Minimising negative ankle and foot X-rays in the Emergency Department -- are the Ottawa ankle rules good enough?

OBJECTIVES: To review fracture rates in patients who had X-rays of either the ankle or foot or both in the Emergency Department of a district hospital. METHOD: A retrospective review of 2589 Emergency Department patients who had X-rays of the ankle or foot or both over a 12-month period. RESULTS: There were 1199 ankle X-rays taken of which 193 (16%) showed a significant fracture. There were 1081 patients who had a foot X-rayed of which 165 (15%) showed a significant fracture. Younger patients had more X-rays but older patients were more likely to have a fracture. There were 309 patients who …

Effect of supramalleolar osteotomy and total ankle replacement on talar position in the varus osteoarthritic ankle: a comparative study.

BACKGROUND: In varus osteoarthritic ankles, joint congruency is usually lost leading to progressive wear of the medial tibiotalar joint. Recent studies have shown that balancing the hindfoot with the aid of supramalleolar osteotomy (SMOT) is an effective method to decrease symptoms and to delay progression of osteoarthritis of the ankle joint. Resurfacing the articular surfaces with total ankle replacement (TAR), in contrast, may compensate for the lost joint congruency and lost stability of the talus at the peritalar joint. However, no literature exists with regard to the overall correction o…

Appearance of reciprocal facilitation of ankle extensors from ankle flexors in patients with stroke or spinal cord injury.

The purpose of the present study was to investigate the pathophysiological role of reciprocal facilitation between antagonistic motoneuron pools in spasticity. The soleus H-reflex was conditioned by prior stimulation of the peroneal nerve in 15 healthy subjects, six hemiplegic patients and 11 spinal cord injured (SCI) patients. The hemiplegic patients were tested from soon after the onset of hemiplegia and up to 2 years later. Whereas stimulation of the peroneal nerve produced short-latency inhibition of the soleus H-reflex in healthy subjects, it produced facilitation in spastic SCI and hemip

Short-Foot Exercises Improve Postural Control and Ankle Instability in Chronic Ankle Instability: A Randomized Controlled Trial.

CONTEXT: Chronic ankle instability (CAI) is associated with weakness and atrophy of the intrinsic foot muscle. Short-foot exercise (SFE) selectively activates the intrinsic foot muscle and has been proposed as a therapeutic intervention. However, evidence supporting its effectiveness in individuals with CAI remains limited. OBJECTIVE: To examine the effects of a 4-week SFE program on static and dynamic postural control and subjective ankle instability in individuals with CAI. DESIGN: Randomized controlled clinical trial. SETTING: Laboratory. PATIENTS OR OTHER PARTICIPANTS: We randomly allo…

Impact of Fatiguing Exercises on Movement Strategies in Chronic Ankle Instability, Lateral Ankle Sprain Copers, and Controls.

While research exists to induce fatigue using isokinetic dynamometers or simple repetition tasks in patients with chronic ankle instability (CAI), there is a lack of research examining landing movement strategies using fatigue protocols that mimic actual sports. Therefore, we aimed to investigate the effects of CAI and fatiguing exercises on the lower-extremity kinematics and kinetics during single-leg drop landings among patients with CAI, lateral ankle sprain (LAS) copers and controls. A cross-sectional study recruited 20 patients with CAI, 20 LAS copers, and 20 controls in a biomechanics la…

The Manchester Charcot Ankle Neuroarthropathy (M-CAN) Classification: A Radiographic-Based System for Assessing Charcot Ankle Neuroarthropathy.

Background Charcot neuroarthropathy (CN) is a complication of long-standing, poorly controlled diabetes, associated with increased risks of amputation and mortality. Currently, there is no dedicated classification system specifically for ankle CN, nor a standardized description of the possible deformities involving the ankle in relation to CN. This study aims to develop and propose the Manchester Charcot Ankle Neuroarthropathy (M-CAN) classification - a descriptive, radiograph-based system designed to categorize ankle deformities and patterns of bone loss in patients with diabetic CN. The M-CA…

Percutaneous Calcaneal Osteotomy Combined With Arthroscopic Lateral Ankle Ligament Reconstruction for Chronic Ankle Instability With Hindfoot Varus.

Chronic ankle instability is the most frequent complication of lateral ankle sprain. Its reported incidence is approximately 40% after the first episode of instability. Although this rate varies depending on the type of activity, there are also certain risk factors associated with this condition, such as hyperlaxity, static or dynamic postural control deficits, and especially, hindfoot varus. If hindfoot varus is not managed when medical treatment fails and surgery is necessary, treatment may be unsuccessful, resulting in a poorer functional outcome and a higher rate of recurrent instability. …
