A variety of methods for maintaining pupillary dilation for intraocular surgery have been advocated because pharmacologic agents may fail to maintain or adequately dilate the pupil. We have used a threaded needle to rapidly dilate the pupils of 19 aphakic, pseudophakic, or phakic eyes without damaging the lens or pseudophakos. Congential ptosis associated with aniridia was found in 3 patients from 2 pedigrees. Nonsurgical aphakia, a condition not previously reported in association with aniridia, was also found in one case. The association of aniridia and congenital ptosis is suggested as evidence for the common mesodermal etiology of both anomalies.

To evaluate the results of congenital cataract extraction and postoperative visual function, we retrospectively reviewed the records of 95 patients who underwent pars plana (plicata) lensectomy or aspiration surgery. A 3-year-old child with posttraumatic aphakia and a corneal scar was treated with a rotational penetrating ipsilateral autokeratoplasty followed by epikeratophakia 2 months later. Results of surgical treatment of secondary glaucoma (caused by uveitis, aphakia, injury) in adults and children are presented. Topical and general corticosteroids were applied immediately after trabeculectomy (deep sclerectomy).
