PURPOSE: To evaluate the outcome of microkeratome-assisted posterior lamellar keratoplasty for replacing diseased endothelium in pseudophakic and aphakic corneal edema.

PURPOSE: To investigate whether central corneal thickness (CCT), endothelial cell characteristics, and intraocular pressure (IOP) are affected in patients with previous congenital cataract surgery and to focus on their clinical significance.
METHODS: CCT and IOP measurements and specular microscopy were performed in 31 eyes of 17 cases of extracted congenital cataracts and 40 eyes of 20 age- and sex-matched participants as control group. We received approval from the U.S. implant Food and Drug Administration and the University's Institutional Review Board to the Artisan lens (Ophtec BV) in both eyes of a patient who was aphakic and had megalocornea. No other intraocular lens would easily solve this patient's need because of the large anterior segment. The patient was having increasing difficulty with aphakic contact lenses because of his work environment. We present almost 5 years of follow-up data of this patient.

PURPOSE: To evaluate the results of cataract surgery in children with IOL implantation.
MATERIALS AND METHODS: Our research involved 106 children (156 eyes) with congenital and traumatic cataract, in age from 18 months to 18 years, who underwent cataract surgery with intraocular lens implantation. We evaluated visual acuity for far vision and binocular vision after surgery.
RESULTS: Visual acuity over 0.5 in unilateral congenital cataract was found in 27.25%, in traumatic cataract in 73%, whereas in bilateral congenital cataract visual acuity over 0.5 we received in 75% of operated children.

Bilateral congenital cataract is the most common cause of treatable childhood blindness. Nuclear cataract is usually present at birth and is nonprogressive, whereas lamellar cataract usually develops later and is progressive. Surgery must be performed promptly in cases with dense congenital cataract; if nystagmus has developed, the amblyopia is irreversible. BACKGROUND: To identify risk factors related to the onset of secondary glaucoma in eyes that have undergone pars plana lensectomy for congenital cataract.
METHODS: We investigated intraocular pressure levels after pars plana lensectomy for congenital cataract in 80 eyes of 44 patients who were followed up for at least 36 months. Mean age at the time of pars plana lensectomy was 11+/-18 months. Mean follow-up period was 116+/-57 months.

An aphakic patient with severe chronic hypotony had an alternative treatment of a traumatic cyclodialysis cleft: a 3-port pars plana vitrectomy, cryotherapy of the cleft, and fluid-gas exchange with subsequent supine positioning. The therapeutic principle was mechanical apposition of the detached ciliary muscle to the scleral spur by the gas bubble and scar induction by cryotherapy. Intraocular pressure increased to within normal ranges, and visual acuity improved over a 15 month follow-up.

Forty one patients aged 29 to 82 years were subjected to cataract extraction in ambulatory conditions. We measured three aspects of vision in children treated for unilateral congenital cataract: visual resolution, the symmetry of optokinetic nystagmus (OKN), and peripheral vision. We performed epikeratophakia for the correction of aphakia in 31 children, ages 2 months to 7 years, who had unilateral congenital or traumatic cataracts. In 27 patients with more than 4 months of follow-up, 23 of 30 (77%) grafts were successful. In the 15 patients with 6 months or more followup, the average increase in corneal curvature was 12.39 diopters.
