Preterm cranial ultrasound scanning is both feasible and effective in a middle-income country.

AIM: Cranial ultrasound is seldom used in middle-income countries, and the burden of preterm brain injury and its relationship to perinatal data is unknown. We assessed cranial ultrasound abnormalities in very low-birthweight (VLBW) infants and correlated the findings with perinatal data. METHODS: VLBW Armenian infants receiving neonatal intensive care in 2012 were scanned from birth to term-equivalent age (TEA). Clinical data were collected prospectively. RESULTS: We studied 100 VLBW infants with a median gestation of 30 weeks. Periventricular white matter echogenicity (PVE) lasting more th

Resuscitator's perceptions and time for corrective ventilation steps during neonatal resuscitation.

BACKGROUND: The 2010 neonatal resuscitation program (NRP) guidelines incorporate ventilation corrective steps (using the mnemonic--MRSOPA) into the resuscitation algorithm. The perception of neonatal providers, time taken to perform these maneuvers or the effectiveness of these additional steps has not been evaluated. METHODS: Using two simulated clinical scenarios of varying degrees of cardiovascular compromise--perinatal asphyxia with (i) bradycardia (heart rate--40 min(-1)) and (ii) cardiac arrest, 35 NRP certified providers were evaluated for preference to performing these corrective meas

Impact of asphyxia on red blood cell folate concentration levels in newborns.

OBJECTIVES: To determine whether red blood cell (RBC) folate concentration levels are correlated with the occurrence of neonatal asphyxia and to study the effects of gestational age, gender, and mode of delivery on RBC folate concentration levels in newborns. BACKGROUND: Asphyxia is one of the frequent causes of morbidity and mortality of newborns. Severe perinatal asphyxia can arise due to many factors. METHODS: In a prospective study, the RBC folate concentrations were determined on day 1 of life in the whole group (n=181) of full-term (n=121) and preterm (n=60) newborns. Immunochemical an

Does therapeutic hypothermia reduce acute kidney injury among term neonates with perinatal asphyxia?--a randomized controlled trial.

OBJECTIVE: The objective of this study is to evaluate whether therapeutic hypothermia reduces the incidence of acute kidney injury (AKI) among term neonates perinatal asphyxia. METHODS: This randomized controlled trial conducted in a tertiary care teaching hospital, south India included 120 term neonates with perinatal asphyxia who were randomized to receive either therapeutic hypothermia or standard supportive care. Renal parameters of neonates in both the groups were monitored and AKI was ascertained as per Acute Kidney Injury Network criteria. RESULTS: The incidence of AKI was less in the

Therapeutic hypothermia modifies perinatal asphyxia-induced changes of the corpus callosum and outcome in neonates.

UNLABELLED: WHAT IS KNOWN ABOUT THIS SUBJECT?: Diffusion-weighted MRI has demonstrated changes in the corpus callosum of term neonates with perinatal asphyxia. The severity of cerebral changes demonstrated using diffusion-weighted MRI is difficult to assess without measuring values of the Apparent Diffusion Coefficient (ADC). WHAT IS NEW?: ADC values of the anterior part of the corpus callosum are slightly higher than of the posterior part in full term infants with perinatal asphyxia. Low ADC values of the corpus callosum were associated with an adverse outcome in infants with perinatal asphyx

[Regression of cystic lesions on brain MRI in a child with hypoxic-ischemic encephalopathy treated with selective head cooling].

The authors present the first case of regression of cystic lesions on brain MRI in a newborn after therapeutic hypothermia in Poland. Multicystic encephalopathy is the most severe form of hypoxic-ischemic encephalopathy and its regression is described very rarely in the literature. Magnetic resonance imaging is an accepted, optimal method of evaluation of the brain and establishing prognosis in children with HIE. After normal pregnancy an emergency cesarean section was performed at 37 weeks gestation due to the markers of intrauterine hypoxia on CTG. The condition of the newborn was serious: 3

[A survey of neonatal births in maternity departments in urban China in 2005].

OBJECTIVE: To study the epidemiology of births in urban China. METHODS: A retrospective study was conducted on neonates born in 2005 in the maternity departments of 72 urban hospitals from 22 provinces in China. RESULTS: A total of 45722 infants born between January 1, 2005 and December 31, 2005 were enrolled. The male to female sex ratio was 1.13:1. Preterm births accounted for 8.1%. The incidence of very low birth weight infants was 0.7%. A total of 99.7% of mothers delivering at term had conceived naturally and 0.3% had experienced assisted reproduction. A total of 98.4% of mothers who de

The high-risk newborns.

The term "high-risk newborns" identifies a group of newborns who very likely will develop a severe acute disease or an adverse outcome. Every day, neonatologists are faced with high-risk newborns that represent the majority of patients admitted to the neonatal intensive care units. Particularly, term newborns with hypoxic-ischemic encephalopathy and very preterm newborns require special surveillance of growth and development. A specific follow-up program should be implemented to monitor the long-term outcome of these patients with special needs.

New pharmacological approaches in infants with hypoxic-ischemic encephalopathy.

New knowledge of the pathophysiology and evolution of hypoxic-ischemic brain injuries has made feasible interventions to improve clinical outcomes for newborns surviving birth asphyxia. Brain injury following hypoxic-ischemic insult is a complex process evolving over hours to days, which provides a unique window of opportunity for neuroprotective treatment interventions. The specific pathologic processes preceding the onset of irreversible cerebral injury appear to be a combination of several mechanisms that are variable according to the severity and duration of the insult and to biochemical m

White-gray matter echogenicity ratio and resistive index: sonographic bedside markers of cerebral hypoxic-ischemic injury/edema?

OBJECTIVE: Head ultrasonography (HUS) is a reliable and easy to perform bedside imaging technique that can give valuable information about degree of brain injury/edema after perinatal asphyxia in term neonates. The goals of our study were to determine whether semiquantitative markers such as standardized white matter/gray matter (WM/GM) echogenicity ratio and resistive index (RI) value measured by HUS differs between asphyxiated term neonates and healthy controls. STUDY DESIGN: Thirty-one carefully selected term neonates who suffered from perinatal hypoxic-ischemic encephalopathy (HIE) were i

3.6 million neonatal deaths--what is progressing and what is not?

Each year 3.6 million infants are estimated to die in the first 4 weeks of life (neonatal period)--but the majority continue to die at home, uncounted. This article reviews progress for newborn health globally, with a focus on the countries in which most deaths occur--what data do we have to guide accelerated efforts? All regions are advancing, but the level of decrease in neonatal mortality differs by region, country, and within countries. Progress also differs by the main causes of neonatal death. Three major causes of neonatal deaths (infections, complications of preterm birth, and intrapar

Neurocognitive outcomes following neonatal encephalopathy.

Neonatal encephalopathy (NE) from perinatal asphyxia (PA) has long been recognized as an important cause of lasting motor impairment in term newborns. NE has also, more recently, been implicated as an important risk factor for cognitive and behavioral difficulties as these children age. Newborns with mild NE appear to have normal neurocognitive outcomes, while those survivors with severe NE tend to have profound impediments. Yet, newborns with moderate NE seem to exhibit a wide range of cognitive outcomes - regardless of motor function - making prognostication in these children difficult in th

[Transitory hypothermia as early prognostic factor in term newborns with intrauterine growth retardation].

INTRODUCTION: According to numerous researches, transitory hypothermia is a part of the neonatological energetic triangle and represents a significant prognostic factor within morbidity and mortality in newborns with intrauterine growth retardation (IUGR), that are, due to their characteristics, more inclined to transitory hypothermia. OBJECTIVE: The aim of the study was an analysis of frequency of transitory hypothermia in term newborns with IUGR, as well as an analysis of frequency of the most frequent pathological conditions typical of IUGR newborns depending on the presence of transitory

Has electronic fetal heart rate monitoring made a difference.

Electronic fetal heart rate monitoring (EFHRM) has revolutionised our understanding of the function of the cardiovascular system of the fetus during labour, and how the fetus responds to hypoxia. However, although it is a sensitive technique for the detection of hypoxia during labour, it is relatively non-specific for detecting the development of acidosis. Moreover, it is highly dependent on accurate interpretation of fetal heart rate (FHR) patterns, which has been shown to be commonly of a low standard in everyday clinical practice. Use of EFHRM has probably reduced the incidence of birth asp

History of neonatal resuscitation - part 3: endotracheal intubation.

Endotracheal intubation to resuscitate neonates was used by Scheel in 1798. A century before endotracheal anesthesia was developed, inventive obstetricians constructed devices for endotracheal intubation of infants and mastered their insertion, localization, and airtight sealing. Fell's laryngoscope, Magill's intubation forceps and tissue-friendly materials were significant contributions of the 20th century to endotracheal intubation of the newborn. The striking absence of scientific studies on the most efficient resuscitation techniques for neonates can be explained by the difficulty to adjus
