Copper deficiency is a rare yet reversible cause of anemia and bone marrow dysplasia that can mimic myelodysplastic syndrome (MDS), especially in patients on maintenance dialysis, who are at increased risk due to trace element losses, chronic inflammation, and impaired absorption. We report a 69-year-old woman on long-term hemodialysis with a history of partial gastrectomy who developed macrocytic anemia refractory to erythropoiesis-stimulating agents (ESAs). Bone marrow examination revealed erythroid dysplasia, hypocellularity, and ring sideroblasts. Although serum copper levels were initiall

We report a case of a 51-year-old woman with Evans syndrome (autoimmune hemolytic anemia and primary immune thrombocytopenia) and hypothyroidism. She was previously diagnosed with Hashimoto's thyroiditis in 1994 (age, 35) and autoimmune hemolytic anemia (AIHA) 3 years ago. She was treated with oral prednisolone. After a period, in which the anemia waxed and waned, there was an abrupt development of thrombocytopenia (nadir 15×10(9)/L) that coincided with the tapering off of prednisolone after 3 years of administration. Because her thrombocytopenia was refractory to prednisolone, we administered

A 6-month-old full-term infant had severe anemia and neutropenia. The patient was being fed cow's milk and a diet of corn flour. Thorough investigation revealed low serum iron concentration, severe hypocupremia, low ceruloplasmin, retardation of bone age, and metaphysial irregularities and spurring. Bone marrow aspirate revealed cytoplasmic vacuolization in precursors of the erythroid and myeloid series and ringed sideroblasts. Therapy with oral iron, folic acid, and vitamin C was futile. Administration of copper sulfate resulted in brisk increase in neutrophils and reticulocytes. The child ma

RATIONALE: Waldenström macroglobulinemia (WM) is a type of immunoglobulin M (IgM)-secreting lymphoplasmacytic lymphoma primarily caused by the accumulation of clonal lymphocytes, lymphoplasmacytes, and plasma cells. The monoclonal IgM proteins secreted by these cells result in high blood viscosity.
PATIENT CONCERNS: A 79-year-old male presented with anemia lasting 10 months accompanied by edema in both lower limbs and testicles.
DIAGNOSES: After a series of tests, including serological testing, serum protein electrophoresis, and genetic testing, the patient was ultimately diagnosed with WM.

A 68-year-old woman with essential thrombocythemia had been treated with hydroxycarbamide and aspirin for 13 years. She exhibited the rapid progression of anemia, and a bone marrow examination showed dysplasia of the erythroid cells, myeloid cells, and megakaryocytes. Karyotype analysis indicated complex abnormalities including der (5;21)(p10;q10). She was diagnosed with myelodysplastic syndrome (MDS), refractory anemia with excess blasts-1 (RAEB-1). Lenalidomide was started, but no improvement in anemia was recorded. Lenalidomide was discontinued due to eosinophilia, basophilia, and a skin ra

Acquired pure red cell aplasia (PRCA) is a rare syndrome characterized by normocytic normochromic anemia with severe reticulocytopenia and absence of erythroid precursors in the bone marrow. For refractory PRCA patients, the low response rate and high toxicity of alternative therapies pose a great challenge. T-cell large granular lymphocyte (T-LGL) leukemia is one of the most common conditions in secondary PRCA and also the most difficult form to manage with an inferior treatment response to other secondary PRCA forms. T-LGL leukemia exhibits sustained activation of the intracellular JAK-STAT

There are now numerous case reports and small series using rituximab in autoimmune diseases. While these data must be interpreted with caution, they suggest that rituximab (RTX) may be a promising addition to the therapeutic armamentarium. In patients with refractory chronic idiopathic thrombocytopenic purpura, treatment with RTX was effective in inducing complete responses in a significant proportion of patients, and these responses were usually durable. RTX has also been shown to be effective and well tolerated in children with refractory autoimmune hemolytic anemia. In patients with IgM ant

Infective endocarditis (IE) complicated by non-immune hemolytic anemia is a rare but clinically significant presentation, particularly in native valve disease, and often reflects severe valvular destruction leading to mechanical red blood cell fragmentation. Patients with end-stage renal disease (ESRD) on hemodialysis are at increased risk of bloodstream infections due to chronic vascular access, making early recognition essential. We report the case of a 36-year-old man with ESRD on hemodialysis via an arteriovenous (AV) graft who presented with acute dyspnea, hypoxia, and severe transfusion-

A 9-year-old girl who had hepatitis-associated aplastic anemia was treated intermittently with methylprednisolone pulse therapy and growth factors (granulocyte-colony stimulating factor (G-CSF), recombinant human erythropoietin (rhEpo) and cyclosporin A (CyA) for over two years. At this time, there was hematological improvement, but chromosome analysis revealed monosomy 7. After six months, there was progression to myelodysplastic syndrome (MDS) (stage in refractory anemia of excess blasts (RAEB)) with monosomy 7, monosomy 6, marker chromosome and with hematological deterioration. She received

Iron overload, characterized by excessive iron deposition, occurs commonly in patients with hereditary or refractory anemias such as beta-thalassemia major, sickle cell anemia, and myelodysplastic syndromes, whose anemia is managed with frequent blood transfusions. Without adequate iron chelation therapy, almost all patients with beta-thalassemia will accumulate potentially fatal iron levels. Myocardial siderosis and resulting cardiac complications are among the leading causes of death in such patients. Unfortunately, even with the administration of effective subcutaneous iron chelation therap

We studied spontaneous cytokine production by peripheral blood mononuclear cells (PBMC) obtained from 14 patients with aplastic anemia (AA) and 28 various myelodysplastic syndromes (MDS). The levels of interleukin-6, interleukin-1 beta, and tumor necrosis factor-alpha in cultured PBMC were measured by ELISA. The average levels of these cytokines were higher in AA or in refractory anemia (RA) than in RA with excess of blasts (RAEB) or in RAEB in transformation (RAEB-T). Marked cytokine overproduction was observed in RA as well as in AA. High cytokine levels were observed in hypocellularity and

OBJECTIVE: To present a case of brucellosis-induced severe autoimmune hemolytic anemia (AIHA) that was refractory to traditional corticosteroid treatment and eventually treated with rituximab apart from antibiotic therapy and to discuss the potential role of rituximab in similar cases of AIHA triggered by an underlying reversible cause.
CASE SUMMARY: A 79-year-old woman was diagnosed with severe AIHA (reticulocyte count 21.5%, hemoglobin 6 g/dL). Initial treatment with prednisone in a regional hospital was not efficacious. Brucellosis was diagnosed by serology; the disease was further complic

While a majority of patients with refractory anemia with ring sideroblasts and thrombocytosis harbor JAK2V617F and rarely MPLW515L, JAK2/MPL-negative cases constitute a diagnostic problem. 23 RARS-T cases were investigated applying immunohistochemical phospho-STAT5, sequencing and SNP-A-based karyotyping. Based on the association of TET2/ASXL1 mutations with MDS/MPN we studied molecular pattern of these genes. Two patients harbored ASXL1 and another 2 TET2 mutations. Phospho-STAT5 activation was present in one mutated TET2 and ASXL1 case. JAK2V617F/MPLW515L mutations were absent in TET2/ASXL1

A patient with refractory anemia with excess blasts (RAEB) and pancytopenia is reported. He was red blood cell (RBC)-transfusion dependent. Karyotype analysis showed a complex cytogenetic abnormality consisting of loss of chromosome 7 and trisomy 8. Without cytotoxic treatment, his complete blood count (CBC) subsequently became normal. He is no longer transfusion dependent and repeat cytogenetic analyses are normal.

Thalidomide is effective in inducing and maintaining clinical remission, as well as mucosal healing, in patients with refractory Crohn's disease (CD). However, long-term use of thalidomide has raised concern because of the high incidence of adverse events. Cardiovascular events induced by thalidomide have been reported in patients with multiple myeloma, amyotrophic lateral sclerosis, and transfusion-dependent refractory anemia. We report here an extremely rare case of sinus bradycardia induced by thalidomide in an adult patient with CD. This patient's heart rate converted back to a normal sinu

Recently, 5-azacitidine has been reported to improve the survival of patients with high-risk myelodysplastic syndrome (MDS) and was approved for the treatment of MDS in Japan. We herein report a case of high-risk MDS in which the patient exhibited a hematological improvement three months after the first cycle of 5-azacitidine therapy. The second cycle of 5-azacitidine was not administered due to a severe pulmonary infection. Bone marrow aspiration revealed a decrease in the level of blast cells from 7.0% to 0.7%, and the subclassification of MDS improved from refractory anemia with excess blas

BACKGROUND: As an initial therapy for Kawasaki disease, high-dose intravenous immunoglobulin (IVIG) is recommended. It is known that intravenous immunoglobulins are associated with hemolytic anemia. However, little is known about hemolytic anemia in Kawasaki disease (KD) patients treated with IVIG. This study was designed to investigate the incidence of hemolytic anemia and factors associated with the development of hemolysis in KD patients treated with IVIG.
METHODS: We performed a 3-year single-center prospective study enrolling 105 KD patients treated with IVIG from January 2021 to May 202

BACKGROUND: Erythropoiesis is a highly regulated and well-characterized developmental process responsible for providing the oxygen transport system of the body. However, few of the mechanisms involved in this process have been elucidated. Checkpoint Kinase 1 (Chk1) is best known for its role in the cell cycle and DNA damage pathways, and it has been shown to play a part in several pathways which when disrupted can lead to anemia.
METHODOLOGY/PRINCIPAL FINDINGS: Here, we show that haploinsufficiency of Chk1 results in 30% of mice developing anemia within the first year of life. The anemic Chk1

Accelerated programmed cell death or apoptosis appears to play an important role in the pathogenesis of myelodysplasia. As overexpression of TNF-alpha has been described to induce cell death in myelodysplasia, treatment with anti-TNF-alpha is currently being explored. Caution is needed because of an increased risk of opportunistic infection during anti-TNF-alpha treatment. We here describe a patient who was treated with anti-TNF-alpha for low risk myelodysplasia and died of invasive mucormycosis.
