Osseous microarchitecture of the scaphoid: Cadaveric study of regional variations and clinical implications.

Bone strength and structure are closely associated with fracture and screw fixation, however osseous micro architecture on scaphoid has not been clearly addressed. We conducted histomorphometric study of the scaphoid using micro CT to find regional variations and differences in the scaphoid to provide better understanding of fracture mechanism and suggest optimal screw position. We divided scaphoid into eight regions and collected regional data from eleven different cadaveric scaphoids. A computer program was used to measure parameters, which includes mean subchondral bone thickness, bone mine

Biomechanical analysis of two fixation methods for proximal chevron osteotomy of the first metatarsal.

PURPOSE: The proximal chevron osteotomy provides high correctional power. However, relatively high rates of dorsiflexion malunion of up to 17 % are reported for this procedure. This leads to insufficient weight bearing of the first ray and therefore to metatarsalgia. Recent biomechanical and clinical studies pointed out the importance of rigid fixation of proximal metatarsal osteotomies. Therefore, the aim of the present study was to compare biomechanical properties of fixation of proximal chevron osteotomies with variable locking plate and cancellous screw respectively. METHODS: Ten matched

Degradation and tissue replacement of an absorbable polyglycolide screw in the fixation of rabbit femoral osteotomies.

Degradation and tissue replacement of a totally absorbable polyglycolide screw, 4.5 millimeters in diameter and thirty millimeters in length, were studied histologically, morphometrically, and radiographically at sequential stages of resorption at as long as thirty-six weeks after fixation of a transverse distal femoral osteotomy in rabbits. The initial mean shear force to failure was 95.0 newtons for the specimens that had been fixed with the polyglycolide screw compared with 257.0 newtons for the distal part of the contralateral, intact femur. The physical appearance of the screw was unalter

Cannulated vs solid screws in the distal block of a lateral condylar plate does not change distal femur fracture strain.

INTRODUCTION: Distal femur fractures are common in the geriatric population with a concurrent high risk of mortality. When treating these injuries, surgeons consider interfragmentary strain at the fracture site an important variable due to its large influence on the potential for fracture healing. The lateral condylar plate is a common implant used to fix distal femur fractures. Modulating different properties of the lateral condylar plate has the potential to change the interfragmentary strain. Our primary research question was therefore whether fracture-site strain in osteoporotic distal fem

Biomechanics of internal fixation in Hoffa fractures - A comparison of four different constructs.

OBJECTIVE: Compare the biomechanical effectiveness of four different bone-implant constructs in preventing fracture displacement under axial loading. METHODS: Twenty artificial femora had a standardized coronally oriented fracture of the lateral femoral condyle, representing a Hoffa fracture classified as a Letenneur type I. Four different fixation constructs were applied to the synthetic bones for biomechanical testing. The constructs consisted of a posterolateral (PL) buttressing locking plate in conjunction with two cannulated lag screws inserted from posterior to anterior (PA) - Group 1;

3D Topology Optimization and Mesh Dependency for Redesigning Locking Compression Plates Aiming to Reduce Stress Shielding.

Current fixation plates for bone fracture treatments are built with biocompatible metallic materials such as stainless steel, titanium, and its alloys (e.g., Ti6Al4V). The stiffness mismatch between the metallic material of the plate and the host bone leads to stress shielding phenomena, bone loss, and healing deficiency. This paper explores the use of three dimensional topology-optimization, based on compliance (i.e., strain energy) minimization, reshaping the design domain of three locking compression plates (four-screw holes, six-screw holes, and eight-screw holes), considering different vo

How to Use Fluoroscopic Imaging to Prevent Intraarticular Screw Perforation During Locked Plating of Proximal Humerus Fractures: A Cadaveric Study.

OBJECTIVE: Intraarticular screw perforation is a common complication of open reduction and internal fixation of proximal humerus fractures. The purpose of this study was to (1) determine the sensitivity and specificity of the fluoroscopic images used to evaluate whether a screw tip has perforated into the glenohumeral joint, and (2) determine the specific fluoroscopic views that best evaluate screw position in the humeral head. METHODS: Twenty-two proximal humeri in 11 lightly embalmed cadavers were instrumented. The articular surface was divided into equal-sized rows (superior, central, infe

Evaluation of stresses in the mandible caused by loading of all-on-four and trefoil concepts under different occlusion conditions using finite element analysis.

OBJECTIVES: This study aimed to evaluate the maximum stresses on the bone, implant, and screw under four loading conditions in two implant-supported prosthesis concepts: Trefoil and All-on-Four. MATERIALS AND METHODS: Three-dimensional mandibular models were created based on the All-on-Four and Trefoil concepts. Maximum bite forces were applied under four conditions: maximum intercuspal position, canine-protected occlusion, group-function occlusion, and cantilever loading. The stresses on cortical bone, implants, and screws were calculated using finite element analysis. RESULTS: The highest

Stab Wound in the Skull Treated with a Medial Supraorbital Craniotomy Through an Incision in the Eyebrow-a Minimally Invasive Approach.

BACKGROUND: The surgical management of osteoporotic pelviacetabular fractures poses distinct challenges due to poor screw purchase, severe comminution of fractures, and the inability to perform prolonged surgeries in patients with significant comorbidities. These fractures necessitate tailored modifications in surgical approaches, implant selection, and techniques based on the patient's overall health, fracture complexity, and bone quality. METHODS: A comprehensive literature search was conducted using PubMed and Google Scholar databases to identify relevant articles on the management of oste

Evaluating osteoporosis and bone quality in the aging spine: modern considerations for surgical management in the geriatric population.

Surgical management paradigms of spinal pathologies in the aging population carry inherent substantial risks, with surgical complications being more prevalent among patients with osteoporosis compared to those with normal bone mineral density. In this narrative review, we aim to highlight important clinical understanding and considerations in perioperative evaluation and management of patients elected to undergo spinal surgery. Osteoporosis is a well-defined risk factor for mechanical complications following spinal surgery, and as such, perioperative optimization of bone health in the setting

Does intramedullary elastic nail augmentation increase resistance to bending stress in plate fixation of long bones? A biomechanical study on lamb cadaveric femurs.

OBJECTIVES: The aim of this study was to evaluate the bending strength of plate-screw fixation augmented with titanium elastic nails (TENs) in a simple long bone fracture model using lamb cadaver femurs. MATERIALS AND METHODS: A total of 24 lamb cadaveric femurs that were osteotomized transversely from the mid-diaphysis with a bone saw were used to obtain a simple fracture model. The femurs were divided randomly into three groups. In Group 1, only plate-screw was used for fixation. In Group 2, plate-screw fixation was augmented with a 2.5-mm TEN. In Group 3, plate-screw fixation was augmented

Simultaneous Use of Both Bilateral Intralaminar and Pedicle Screws for C2 Stabilization.

Four patients underwent stabilization surgery using both bilateral C2 pedicle screw (PS) and intralaminar screw (LS). Neural and vascular injury resulting from incorrect screw placement was assessed using computed tomography (CT). The evaluation of bone union was assessed by lateral flexion-extension X-ray films and CT. The symptoms were improved in all patients. There were no intraoperative complications. Furthermore, there were no cases of neurological worsening or vascular injury from incorrect screw placement. Failure of instrumentation or screw loosening during the follow-up period did no

Pedicle Screw With Increased Cortical Purchase Can Be Inserted With Same Accuracy as the Screw in Straightforward Trajectory Using 3D Modeling Landmarks.

STUDY DESIGN: Comparison, in terms of insertion accuracy and biomechanical performance, between an increased cortical purchase and straightforward pedicle screw trajectory. OBJECTIVE: This study aims to compare a trajectory with increased cortical purchase to the more common straightforward trajectory in terms of strength and insertion accuracy using real-time navigation. SUMMARY OF BACKGROUND DATA: In previous studies, it was suggested that pedicle screw pullout strength is strongly correlated with bone mineral density, and using a more cortical tract allows a greater portion of the denser

A biomechanical study of two different pedicle screw methods for fixation in osteoporotic and nonosteoporotic vertebrae.

In reconstruction of the osteoporotic spine, patients often show poor outcome because of pedicle screw failure. This study used osteoporotic and nonosteoporotic vertebrae to determine the difference in fixation strength between pedicle screws inserted straight forward and pedicle screws inserted in an upward trajectory toward the superior end plate (i.e., end-plate screws). There is some evidence to suggest that end-plate screws have a strength advantage. The particular focus was on osteoporotic vertebrae. Thirty-three vertebrae (T10-L2) were harvested. The bone mineral density (BMD) was measu

[Elastic plate osteosynthesis, biomechanics, indications and technique in comparison with rigid osteosynthesis].

Classical stable plate osteosynthesis with its anatomical repositioning, absolute stability between fragments and medial support should only be applied to joint fractures and spongy bone. In cortical bone, the anatomical reposition connected with the plate promotes bone necrosis along the fracture and prevents callus formation. Direct cortical synthesis, a method also known as "primary bone healing", serves the bone's revascularisation and is not necessarily aimed at healing. Thus, this may also be termed "necrosis healing". Along the shaft of long bones, elastic plate osteosynthesis, a biolog

Early post-operative performance of an anatomically designed hybrid thread interference screw for ACL reconstruction: A comparative study.

Although the anterior cruciate ligament (ACL) reconstruction using interference screw is a well-accepted surgical procedure, patients still suffer graft failure in the initial rehabilitation phase. Graft fixation stability of a newly designed anatomical hybrid thread tapered interference screw (AHTTIS) was compared with a conventional standard one (CSIS) by conducting in-vitro mechanical tests. According to the CSIS manufacturer's instruction, eight tapered bone tunnels, matching AHTTIS geometry, and eight straight cylindrical tunnels were drilled in artificial bone blocks. Bovine tendon graft

Preliminary evaluation of the Sternum Screw: a novel method for improved sternal closure to prevent dehiscence.

Sternal dehiscence is a relatively rare but serious complication of sternal closure with an unacceptable mortality and morbidity rate. The 6 mm cannulated Sternum Screws are a novel approach that aims to prevent dehiscence. The screws are placed on either side of the sternotomy and wire threaded through the cannula and closed customarily. The Sternum Screws make the bone non-penetrable at the same time retaining the familiarity of conventional stainless steel wire. This novel technique was tested in sheep sterna to compare its efficacy to conventional wire closure. Using tensile testing in a t
