Mass spectrometry of the soot left after ethylene oxide explosion answers some questions on the crash of Polish Air Force Flight 101.

The Polish TU 154M plane, Polish Air Force Flight 101, had crashed near Smolensk on 10th of April 2010. The crash was investigated by The Interstate Aviation Committee, whose conclusions were questioned by a number of Polish scientists. The cause of the crash still appears to be incompletely documented and requires additional evidence. In this paper, investigations of a solid material eluted from a piece of cloth of one of the victims of the crash are described. High resolution mass spectrometry was applied to analyze the soot left after controlled ethylene oxide (EO) explosions, performed und…

Aeromedical Implications of Cerebral Cavernomas.

BACKGROUND: Cavernomas, cavernous angiomas, or cerebral cavernous malformations are clusters of endothelium-lined blood vessels usually found in the brain. With the increasing use of radiological imaging, these are being detected incidentally in asymptomatic aircrew. The UK Civil Aviation Authority (CAA) experience of cavernomas is described and the aeromedical concerns, that is, the risk of epilepsy, hemorrhage, and the development of a neurological deficit, are considered.METHODS: A search of the CAA database between 1990 and 2020 was performed for the term cavernoma. The gender, age at diag…

Letter to Editor. Prerequisites for psychiatric examination detecting the intention to commit suicide.

The recent plane crash caused by the pilot increased the interest in the possibility of medical examination, which would be able to detect the intention of committing suicide. The development of such a diagnostic procedure is not only important for the prevention of events in the civil and military aviation, but also due to increase in the incidence of various suicide terrorist acts. The author expresses his opinion on the nature of such examination, due to his experience of working in Acute Poisoning Treatment Centre. The Centre admits about 1 000 patients per year, who have been rescued afte…

EACTS guidelines for the use of patient safety checklists.

The Safety Checklist concept has been an integral part of many industries that face high-complexity tasks for many decades and in industries such as aviation and engineering checklists have evolved from their very inception. Investigations of the causes of surgical deaths around the world have repeatedly pointed to medical errors that could be prevented as an important cause of death and disability. As a result, the World Health Organisation developed and evaluated a three-stage surgical checklist in 2007 demonstrating that complications were significantly reduced, including surgical infection…

[Wars in the history of rheumatology].

Some important discoveries in the history of rheumatology happened during war periods. It is well known that arthritis associated with conjunctivitis and urethritis, following dysenteric episodes, has been described during the First World War from the German Hans Reiter and, nearly contemporarily, from the French Nöel Fiessinger and Edgar Leroy. Less known is instead the fact that the first cases of sympathetic algoneurodystrophy have been reported by the American Silas Weir Mitchell in soldiers wounded by fire-arms, during the Civil War of Secession. Other war episodes have been crucial for t

Evolution of surgical skills training.

Surgical training is changing: one hundred years of tradition is being challenged by legal and ethical concerns for patient safety, work hou … l invasive surgery. An innovative and progressive approach, borrowing experiences from the field of aviation, can provide the foundation for the next century of surgical training, ensuring the quality of the product. As the technology develops, the way we practice will continue to evolve, to the benefit of physicians and patients.

High altitude medicine and physiology in the former Soviet Union.

The countries of the former Soviet Union have a long history of studies in high-altitude medicine and physiology, but much of the informatio … ently Oleg G. Gazenko (1918-) and his colleagues have carried out extensive studies associated with aviation medicine and the selection of cosmonauts for the Soviet space program. Among many topics, one is the study of how adaptation to one type of physiologic stress (e.g., hypoxia) can improve tolerance to another stress (e.g., acceleration).

Virtual reality exposure therapy and standard (in vivo) exposure therapy in the treatment of fear of flying.

This controlled clinical trial tested virtual reality exposure (VRE) therapy for the fear of flying (FOF), a relatively new and innovative way to do exposure therapy, and compared it to standard (in vivo) exposure therapy (SE) and a wait list (WL) control with a 6- and 12-month follow-up. Eighty-three participants with FOF were randomly assigned to VRE, SE, or WL. Seventy-five participants, 25 per group, completed the study. Twenty-three WL participants completed randomly assigned treatment following the waiting period. Treatment consisted of 4 sessions of anxiety management training followed

Physiological monitoring as an objective tool in virtual reality therapy.

The goals of this study were twofold: (1) to investigate nonphobics' and phobics' physiological response in virtual environments, and (2) to analyze the trend of phobics' physiology during virtual reality (VR) treatment. As a measure of physiology, heart rate, skin resistance, and skin temperature were acquired. The data for two groups of participants were analyzed: 22 nonphobic participants (mean = 32 +/- 9.4 years) and 36 subjects with fear of flying (mean = 40 +/- 12.1 years) who met the DSM-IV criteria for fear of flying. As a result, skin resistance showed significant differences between

The prevalence of internet addiction and its association with quality of life among inflight security officers based on a national survey: a network analysis perspective.

BACKGROUND: Given the heavy responsibilities placed on inflight security officers (IFSO) to ensure passenger safety and eliminate inflight hazards, they often turn to Internet use to cope with their work pressure. This study examined the prevalence of internet addiction (IA) among IFSO in China, and its associated factors, relationship with quality of life (QOL), and network structure. METHODS: This was a cross-sectional study based on a national survey. Expected influence (EI) was used to identify the most central nodes within the network model. RESULTS: Among 3,475 IFSO included in this st

Risk factors for injuries during airborne static line operations.

US Army airborne operations began in World War II. Continuous improvements in parachute technology, aircraft exit procedures, and ground landing techniques have reduced the number of injuries over time from 27 per 1,000 descents to about 6 per 1,000 jumps. Studies have identified a number of factors that put parachutists at higher injury risk, including high wind speeds, night jumps, combat loads, higher temperatures, lower fitness, heavier body weight, and older age. Airborne injuries can be reduced by limiting risker training (higher wind speeds, night jumps, combat load) to the minimum nece

The sensitivity and specificity of control surface injuries in aircraft accident fatalities.

Among the important determinations that aircraft crash investigators try to make is which occupant of an aircraft was attempting to control the aircraft at the time of the crash. The presence or absence of certain injuries of the extremities is used to help make this determination. These "control surface injuries" reportedly occur when crash forces are applied to a pilot's hands and feet through the aircraft's controls. We sought to clarify the significance of these injuries and the frequency with which their presence indicates that the decedent was the person that might have been trying to co

Assessing the safety of anthrax immunization in US Army aircrew members via physical examination.

OBJECTIVE: Anthrax in weaponized form is the bioterrorism agent of most concern. Questions raised about the safety of the anthrax vaccine can be addressed by comparing immunized and unimmunized people in population-based studies. METHODS: A retrospective evaluation of data from periodic physical examinations collected on anthrax-immunized and -unimmunized US Army aircrew members between 1998 and 2005 was performed to evaluate the safety of anthrax immunization. Mean changes in variables found on physical examination and laboratory analysis were compared by use of t tests. Multiple linear regr
