Tuberculous arthritis of sacroiliac joint with abscess formation: a case report.

Extrapulmonary tuberculosis at times appears to be an unusual presentation of the disease. Its incidence has not decreased, though a slow decline in pulmonary tuberculosis has been observed over the past decades around the world. The diagnosis of extrapulmonary tuberculosis is still rather difficult today. Tuberculosis of the bones and joints, which constitutes a smaller part of extrapulmonary tuberculosis, is relatively rare in documented cases and can yield variable clinical manifestations. This study describes a young male patient who developed a rare condition of tuberculous arthritis of t

Brucellar prosthetic arthritis in a total knee replacement.

The infecting pathogens most commonly implicated in prosthetic joint infections are staphylococci, streptococci, and gram-negative rods. Prosthetic infections caused by Brucella spp. are rarely described in the literature. Treatment of prosthetic infections remains complex and controversial, the most accepted course being antibiotic treatment with removal or retention of prosthetic components. The case of a 60-year-old man who developed Brucella septic arthritis of the right knee in a total knee replacement is reported. Conservative treatment using a three-drug therapy was employed, with excel

Reactive arthritis associated with Shigella dysenteriae type 1 infection.

Shigella dysenteriae type 1 causes the most severe form of bacillary dysentery. The spectrum of illness ranges from mild watery diarrhoea to severe bloody diarrhoea. Shigellosis is often associated with intestinal complications, including intestinal perforation, intestinal obstruction, toxic dilatation of the colon, and prolapse of the rectum; systemic complications include septicaemia, hyponatraemia, hypoglycaemia, seizure, encephalopathy, haemolytic-uraemic syndrome, and malnutrition. Arthritis and conjunctivitis are rare extra-intestinal complications of shigellosis. Annually, about 110,000

[Non-specific sacroiliitis].

We present retrospectively five cases of non-specific sacroileitis which were established and treated in Special Orthopaedic Hospital Banjica Belgrade in the period of 12 years, from 1982 to 1994. Patients have been from 17 to 27 years of age. All of them had been treated initially in other institutions under different diagnosis: specific sacroileitis, specific coxitis, specific spondylitis, abscess of gluteal region and discarthrosis and lumboishialgia. All of the patients have been treated by antibiotics, one by tuberculostatics, one surgically by drainage and one by coxofemoral spica cast.

Transient regional osteoporosis presenting as a septic arthritis.

A 52 year old man developed a painful swollen left knee. Clinically there was an effusion and radiographs showed local osteoporosis. Sepsis was suspected but tests proved negative and the symptoms gradually settled. During the course of the following year he developed similar pain migrating from the left foot to the right knee then to the right foot. Investigations showed a right knee effusion and 'punctate' radiographic lucencies in the patellae thought to be erosions. A bone scan demonstrated increased uptake at all four sites and sepsis was once again suspected. Biopsies and culture proved

Septic arthritis in heroin addicts.

Over a 6-year period (1982 to 1988), 36 episodes of septic arthritis were diagnosed in 35 heroin addicts from Barcelona, Spain. Thirty (86%) were men and five (14%) were women, with a mean age of 24 years (range, 14 to 39). Twenty-nine episodes (80%) were monoarticular and seven (20%) were oligoarticular. The sacroiliac (16 cases), sternoclavicular (8), hip (5), and shoulder (4) joints were most frequently infected. Staphylococcus aureus and Pseudomonas aeruginosa were the etiological agents in 75% and 11% of episodes, respectively. Response to antibiotic treatment was good in 32 cases (90%),

Pyogenic arthritis caused by Streptococcus milleri in a nonimmunocompromised host.

We describe a case of pyogenic arthritis caused by S. milleri (S. anginosus) in a non-immunocompromised host. No other septic focus was found. S. milleri is now widely recognized as an important cause of pyogenic infection, though it eluded suspicion for many years. The rarity of reports of articular infections by S. milleri might be due to problems associated with its recognition.

Septic arthritis due to group C streptococcus: report and review of the literature.

A case of polyarticular septic arthritis due to Lancefield group C streptococcus is described and the clinical details of 9 reported cases of septic arthritis due to group C streptococcus reviewed. Several features of the presentation and course of these patients, including polyarticular involvement (4/10), bacteremia (5/10), clinically important dysfunction in other systems (4/10 patients: cardiac, 3; CNS, 2; pneumonia, 1; gastrointestinal, 1), fatal outcome (3/10 patients, 2 during the course of active infection) and poor functional outcome in affected joints (4/7 surviving patients), serve

[Clinico-biological aspects of myositis due to Trichinella T3 with special regard to a rheumatologic study].

One hundred and fifty patients living in Salsomaggiore Terme developed signs and symptoms of trichinellosis after eating infected horse meat. The etiological agent was identified as Trichinella T3. This large outbreak gave us the opportunity of studying the rheumatic manifestations during this kind of parasitic disease. Myositis was observed in 62% of the entire series whereas arthralgias were recorded in only 20% of the patients. The muscle pain was described in several ways, but in the majority it appeared during isokinetic movements. The scapular girdle was involved much more than the pelvi

Yersinia pseudotuberculosis infection as a cause of reactive arthritis as seen in a genitourinary clinic: case report.

Patients are often referred to the genitourinary clinic for screening for urethral infections after a clinical diagnosis of Reiter's syndrome or reactive arthritis. We report a case of reactive polyarthritis in which serological evidence of Yersinia pseudotuberculosis infection was found in the absence of the other common precipitating organisms, such as Chlamydia spp, Klebsiella spp, Shigella spp, Campylobacter spp, or Yersinia enterocolitica.

Gonococcal arthritis case report.

In the case reported, the diagnosis of gonococcal arthritis unfolded over the course of 1 week and was not fully conclusive until presumptive therapy was initiated. Although the clinical, microbiologic, and immunologic characteristics of gonococcal arthritis can be differentiated from other types of bacterial arthritides, not all textbook symptoms are present at one time in one particular case. There also are subtle signs that are involved. In this case study, there was little definitive evidence that stood out to confirm the diagnosis. It was the interdependence of a complete history, serolog

Group F Streptococcus. An unusual cause of arthritis.

Bacterial arthritis is a serious infection in children occurring either as the result of direct inoculation of organisms into the joint space, of spreading from a contiguous focus of infection, or of hematogenous seeding of the synovium. Although the etiologic agent can be identified in 60% to 70% of cases, joint fluid cultures may remain negative in a substantial number of patients. The most likely etiologic agent depends both on the route of infection and the age of the child. Overall Staphylococcus aureus, Hemophilus influenzae type b and streptococci are the leading causes of bacterial art
