Activation of T cells recognizing self 60-kD heat shock protein can protect against experimental arthritis.

Lewis rats are susceptible to several forms of experimental arthritis-induced using heat-killed Mycobacterium tuberculosis (adjuvant arthritis, or AA), streptococcal cell walls, collagen type II, and the lipoidal amine CP20961. Prior immunization with the mycobacterial 65-kD heat shock protein (hsp65) was reported to protect against AA, and other athritis models not using M. tuberculosis, via a T cell-mediated mechanism. Hsp65 shares 48% amino acid identity with mammalian hsp60, which is expressed at elevated levels in inflamed synovia. Several studies have reported cross-reactive T cell recog

Juvenile rheumatoid arthritis.

The etiology and pathogenesis of juvenile rheumatoid arthritis remains unknown; however, research using new techniques is revealing information on the roles of immunogenetics, cellular immunity, and humoral immunity in these disorders. Interest continues in infection as a potential trigger of juvenile rheumatoid arthritis, as reactivity to infectious agents in synovial lymphocytes is sought. Reactivity to heat-shock proteins suggests a pathogenetic role for this class of proteins as well. Careful analysis of outcome in children with systemic-onset juvenile rheumatoid arthritis has identified c

Possible involvement of group A streptococci in the pathogenesis of psoriatic arthritis.

Bacteria have been implicated in the form of Reiter's syndrome, known as reactive arthritis. If bacteria are important in the related spondyloarthropathy psoriatic arthritis, they may be those comprising skin flora. We found elevated levels of antibody to the streptococcal exotoxin antideoxyribonuclease-B in psoriatic arthritis patients in comparison to patients with psoriasis without arthritis, rheumatoid arthritis, other forms of dermatitis, and normal controls. Patients with peripheral arthritis were more likely than psoriatic spondylitis patients to be antideoxyribonuclease-B positive. Str

TAK-603 selectively suppresses Th1-type cytokine production and inhibits the progression of adjuvant arthritis.

We have shown that TAK-603, a new anti-rheumatic drug, is more effective in animal models in which cellular immunity plays a central role. Here, we studied the effect of the drug on Th1 cytokines, which are dominantly produced in this type of immune reaction, in an in vitro system and an in vivo model. We established Th1- and Th2-dominant T-cell lines, and studied the effect of TAK-603 on their cytokine production. Th1 cell lines were BALB/c mouse allo-reactive T cells and C57BL mouse mite antigen-reactive T cells, and the Th2 cell line was BALB/c mouse ovalbumin-reactive T cells. TAK-603 supp

[An elderly case with group B streptococcal bacteremia, subcutaneous abscess and reactive polyarthritis].

We report a 77-year-old woman with Group B streptococcal bacteremia, subcutaneous abscess and reactive polyarthritis. Two years previously she suffered from atrial fibrillation and osteoarthritis of the knee. After she was admitted for treatment of the knee joint with hyaluronate sodium, she complained of pain in the left shoulder and both knees. Pyogenic arthritis was suspected and administration of cefazolin was started immediately after blood culture. One set of blood cultures showed Group B streptococcus. Therefore the antibiotic was changed to ampicillin. To investigate the cause of polya

Efficacy and safety of arthroscopic treatment for native acute septic arthritis of the hip joint in adult patients.

BACKGROUND: As septic arthritis is time-dependent and has a propensity for irreversible joint damage, early diagnosis and treatment are needed. Frequently, adult patients with septic arthritis cannot undergo invasive surgery because of comorbidities and a weakened immune system. Hip arthroscopic irrigation and debridement for native acute septic arthritis of the hip joint have been performed as the first choice of treatment for patients of all ages. This study aimed to assess the efficacy and safety of arthroscopic management for native acute septic arthritis of the hip joint in adult patients

Assessing clinical competence: recognition of case descriptions of rheumatic diseases by general practitioners.

The objective of this study was to detect strengths and weaknesses in the diagnosis of rheumatic diseases by general practitioners in order to set up post-graduate training accordingly and to assess whether open-ended questions give results comparable with multiple choice-type questions. Fifty-one general practitioners were given eight written cases: rheumatoid arthritis (RA), ankylosing spondylitis (AS), reactive arthritis, psoriatic arthritis (PsA), systemic lupus erythematosus (SLE), gout, polymyalgia rheumatica and pseudogout. Only signs and symptoms were provided. All cases were derived f

Association of antibiotic treatment-resistant Lyme arthritis with T cell responses to dominant epitopes of outer surface protein A of Borrelia burgdorferi.

OBJECTIVE: To explore further the association of antibiotic treatment-resistant Lyme arthritis and T cell reactivity with outer surface protein A (OspA) of Borrelia burgdorferi, including the identification of T cell epitopes associated with this treatment-resistant course. METHODS: The responses of peripheral blood and, if available, synovial fluid lymphocytes to B burgdorferi proteins, fragments, and synthetic peptides, as determined by proliferation assay and interferon-gamma production, were compared in 16 patients with treatment-responsive and 16 with treatment-resistant Lyme arthritis.

Thymus, innate immunity and autoimmune arthritis: interplay of gene and environment.

A hypomorphic mutation of the gene encoding zeta-associated protein-70 (ZAP-70), a signaling molecule in T cells, produces autoimmune arthritis in mice under a microbially conventional condition but not in a clean environment. The genetic anomaly alters thymic selection of self-reactive T cells as well as natural regulatory T cells and their respective functions. Highly self-reactive polyclonal T cells, including arthritogenic ones, thus produced by the thymus strongly recognize self-antigens presented by antigen-presenting cells, stimulate them to up-regulate co-stimulatory molecules and secr

The significance of controls in Lyme arthritis of children.

In order to better define the specificity of Lyme disease (LD) seropositivity and seronegativity during a study of Lyme arthritis in a pediatric rheumatology population, three groups of controls are in the process of being developed. Children with other rheumatic disorders. Siblings and parents of children with Lyme arthritis. Individuals with intense environmental exposure to LD. Sera from each have been tested for antibody to Borrelia burgdorferi (Bb), by indirect immunofluorescence assay (IFA), enzyme linked immunosorbent assay (ELISA), or both. Of the 57 children with rheumatic disorders o

Tryptophanyl tRNA synthetase is an alternative synovial biomarker for diagnosis of septic arthritis in knee joint.

BACKGROUND: To evaluate the diagnostic characteristics of tryptophanyl tRNA synthetase (WRS) for the diagnosis of septic arthritis of the knee joint and to determine whether it is a reliable and sensitive synovial biomarker for discriminating septic arthritis from other types of arthritis. METHODS: Patients joint effusions for which septic arthritis was suspected were prospectively recruited between January 2019 and September 2020. A total of 9 patients had septic arthritis, 6 had acute gout attack, 1 had an acute flare of chronic rheumatic arthritis, and 46 had pseudogout or reactive arthrop

Persistent synovial lymphocyte responses to mumps and adenovirus antigens.

A man in his 50s with rheumatoid arthritis showed maximal synovial lymphocyte reactivity to mumps antigen on 9 of 10 testings over a period of 6 years; peripheral blood lymphocytes showed no significant responses to mumps antigen in 5 testings over 5 years. A boy of 15 with recurrent arthritis of the right knee showed maximal synovial lymphocyte reactivity to adenovirus antigen. This reactivity was again present during a subsequent episode more than 3 years later; peripheral blood lymphocytes showed no such response to adenovirus antigen.

Animal models of intestinal and joint inflammation.

Recent rodent models have been exploited to explore mechanisms of intestinal and joint inflammation. HLA-B27 transgenic rats develop colitis, gastritis, and arthritis when raised in a conventional environment, but have no evidence of inflammation under germfree (sterile) conditions. Metronidazole treatment attenuates gastrointestinal inflammation, suggesting that anaerobic bacteria are important. Experimental bacterial over-growth of predominantly anaerobic bacteria reactivates arthritis in Lewis rats which have been previously injected intra-articularly with bacterial cell wall polymers. T cell responses to polypeptide fractions of Borrelia burgdorferi in patients with Lyme arthritis.

Among 6 patients with prolonged episodes of Lyme arthritis, the mean response of peripheral blood lymphocytes (PBL) to all Borrelia burgdorferi antigens (stimulation index [SI] 46) was greater than that among 5 patients with brief attacks of Lyme arthritis (SI 13; P less than 0.1), as well as that among 7 control patients with rheumatoid arthritis and among 6 normal control subjects (in both instances SI 3; P less than 0.05). In individual patients with brief episodes of Lyme arthritis, PBL had similar low levels of reactivity with the 20-kd, 31-kd, 34-kd, 41-kd, 55/58-kd, and 66-kd spirocheta
