Marginal bone stability using 3 different flap approaches for alveolar split expansion for dental implants: a 1-year clinical study.

PURPOSE: Width augmentation for the alveolar process using alveolar split procedures has not been studied in a comparative study with regard to marginal bone stability. Most research in this regard has used implant osseointegration as an endpoint for the success of the bone grafting procedure. The purpose of the present clinical trial was to retrospectively evaluate the stability of the buccal crestal bone around dental implants placed into alveolar split graft sites using 3 different approaches: a minimal flap, a partial-thickness flap, and a full mucoperiosteal flap. Implants were placed sim

Evaluation of Maxibloc Bone Grafts Biocompatibility and Osteoconductivity: An Animal Study.

CONTEXT: Alveolar bone resorption poses a significant challenge in rehabilitating edentulous patients. Ridge augmentation materials can stimulate bone growth in deficient areas. AIM: To evaluate the biocompatibility and osteoconductivity of maxibloc bone grafts in rabbits. MATERIALS AND METHODS: Maxibloc was packed into bony voids created surgically in the mandible and femur of rabbits, which were sacrificed at the fourth, eighth, and twelfth weeks. Radiography and histopathology evaluations assessed the osteoconductivity; hematology and toxicology evaluations assessed the biocompatibility o

Bioactive-glass in periodontal surgery and implant dentistry.

Bioactive-glass (B-G) is a material known for its favorable biological response when in contact with surrounding fibro-osseous tissues, due not only to an osteoconductive property, but also to an osteostimulatory capacity, and superior biocompatibility for use in human body. The objectives of this paper are to review recent studies on B-G in periodontal and implant therapy, describing its basic properties and mechanism of activity as well as discoursing about state of art and future perspective of utilization. From a demonstrated clinical benefit as bone graft for the elimination of osseous de

Maxillary lateral incisor implants: planning with the aid of orthodontics.

Implants are commonly used to replace congenitally missing lateral incisors in adolescent orthodontic patients. However, these restorations are often challenging for the orthodontist, surgeon, and restorative dentist. In some patients the space across the alveolar crest is too narrow to permit the surgeon to place the implant. Occasionally the root apices of the adjacent central incisor and canine are in close proximity. In other cases the ridge thickness could be inadequate and require augmentation. When the orthodontist opens the space, the papilla heights are adversely affected. Some adoles

The use of freeze-dried rib and hydroxylapatite in the treatment of a fracture occurring in a patient with familial facial osteodystrophy.

The treatment of repeated pathologic fractures of the mandible has long taxed the ingenuity of surgeons. The complexity of the problem increases with the recognition of a metabolic defect that renders the facial bones hypoplastic. In the case presented, a tentative diagnosis of familial facial osteodystrophy was made at the National Institutes of Health. Since that time, the patient has undergone autogenous onlay grafting, which has completely resorbed. Because of continuous resorption, a number of pathologic fractures have occurred. The most recent fracture was treated by means of a freeze-dr

Clinical, radiographic, and histological analyses of calcium phosphate cement as filling material in maxillary sinus lift surgery.

BACKGROUND: The installation of dental implants in the posterior maxilla is often faced with resorbed alveolar processes, resulting from a combination of pneumatization of the maxillary sinus, the effects of periodontal disease, and physiological bone resorption. The sinus lift surgery has been practiced since 1980 with the aim to increase bone height in this region for an implant supported prosthetic rehabilitation, and various filling materials have been used for such. OBJECTIVES: This study aimed to clinically, radiographically, and histologically evaluate a preparation of calcium phosphat

Particulate Bone Grafts vs Split Bone Cortical Plates: Resorbable and Nonresorbable Membranes in Vertical Augmentation.

This study aimed to evaluate vertical bone gain in the anterior edentulous mandible using the split cortical bone plate technique with 2 different graft and membrane combinations: autogenous particulate grafts with titanium-reinforced polytetrafluoroethylene membrane (T1) and a mixture of autogenous and xenograft particulate grafts with a collagen membrane (Bio-Gide) (T2). Twenty-two patients presenting with severe vertical alveolar ridge atrophy were randomly allocated into 2 equal groups with 11 participants in each group. Cone beam computerized tomography was used to assess vertical bone ga

Acellular dermal matrix allograft. The results of controlled randomized clinical studies.

The aim of this presentation was to discuss the effectiveness of the acellular dermal matrix in root coverage therapy and in alveolar ridge augmentation, based on three controlled randomized clinical trials conducted by our research team (Novaes Jr et al., 2001; Barros et al., 2005; Luczyszyn et al., 2005). The first and second studies highlight the allograft's performance in the treatment of gingival recession. In both studies, clinical parameters were assessed prior to surgery and 6 or 12 months post-surgery. The first one compared the use of the acellular dermal matrix with the subepithelia…

Mucoperiosteal Flap Healing During Vertical Bone Augmentation using Titanium Mesh: A Study in Beagle Dogs.

INTRODUCTION AND AIMS: The periosteal releasing incision (PRI) is often used in guided bone regeneration (GBR) with titanium (Ti) mesh to reduce the tension of mucoperiosteal flaps (MPF) and close the wound, but it can easily lead to keratinized gingiva (KG) narrowing and a high wound dehiscence rate. Therefore, it is necessary to explore alternative methods such as open healing. The aim of this study was to observe whether a MPF without a PRI and primary closure could heal on a titanium membrane surface in vertical bone augmentation and to measure the change in KG width. METHODS: The bilater

Ramus Frame Dental Implant Functional Survival and Safety in Severely Atrophic Edentulous Human Mandibles.

Ramus frame dental implants were retrospectively studied in 360 adults with severely atrophic edentulous mandibles. Patient records up to 12 years post-treatment were independently reviewed after a single clinician surgically placed titanium long-arm ("Tatum") ramus frame implants and immediately loaded them with a mandibular overdenture. A total of 11 ramus frames were removed at 19 to 109 months post-treatment, mostly due to supramucosal bar fracture (N = 6) or mobility (N = 3). Kaplan-Meier product-limit analysis revealed the post-treatment survival probability for functional ramus frame im

The use of 3D models to improve sinus augmentation outcomes - A case report.

Sinus augmentation is a predictable procedure that is often required when restoring the posterior maxilla with dental implants. Even with high success rates, careful pre-surgical planning is crucial. A 3D model is a valuable aid for the clinician as it allows for pre-operative simulation, which can reduce surgical time, reduce the risk of intra-operative complications and decrease the potential for error. The aim of this case report is to focus on how such a model is useful when undertaking a sinus augmentation procedure with simultaneous implant placement.

Complications Associated With the Use of Recombinant Human Bone Morphogenic Protein-2 in Ridge Augmentation: A Case Report.

This case report aims to describe in detail a complication associated with resorption of regenerated bone following implant placement and ridge augmentation using recombinant human bone morphogenic protein-2 (rhBMP-2) in combination with allograft and xenograft. Bilateral maxillary sinus and ridge augmentation procedures were completed using rhBMP-2 combined with allograft and xenograft. Five months later, significant bone augmentation was achieved, which allowed for the placement of 4 implants. Upon stage 2 surgery, significant dehiscence was noted in all implants. Treatment steps to address

Flapless alveolar ridge preservation utilizing the "socket-plug" technique: clinical technique and review of the literature.

It has been documented that after every extraction of one or more teeth, the alveolar bone of the respective region undergoes resorption and atrophy. Therefore, ridge preservation techniques are often employed after tooth extraction to limit this phenomenon. The benefits of a flapless procedure include maintenance of the buccal keratinized gingiva, prevention of alterations to the gingival contours, and migration of the mucogingival junction that are often experienced after raising a flap. The purpose of this article is to review the literature concerning flapless ridge preservation techniques

Vertical ridge augmentation of atrophic posterior mandible using an inlay technique with a xenograft without miniscrews and miniplates: case series.

BACKGROUND: Rehabilitation of partially or totally edentulous posterior mandible with implant-supported prosthesis has become a common practice in the last few decades, with reliable long-term results. The use of miniscrews and miniplates have been reported to increase the risk of fracture of the osteotomy segments. The purpose of this case series was to use an inlay technique, without the use of miniscrews and miniplates for stabilization of the transported bone fragments. MATERIALS AND METHODS: Nine consecutive patients (six men and three women) aged between 26 and 51 years (mean 44 years)
