The strong relationship between bipolar and substance-use disorder.

Bipolar disorder and substance-use disorders commonly occur in the same individual. In fact, bipolar disorder has a higher prevalence of substance-use disorders than any other psychiatric illness. Individuals with both disorders have a more severe course of bipolar disorder, including earlier onset, more frequent episodes, and more complications, including anxiety- and stress-related disorders, aggressive behavior, legal problems, and suicide. Bipolar and substance-use disorders share common mechanisms, including impulsivity, poor modulation of motivation and responses to rewarding stimuli, an…

Late-onset bipolar disorder: a case for careful appraisal.

Bipolar disorder is a chronic mental illness with a profound personal and public health costs attached to it. The majority of patients suffering from bipolar disorder have an onset prior to the fifth decade of their lives. However, a significant number of patients have onset of illness after age 50, commonly referred to as late-onset bipolar disorder. Age of onset can have a significant impact on the nature and course of bipolar illness. We present an interesting case of late-onset bipolar disorder while evaluating secondary mania. The patient was a 76-year-old African American woman who prese…

Bipolar disorder: new perspectives in health care and prevention.

OBJECTIVE: High rates of misdiagnosis, delayed diagnosis, and lack of recognition and treatment of comorbid conditions often lead patients with bipolar illness to have a chronic course with high disability, unemployment rates, and mortality. Despite the recognition that long-term outcome of bipolar disorder depends on systematic assessment of both interepisodic dysfunctional domains and comorbid psychiatric and medical conditions, treatment of bipolar disorder still focuses primarily on alleviation of acute symptoms and prevention of future recurrences. We propose here to review the evidence o…

Do positive emotions predict symptomatic change in bipolar disorder?

OBJECTIVES: Bipolar disorder is associated with positive emotion disturbance, though it is less clear which specific positive emotions are affected. METHODS: The present study examined differences among distinct positive emotions in recovered bipolar disorder (BD) patients (n = 55) and nonclinical controls (NC) (n = 32) and whether they prospectively predicted symptom severity in patients with BD. At baseline, participants completed self-report measures of several distinct trait positive emotions. Structured assessments of diagnosis and current mood symptoms were obtained for BD participants.…

Oxidative imbalance in bipolar disorder subtypes: a comparative study.

OBJECTIVE: The oxidants are related with the membrane-associated pathologies in the central nervous system and may have an important role in neuropsychiatric disorders. Several studies were performed on the effects of free radicals in bipolar disorder. However, there are no studies investigating the effects of free radicals both in the subtypes of BD (Bipolar disorders I and II) and in antidepressant induced mania (AIM). In this study, we aimed to investigate the status of oxidative metabolism in BD and its subtypes. METHODS: 94 bipolar patients (BD I-II and AIM) diagnosed according to DSM IV…

Is 'bipolar disorder' the brain's autopoietic response to schizophrenia?

Evidence is accumulating that schizophrenia and bipolar disorder are related conditions. This paper proposes a particular form of relatedness. If 'schizophrenia' is a mind/brain 'trapped' between waking and dreaming, in a disordered in-between state, then bipolar 'disorder' could actually be an attempt to restore order. The mind/brain is a self-producing, self-organizing system. Autopoiesis applies to such systems. Neuromodulation accomplishes self-organization in the mind/brain. If schizophrenia is a state in-between waking and dreaming, characterized by aminergic/cholinergic interpenetration…

Psychiatric reactions to isotretinoin in patients with bipolar disorder.

BACKGROUND: Isotretinoin (Accutane(R)) has been available for the treatment of severe cystic acne for about twenty-five years. There have been several reports of adverse psychiatric reactions to isotretinoin, including depressive symptoms and suicide. However, there have been only three case reports of patients with bipolar disorder (BD) who experienced an untoward psychiatric side effect while receiving isotretinoin treatment. In this study, the psychiatric side effects from isotretinoin were assessed in a larger group of BD patients than has previously been reported. METHODS: A retrospectiv…

A linkage search for joint panic disorder/bipolar genes.

There is comorbidity and a possible genetic connection between Bipolar disease (BP) and panic disorder (PD). Genes may exist that increase risk to both PD and BP. We explored this possibility using data from a linkage study of PD (120 multiplex families; 37 had > or =1 BP member). We calculated 2-point lodscores maximized over male and female recombination fractions by classifying individuals with PD and/or BP as affected (PD + BP). Additionally, to shed light on possible heterogeneity, we examine the pedigrees containing a bipolar member (BP+) separately from those that do not (BP-), using a

Family-based association of YWHAH in psychotic bipolar disorder.

YWHAH is a positional and functional candidate gene for both schizophrenia and bipolar disorder (BP). This gene has been previously shown to be associated with both disorders, and the chromosome location (22q12.3) has been repeatedly implicated in linkage studies for these disorders. It codes for the eta subtype of the 14-3-3 protein family, is expressed mainly in brain, and is involved in HPA axis regulation. We investigated the association of YWHAH with BP in a large sample, consisting of 1211 subjects from 318 nuclear families including 554 affected offspring. We tested for association with…

Bipolar disorder and mechanisms of action of mood stabilizers.

Bipolar disorder (BD) is a major medical and social burden, whose cause, pathophysiology and treatment are not agreed on. It is characterized by recurrent periods of mania and depression (Bipolar I) or of hypomania and depression (Bipolar II). Its inheritance is polygenic, with evidence of a neurotransmission imbalance and disease progression. Patients often take multiple agents concurrently, with incomplete therapeutic success, particularly with regard to depression. Suicide is common. Of the hypotheses regarding the action of mood stabilizers in BD, the "arachidonic acid (AA) cascade" hypoth…

Dysfunctional beliefs in bipolar disorder: hypomanic vs. depressive attitudes.

BACKGROUND: To date the effect of cognitive behavioural therapy modified for bipolar disorder has been inconsistent and sometimes disappointing. However studies exploring cognitive style in bipolar disorder have not identified the unique patterns of beliefs specific to bipolar disorder. The current study examines whether Mansell's hypomania-related dysfunctional belief scale specifically identifies bipolar disorder patients. METHOD: Forty remitted bipolar patients, twenty remitted unipolar patients and twenty healthy controls completed the Hypomanic Attitudes and Positive Prediction Inventory…

P2RX7: A bipolar and unipolar disorder candidate susceptibility gene?

The chromosomal region 12q24 has been previously implicated by linkage studies of both bipolar disorder and unipolar mood disorder and we have reported two pedigrees segregating both bipolar disorder and Darier's disease that show linkage across this region. The gene P2RX7 is located in this chromosomal region and has been recently reported as a susceptibility gene for bipolar disorder and unipolar depression. The non-synonymous SNP rs2230912 (resulting in amino-acid polymorphism Q460R) showed the strongest association and has been postulated to be pathogenically relevant. We have investigated…

Where should bipolar disorder appear in the meta-structure?

BACKGROUND: The extant major psychiatric classifications, DSM-IV and ICD-10, are purportedly atheoretical and largely descriptive. Although this achieves good reliability, the validity of a medical diagnosis is greatly enhanced by an understanding of both risk factors and clinical history. In an effort to group mental disorders on the basis of risk factors and clinical manifestations, five clusters have been proposed. The purpose of this paper is to consider the position of bipolar disorder (BPD), which could be either with the psychoses, or with emotional disorders, or in a separate cluster. …

Weight gain with atypical antipsychotic drugs in bipolar disorder.

Diana is a 21-year-old student who lives with her parents. She had been enrolled in college but has taken a temporary leave of absence due to the instability of her bipolar disorder. Since being diagnosed at age 18, Diana's treatment team has been unable to find a medication regimen that would maintain her stability. She has had several breakthrough manic episodes that resulted in significant risky and inappropriate behaviors on campus. The cognitive, motivational, and energy limitations of depression left her unable to keep up with school requirements. Dr. Spiker, her regular psychiatrist, pr…

Treatment options for children and adolescents with bipolar disorder.

Bipolar disorder is an increasingly common diagnosis in children and adolescents. Although psychosocial interventions are important for these young patients, treatment guidelines presently focus on pharmacologic therapy in the acute treatment of pediatric bipolar I disorder. Placebo-controlled studies have been conducted with lithium, anticonvulsants, and atypical antipsychotics, but more research is needed, especially in areas other than acute manic and mixed episodes. Additional studies are needed to determine the safest and most effective agents for treating children and adolescents with bi…

Adjunctive psychotherapy for bipolar disorder: state of the evidence.

OBJECTIVE: Psychotherapy has long been recommended as adjunctive to pharmacotherapy for bipolar disorder, but it is unclear which interventions are effective for which patients, over what intervals, and for what domains of outcome. This article reviews randomized trials of adjunctive psychotherapy for bipolar disorder. METHOD: Eighteen trials of individual and group psychoeducation, systematic care, family therapy, interpersonal therapy, and cognitive-behavioral therapy are described. Relevant outcome variables include time to recovery, recurrence, duration of episodes, symptom severity, and …

Increased serum neurotrophin-4/5 levels in bipolar disorder.

Neurotrophins are central to several aspects of central nervous system function, and emerging evidence links these growth factors to mood disorders. The purpose of this study was to investigate serum neurotrophin-4/5 (NT-4/5) levels in patients with bipolar disorder, both within mood episodes and in euthymia. Patients with bipolar I disorder (n=154) and controls (n=30) had their NT-4/5 serum levels assayed using an ELISA. Levels of NT-4/5 levels were significantly higher in bipolar disorder patients than in controls; NT-4/5 levels were increased in mania, depression and euthymia, but not signi…

Decision making in bipolar disorder: a cognitive modeling approach.

A formal modeling approach was used to characterize decision-making processes in bipolar disorder. Decision making was examined in 28 bipolar patients (14 acute and 14 remitted) and 25 controls using the Iowa Gambling Task (Bechara et al., 1994), a decision-making task used for assessing cognitive impulsivity. To disentangle motivational and cognitive aspects of decision-making processes, we applied a formal cognitive model to the performance on the Iowa Gambling Task. The model has three parameters: The relative impact of rewards and punishments on evaluations, the impact of recent and past p…
