Spinal Anaesthesia is Safe in a Patient with Wolff-Parkinson-White Syndrome Undergoing Evacuation of Molar Pregnancy.

Wolff-Parkinson-White (WPW) syndrome is an uncommon cardiac condition where there is an abnormal band of atrial tissue connecting atria and ventricles which can electrically bypass atrioventricular node. The anaesthetic management in these patients is challenging as life threatening complications can occur perioperatively like paroxysmal supraventricular tachycardia and atrial fibrillation. Also, regional anaesthetic technique like subarachnoid block is a safe and cost effective alternative to general anaesthesia as it avoids polypharmacy. We report the successful anaesthetic management of Wol…

Complete atrioventricular block in adult Sjögren's syndrome with anti-Ro autoantibody.

Anti-Ro autoantibody is associated with Sjögren's syndrome (SS), systemic lupus erythematosus (SLE), and neonatal lupus syndrome (i.e., congenital complete heart block in newborns). Generally, the adult atrioventricular (AV) node is believed to be relatively resistant to the scarring effects of anti-Ro/anti-La autoantibodies. However, there have been some reports of adult complete AV block in SS and SLE patients. Here, we report a case of complete heart block in primary SS with anti-Ro autoantibodies, with no other risk factor for the development of heart block, and review their etiological as

Limited Wegener's granulomatosis presenting with complete heart block.

Complete heart block associated with Wegener's granulomatosis (WG) is rare, and has not previously been reported with 'limited' WG. The case of a 36-year-old man who presented with complete heart block due to 'limited' WG [positive cytoplasmic antineutrophil cytoplasmic antibodies (c-ANCA) on indirect immunofluorescence, positive serum antibodies to proteinase-3, and inflammatory sinus disease seen on computerized tomography (CT) without renal or pulmonary involvement] is presented. In addition, a gallium-scan fused with a myocardial perfusion scan and cardiac magnetic resonance imaging (MRI)

Successful interventional treatment of an octogenarian presenting with syncope and Ebstein's anomaly of the tricuspid valve.

Syncope is a frequent occurrence in the elderly and cardiac causes are common. Therefore, accurate diagnosis is essential since specific treatments are often required. We present an 85-year-old woman with recurrent syncope secondary to sinus node reentrant tachycardia (SNRT), which was successfully treated by radiofrequency ablation. This is the first reported description of SNRT and its successful treatment in Ebstein's anomaly. Atrial arrhythmias and accessory atrioventricular bypass pathways are common in Ebstein's anomaly and are briefly discussed. Our patient is alive at age 87, which is

Doppler echocardiography with extended transesophageal atrial pacing: predicting the efficacy of permanent atrial pacing in the patient with a small left ventricle and sinus node dysfunction.

A 2100-g neonate underwent a two-ventricular surgical repair of a right ventricle-dominant unbalanced atrioventricular septal defect associated with the heterotaxy syndrome and sinus node dysfunction. Postoperative congestive heart failure persisted despite bradycardia management by temporary ventricular pacing. Spectral Doppler echocardiographic analysis of pulmonary venous inflow and aortic outflow patterns demonstrated significant improvement with transesophageal atrial pacing. Extended transesophageal pacing was performed for two days, resulting in dramatic clinical improvement. This is th

Paroxysmal nodal reentrant tachycardia. Surgical cure with preservation of atrioventricular conduction.

In a patient with repetitive disabling tachycardias refractory to pharmacologic treatment, the electrophysiologic study suggested the existence of atrioventricular nodal reciprocating tachycardia. During ventricular pacing, endoepicardial mapping of the lower atrium showed the atrial breakthrough point in an area of the lower interatrial septum close to the AV node crista. A selective atriotomy was performed. The postoperative electrophysiologic studies showed absence of ventriculoatrial conduction at several ventricular pacing rates, while antegrade conduction is preserved. The patient remain

[Transesophageal electric heart stimulation in the diagnosis of paroxysmal supraventricular tachycardias].

Thirty-one patients with paroxysms of supraventricular tachycardia (SVT) were examined using trans-esophagus electrical stimulation of the left atrium. SVT paroxysms were provoked in 23 patients. An analysis of the esophagus electrogram recorded revealed SVT paroxysms in the presence of the latent Wolff-Parkinson-White (WPW) syndrome in 12 patients, orthodromic SVT in the presence of the WPW syndrome in seven, and paroxysmal reciprocal atrioventricular node tachycardia in four patients. Paroxysms were also induced in three out of 8 patients with atrial fibrillation. Two patients displayed a fa…

Clinical studies on the sick sinus syndrome.

1) Overdrive suppression is clinically useful to evaluate depressed cardiac automaticity in the sick sinus syndrome. 2) His bundle electrograms revealed frequent association of atrioventricular conduction defects with the depressed automaticity in this syndrome. 3) Especially in BTS, marked intra-atrial conduction defect was observed, and this conduction defect may play a role for development of re-entrant tachycardia. 4) Pathologic changes in the sick sinus syndrome are supposed to involve the whole atrial tissues including not only the S-A nodal region but the A-V node and its approaches.

Effects of right bundle branch block during atrioventricular nodal reentrant tachycardia.

BACKGROUND: The significant role of bundle branch block during atrioventricular nodal reentrant tachycardia (AVNRT) is not clear. The purposes of this study were to study the effects of complete right bundle branch block (RBBB) on electrophysiological parameters during AVNRT and to define the significance of complete RBBB during AVNRT. METHODS AND RESULTS: According to characteristics of electrocardiogram during sinus rhythm and AVNRT, 50 patients who underwent catheter ablation for slow-fast AVNRT were divided into three groups. Group I included 20 patients who had narrow QRS (< or = 110 ms)

QRS complex voltage changes associated with supraventricular tachycardia.

INTRODUCTION: The aim of this study was to evaluate the changes in ventricular complex voltage associated with narrow QRS supraventricular tachycardia (SVT). METHODS AND RESULTS: One hundred forty-five patients undergoing catheter ablation for SVT, 85 with AV nodal reentrant tachycardia (AVNRT) and 60 with AV reentrant tachycardia (AVRT) due to a concealed accessory pathway, were studied. Four consecutive tachycardia beats and four consecutive sinus beats were analyzed, excluding the last tachycardia complex and the first sinus one. For each of the 12 leads, the QRS complex voltage was measur

Concomitant reentrant tachycardias from concealed accessory atrioventricular bypass tract and atrioventricular nodal reentry in a patient with Williams syndrome.

INTRODUCTION: Autoimmune cytopenias have been very rarely reported in conjunction with POEMS (polyneuropathy, organomegaly, endocrinopathy, M-protein, and skin changes syndrome). To the best of our knowledge, there is no reported case in the existing literature of an association of Evans syndrome with POEMS syndrome. CASE DESCRIPTION: A 55-year-old female patient, with a medical history of well-controlled dermatomyositis, developed fulminant autoimmune haemolytic anaemia with immune thrombocytopenic purpura, thus fulfilling the diagnosis of Evans syndrome. To control the disease, acute manage

Accessory pathway reciprocating tachycardia.

Patients who have an accessory pathway (AP) of atrioventricular (AV) conduction may develop circus movement tachycardia otherwise known as atrioventricular re-entrant tachycardia (AVRT). Orthodromic AVRT is the most common form. It occurs as a result of antegrade conduction through the normal AV conduction system and retrograde conduction to the atria via the AP. Less commonly, conduction occurs in the opposite direction resulting in antidromic AVRT. Tachycardia may also involve multiple APs which may provide both antegrade and retrograde conduction and may alternate antegradely or retrogradel

[Left ventricular systolic time intervals during paroxysmal supraventricular tachycardia: the difference between A-V nodal re-entry and A-V re-entry].

In this study, the differences in hemodynamic changes during paroxysmal supraventricular tachycardia (PSVT) between A-V nodal re-entry and A-V re-entry were evaluated. In 8 patients with A-V nodal re-entrant tachycardia and 10 with A-V re-entrant tachycardia, electrophysiological studies were performed to measure systolic time intervals (pre-ejection period: PEP, ejection time: ET, PEP/ET ratio: PEP/ET). These measurements were obtained in the control state (atrial pacing at 90/min) and during PSVT with simultaneous recordings of electrocardiogram and femoral arterial pulse tracing. During PSV

Determining the Site of Accessory Pathways in Orthodromic Reciprocating Tachycardia by Using the Response to Right Ventricular Pacing.

BACKGROUND: Postpacing interval (PPI) after right ventricular (RV) pacing entrainment minus tachycardia cycle length (TCL) with a correction for atrioventricular (AV) node delay (corrected PPI-TCL) was useful to differentiate atrioventricular node reentrant tachycardia (AVNRT) from orthodromic reciprocating tachycardia (ORT). However, the value of corrected PPI-TCL in determining the site of the accessory pathway (AP) in ORT has not been investigated. The purpose of this study was to assess whether the corrected PPI-TCL is useful in differentiating ORT using a left-sided AP from a right-sided …

The strands of the myocytes in the sinoatrial node were oriented linearly, while those in the atrioventricular node formed a reticular network. The intercalated discs in both nodal cells were underdeveloped, having few microprojections. Myocytes in the atrioventricular bundle (His) and the right limb were arranged in parallel, and were characterized by the presen …

Flecainide for supraventricular tachycardia in children.

The clinical efficacy, adverse effects and pharmacokinetics of flecainide were evaluated in 16 pediatric and young adult patients with supraventricular tachycardia (SVT). Patients had received an average of 2.8 drugs before flecainide was tried. The following mechanisms of supraventricular arrhythmias were determined in patients by intracardiac electrophysiologic studies: atrioventricular node reentry, 4; reentry through an accessory connection, 7; atrial automatic focus, 2; atrial flutter, 3. Twelve patients had normal cardiac anatomy and 4 had congenital heart disease. Each patient received …

Harder Roads to Trek? Paradoxical Slowing of an Atrioventricular Reentrant Tachycardia With Contralateral Bundle Branch Block.

Slowing of the tachycardia with increase in cycle length and increase in VA time with ipsilateral bundle branch block is a characteristic feature of accessory pathway mediated AVRT. Contralateral bundle branch block has no effect on the tachycardia as it is not a part of the tachycardia circuit. We present an interesting phenomenon in which contralateral bundle branch block resulted in tachycardia slowing in a case of WPW syndrome.

Predictors of acute and long-term success of slow pathway ablation for atrioventricular nodal reentrant tachycardia: a single center series of 1,419 consecutive patients.

BACKGROUND: Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common mechanism of supraventricular tachycardia. Slow pathway (SP) ablation is the first-line treatment approach with a high acute success rate and a low risk of inadvertent complete atrioventricular (AV) block. However, there is still some uncertainty as to the most appropriate procedural endpoints and the impact of these on risk of recurrence. We report the acute and long-term results of SP ablation in a large single-center consecutive series and analyze predictors of acute success and late recurrence. METHODS: Th

Clinical symptoms of limited exercise capacity linked to AAI-DDD functionality: An in silico and in vivo approach.

BACKGROUND: Exercise capacity is an important aspect of quality of life in patients undergoing pacemaker implantation. Device algorithms for ventricular pacing avoidance have been developed to avoid unnecessary and potentially harmful effects of right ventricular pacing. However, little data exists on the immediate response of these algorithms to sudden AV block during exercise. METHODS: The ventricular pacing avoidance algorithms of four pacemaker manufacturers were tested in an ex-vivo model. The RSIM-1500-USB Device-Interactive Heart Simulator (Rivertek Medical Systems, Inc.) was used to s
