Case study: diagnosis and operative management of de Garengeot hernia without appendectomy during the COVID-19 pandemic.

de Garengeot herniae have been reported in <100 cases in literature. They are characterized by an incarcerated femoral hernia containing the … ve a clinical suspicion for de Garengeot herniae for patients, presenting with a right groin bulge. Appendectomy may be safely avoided, eliminating appendectomy-associated morbidity and avoiding hospital transfer and the associated risk of COVID-19 exposure.

Language-Related Disparities in Pain Management in the Post-Anesthesia Care Unit for Children Undergoing Laparoscopic Appendectomy.

Race and ethnicity are associated with disparities in pain management in children. While low English language proficiency is correlated with minority race/ethnicity in the United States, it is less frequently explored in the study of health disparities. We therefore investigated whether English language proficiency influenced pain management in the post-anesthesia care unit (PACU) in a cohort of children who underwent laparoscopic appendectomy at our pediatric hospital in San Francisco. Our primary exposure was English language proficiency, and our primary outcome was administration of any opi…

Associations Between Sonographic Findings and Operative Time of Transumbilical Laparoscopic-Assisted Appendectomy for Acute Appendicitis in Children.

OBJECTIVE. The purpose of this study is to evaluate the association between sonographic findings and the operative time of transumbilical laparoscopic-assisted appendectomy (TULAA) for appendicitis in children. MATERIALS AND METHODS. We included 131 children who underwent ultrasound within 24 hours before TULAA. We evaluated the associations between operative time and patients' demographics, as well as the following sonographic findings: maximum outer wall diameter of the appendix, appendicolith, ascites (not echogenic), echogenic ascites, abscess formation, increased intraabdominal fat echo i…

Feasibility and Efficacy of Using Hem-o-lok Polymeric Clips in Appendicular Stump Closure in Laparoscopic Appendectomy.

Laparoscopic appendectomy is becoming the gold standard for the management of acute appendicitis. Appendicular stump closure is a critical step in this procedure, and several methods have been used to secure the appendicular stump during laparoscopic appendectomy. We highlight the feasibility and efficacy of 'Hem-o-lok' polymeric clips in securing the base of the appendix. We selected 20 consecutive patients who underwent laparoscopic appendectomy using a single or double 'Hem-o-lok' clip to secure the base of the appendix for acute appendicitis from October 2011 to August 2013. Thirteen study…

Midterm outcome of transumbilically laparoscopic-assisted versus laparoscopic and open appendectomy in children--a matched prospective study.

BACKGROUND: Prospective data of transumbilically laparoscopic-assisted appendectomies (TULAA) is absent in the pediatric population. We therefore compared the midterm outcome of TULAA with open (OA) and laparoscopic (LA) appendectomies in children with appendicitis in a matched prospective study. METHODS: A total of 20 patients operated with TULAA were matched to 20 cases operated by LA and OA, respectively, according to sex, age, and histology of the resected appendix. All 60 children were evaluated during a 3-month follow-up visit. RESULTS: The subjective pain level after discharge, the ra

Multiple pyogenic liver abscesses formed after appendectomy: the role of percutaneous drainage in a critically ill patient.

UNLABELLED: Multiple pyogenic liver abscesses formed after appendectomy and their percutaneous treatment with multiple catheters have been rarely described. We report a case of multiple pyogenic liver abscesses in a critically ill patient, formed after appendectomy and treated successfully by antibiotics and drainage with six catheters that were introduced simultaneously under ultrasound control. Even though this was a case of liver abscess secondary to appendicitis, today very rare in Western countries, but still a serious complication in developing countries, it was successfully resolved by …

Prevention of infectious complications after laparoscopic appendectomy for complicated acute appendicitis--the role of routine abdominal drainage.

PURPOSE: Complicated acute appendicitis is still associated with an increased morbidity. If laparoscopy has been accepted as a valid approach, some questions remain concerning intra-abdominal abscess formation. Routine prophylactic drainage of the abdomen has been proposed. However, this practice remains a matter of debate, poorly validated in the literature. With the present study, we investigated the impact of drainage in laparoscopic appendectomy for complicated appendicitis. METHOD: This is a case match study of consecutive patients operated on by laparoscopy in a single institution. One …

[The ilioinguinal and iliohypogastric nerves. The anatomic bases in preventing postoperative neuropathies after appendectomy, inguinal herniorraphy, caesareans].

STUDY AIM: To identify danger areas in surgical incisions (appendectomy, inguinal and Pfannenstiel incisions) by mapping the course of ilioinguinal and iliohypogastric nerves. MATERIAL AND METHODS: The courses of the ilioinguinal and iliohypogastric nerves from 37 unembalmed adult cadavers were mapped from their lateral emergence on the internal obliquus nerve to their midline termination in reference to anatomic landmarks (anterior superior iliac spine, midline, upper border of the pubic symphysis). With use of a mapping technique, the standard courses of both nerves were compared with sites…

Timing of laparoscopic appendectomy and impacts on outcomes: A retrospective study of patients across in-hours, after-hours and holidays.

BACKGROUND: Acute appendicitis is a common surgical emergency, and appendectomy is essential to prevent complications such as perforation and peritonitis. Although the procedure is routine, outcomes may be affected by surgery timing, staffing, and hospital resources. Evidence on whether off-hour operations increase complications is conflicting. This study aimed to assess whether laparoscopic appendectomy performed during weekday in-hours, after-hours, or holiday periods influences postoperative outcomes. AIM: To investigate clinical outcomes of laparoscopic appendectomy performed during weekd…

Dissecting the potential impact of appendectomy on colorectal cancer through metabolites and inflammatory proteins: a Mendelian randomization study.

INTRODUCTION: Traditional longitudinal epidemiological investigations have demonstrated an association between appendectomy and the cumulative incidence of colorectal cancer (CRC). However, there is a paucity of evidence for a causal relationship between appendectomy and CRC from alternative methodologies. MATERIAL AND METHODS: We extracted summary statistics for CRC, appendectomy, 1,400 metabolites, and 91 inflammatory proteins from the largest European ancestry genome-wide association studies (GWAS) and the FinnGen study. Using two-sample bidirectional Mendelian randomization (MR), we explo…

Amyand's hernia complicated with appendicitis: an interval approach with transabdominal laparoscopic appendectomy and laparoscopic hernioplasty-a case report.

Amyand's hernia is a rare classification of inguinal hernia where the vermiform appendix is located within the hernial sac. This case report discusses a patient presenting with abdominal and groin pain shown to be an Amyand's hernia complicated by acute appendicitis on computed tomography. The patient was treated with a two-stage approach, involving transabdominal laparoscopic appendectomy and elective laparoscopic hernioplasty after recovery. As some controversy exists regarding the timing of appendectomy and hernioplasty, we examine the benefits of the interval approach in the reduction of p…
