became panic-free. The findings suggest that when panic treatment research excludes people with serious phobias, as it has routinely done in recent years, an overly positive …

Infrequent and limited-symptom panic attacks.

Due to their prevalence and relationship to agoraphobia, panic attacks are receiving considerable attention. The DSM-III requires that they include at least four associated symptoms and occur at least weekly for a diagnosis of panic disorder. This study of panic symptoms and frequency in 68 panic sufferers addresses the validity of these DSM-III requirements in terms of symptom patterns and phobic avoidance. Using chi-square, Fisher, and t-test analyses, this study found no clinical distinction between frequent (occurring at least weekly) and infrequent panic attacks. When comparing limited-sy…

Epidemiology of anxiety disorders in Florence.

A structured interview designed to diagnose anxiety disorders according to DSM-III (plus infrequent panic attacks and generalized anxiety disorder using DSM-III-R criteria) was given to 1110 people registered with 6 general practitioners (GPs), whether they consulted the doctor or not. As each citizen in Italy has to be registered with a GP, the sample was representative of the population. The interviews were carried out by the GPs, who were also third- or fourth-year trainees in psychiatry. The lifetime prevalence and point prevalence were: 0.36% and 0.27% for agoraphobia; 0.90% and 0.72% for…

Correlations of splitting and phobic anxiety with dreaming.

Dream characteristics of 28 women from a graduate counseling program were correlated with measures of phobic anxiety, splitting, and sleepiness. Significant correlations between splitting and recurrent nightmares (.68), agoraphobia and dreams about death (.44), and global phobia and recurrent nightmares (.56) were obtained. Results are discussed in terms of how phobic anxieties and splitting may relate to traumatic content and the dream process.

Comparison of the DSM-5 and ICD-10: panic and other anxiety disorders.

Anxiety disorders are the most prevalent mental disorders and are associated with substantial healthcare costs and a high burden of disease. In this article, changes in the new Diagnostic and Statistical Manual for Mental Disorders (the DSM-5) with respect to panic disorder/agoraphobia, generalized anxiety disorder, social anxiety disorder, specific phobias, and selective mutism are compared with the International Classification of Diseases (ICD-10) system.

[Isolated sleep paralysis in patients with disorders due to anxiety crisis].

The relationship between isolated paralysis and panic disorder in adults is analyzed in this study. The patients included in this work presented anxiety disorder with agoraphobia and were compared with a group of normal controls. The percentage of patients with sleep paralysis was 40% significantly higher than those encountered in the control group (20%). The existence of this syndrome do not seem to modify the clinical manifestation or severity of panic disorder.

The agoraphobic dental patient: report of case.

Agoraphobia is considered to be the most common, yet distressing, phobia for adult phobic patients. Little has been written concerning the identification, problems, and treatment of dental disease in the agoraphobic patient. After a review of the clinical features, epidemiology, causes, and medical treatment of the syndrome, a case study is presented, showing how dental treatment can be a valuable part of the behavioral treatment of this patient.

Paranoid and schizoid PDs were most strongly related to dysthymia, mania, panic disorder with agoraphobia, social phobia and generalized anxiety disorder, while histrionic and antisocial PDs were most strongly related to mania and panic disorder with agoraphobia. Results of this study highlight the need for further research on overlapping symptomatology …

The treatment of panic disorder in a clinical setting: a 12-month naturalistic study.

In the treatment of panic disorder double-blind controlled studies have demonstrated that imipramine (IMI) was effective at doses higher than 125 mg/day. However a high rate of dropouts because of side effects has been reported in these subjects. In clinical settings, the administration of benzodiazepines (BDZ) in combination with IMI has been proposed to reduce the frequency and severity of side effects. In this naturalistic study, 49 patients affected by panic disorder with agoraphobia (n = 36) or without agoraphobia (n = 13) were treated with IMI plus lorazepam (LRZ) and followed for 12 mon…

Unrecognized prevalence of panic disorder in primary care, internal medicine and cardiology.

Research over the last 25 years has delineated the syndrome of panic disorder/agoraphobia from a myriad of medical, cardiologic and psychiatric diagnoses. This syndrome is characterized by the sudden onset of episodes of panic and terror, accompanied by extreme physiologic symptoms including palpitations, tachycardia, chest pain, shortness of breath, trembling, faintness, etc. These patients become quite anxious and hypochondriacal and begin to avoid certain situations in which they feel a recurrence of a panic attack would be dangerous or embarrassing. This avoidance (agoraphobia) typically i…

Predictors of outcome in residential cognitive and interpersonal treatment for social phobia: do cognitive and social dysfunction moderate treatment outcome?

BACKGROUND: The predictors of residential cognitive (RCT) and residential interpersonal Treatment (RIPT) for social phobia were explored. (1 … ects with poor general functioning were less improved following treatment. Subjects with concurrent agoraphobia responded better with RCT than subjects without agoraphobia. (2) Age of onset and expectations were the most powerful predictors of post treatment outcome. CONCLUSION: Some patient characteristics appear to impact outcome with RIPT and RCT differen …

Internet-based treatment for panic disorder: does frequency of therapist contact make a difference?

Internet-based interventions with therapist support have proven effective for treating a range of mental health conditions. This study examined whether frequency of therapist contact affected treatment outcomes. Fifty-seven people with panic disorder (including 32 with agoraphobia) were randomly allocated to an 8-week Internet-based cognitive behavioural treatment intervention (Panic Online) with either frequent (three e-mails per week) or infrequent (one e-mail per week) support from a psychologist. Posttreatment, intention-to-treat analyses revealed that both treatments were effective at imp…

When remote analysis is used as an attack on the frame.

Henri Rey's theory of claustro-agoraphobia is helpful for the understanding of the difficulties that can arise when the frame of treatment is modified in remote analysis or psychoanalytic psychotherapy. The felt intense closeness and the actual distance of remote analysis lend themselves to a defensive position in remote analysis when the patient feels at once trapped in the analysis and yet unable to be without the analysis. I suggest here that such claustro-agoraphobic fears arise particularly at times in the analytic process in which change is imminent. The patient is then confronted with g…
