Changes in intrinsic functional brain networks following blast-induced mild traumatic brain injury.

OBJECTIVE: Blast-induced mild traumatic brain injuries (mTBI) commonly go undetected by computed tomography and conventional magnetic resonance imaging (MRI). This study was used to investigate functional brain network abnormalities in a group of blast-induced mTBI subjects using independent component analysis (ICA) of resting state functional MRI (fMRI) data. METHODS: Twenty-eight resting state networks of 13 veterans who sustained blast-induced mTBI were compared with healthy controls across three fMRI domains: blood oxygenation level-dependent spatial maps, time course spectra and function…

Examination of blood-brain barrier (BBB) integrity in a mouse brain tumor model.

The present study evaluates, both functionally and biochemically, brain tumor-induced alterations in brain capillary endothelial cells. Brain tumors were induced in Balb/c mice via intracranial injection of Lewis Lung carcinoma cells into the right hemisphere of the mouse brain using stereotaxic apparatus. Blood-brain barrier (BBB) permeability was assessed at various stages of tumor development, using both radiolabeled tracer permeability and magnetic resonance imaging with gadolinium diethylene-triamine-pentaacetate contrast enhancement (Gad-DTPA). The expression of the drug efflux transport…

Brain pericytes from stress-susceptible pigs increase blood-brain barrier permeability in vitro.

BACKGROUND: The function of pericytes remains questionable but with improved cultured technique and the use of genetically modified animals, it has become increasingly clear that pericytes are an integral part of blood-brain barrier (BBB) function, and the involvement of pericyte dysfunction in certain cerebrovascular diseases is now emerging. The porcine stress syndrome (PSS) is the only confirmed, homologous model of malignant hyperthermia (MH) in veterinary medicine. Affected animals can experience upon slaughter a range of symptoms, including skeletal muscle rigidity, metabolic acidosis, t…

Brain surface motion imaging to predict adhesions between meningiomas and the brain surface.

INTRODUCTION: "Brain surface motion imaging" (BSMI) is the subtraction of pulse-gated, 3D, heavily T2-weighted image of two different phases of cerebrospinal fluid (CSF) pulsation, which enables the assessment of the dynamics of brain surface pulsatile motion. The purpose of this study was to evaluate the feasibility of this imaging method for providing presurgical information about adhesions between meningiomas and the brain surface. METHODS: Eighteen cases with surgically resected meningioma in whom BSMI was presurgically obtained were studied. BSMI consisted of two sets of pulse-gated, 3D,…

Ethical brain stimulation - neuroethics of deep brain stimulation in research and clinical practice.

Deep brain stimulation (DBS) is a clinically established procedure for treating severe motor symptoms in patients suffering from end-stage Parkinson's disease, dystonia and essential tremor. Currently, it is tested for further indications including psychiatric disorders like major depression and a variety of other diseases. However, ethical issues of DBS demand continuing discussion. Analysing neuroethical and clinical literature, five major topics concerning the ethics of DBS in clinical practice were identified: thorough examination and weighing of risks and benefits; selecting patients fair…

Prospective assessment of patient-rated symptoms following whole brain radiotherapy for brain metastases.

To prospectively assess patient-rated symptoms in patients with brain metastases treated with whole brain radiotherapy, these patients were asked to rate their symptoms on the Edmonton Symptom Assessment Scale (ESAS) before, and 1, 2, 4, 8, and 12 weeks following the radiation treatment. ESAS evaluates pain, fatigue, nausea, depression, anxiety, drowsiness, appetite, sense of well-being, and shortness of breath on a scale of 0-10 (0=absence of symptom and 10=worst possible symptom). Patients with a language barrier or significant cognitive impairment were excluded. The mean difference of ESAS …

Brain asymmetries in autism and developmental language disorder: a nested whole-brain analysis.

We report a whole-brain MRI morphometric survey of asymmetry in children with high-functioning autism and with developmental language disorder (DLD). Subjects included 46 boys of normal intelligence aged 5.7-11.3 years (16 autistic, 15 DLD, 15 controls). Imaging analysis included grey-white segmentation and cortical parcellation. Asymmetry was assessed at a series of nested levels. We found that asymmetries were masked with larger units of analysis but progressively more apparent with smaller units, and that within the cerebral cortex the differences were greatest in higher-order association c…

Ethics roundtable debate: child with severe brain damage and an underlying brain tumour.

A young person presents with a highly malignant brain tumour with hemiparesis and limited prognosis after resection. She then suffers an iatrogenic cardiac and respiratory arrest that results in profound anoxic encephalopathy. A difference in opinion between the treatment team and the parent is based on a question of futile therapy. Opinions from five intensivists from around the world explore the differences in ethical and legal issues. A Physician-ethicist comments on the various approaches.

Effects of brain hypothermia on brain edema formation after intracerebral hemorrhage in rats.

Patients with intracerebral hemorrhage (ICH) may deteriorate progressively after the initial ictus because of the brain edema around the hematoma. Recently, thrombin has become known to play an important role in the brain edema formation after ICH. In this study, we examined the effect of brain hypothermia on brain edema formation after hematoma and thrombin injection into the brain in rats and clarified the mechanism of hypothermia on brain damage. Anesthetized Sprague-Dawley rats received an injection of 100 microL of autologous blood or 10 units of bovine thrombin into the basal ganglia. An…

Brain death and transcranial Doppler: experience in 130 cases of brain dead patients.

BACKGROUND AND PURPOSE: Diagnosis of brain death requires confirmation of the clinical diagnosis by appropriate tests, generally electroencephalography (EEG) and angiography. The diagnostic limitations or logistical problems inherent to these tests indicate the need to develop other more appropriate methods. The results obtained with transcranial Doppler (TCD) led us to conduct this prospective study of TCD recordings in brain dead patients. METHODS: 130 patients, aged 2-88 years were diagnosed as brain dead between July 1987 and June 1993. Clinical criteria were confirmed in all cases by EEG…

Correlation among lipid peroxidation, brain energy metabolism and brain oedema in cerebral ischaemia.

In order to study the involvement of lipid peroxidation in the generation of ischaemic cell damage, sequential measurements were made on the … able states during the breakdown of lipid hydroperoxide. The recovery of ATP in the 5 min ischaemic brain after reflow with ATP stopped at approx. 60% of the pre-ischaemia level, whereas the water content of the cortex showed an increase after 5 min of recirculation, and a gradual recovery thereafter for 30 min. There was a strong correlation between th …

Brain-stem auditory evoked potentials in children with brain-stem or cerebellar dysfunction.

Brain-stem auditory evoked potentials (BAEPs) were recorded in 23 children who had signs of brain-stem or cerebellar dysfunction. In patients with brain-stem gliomas, BAEPs were abnormal in all except one, in whom involvement of the brain-stem auditory pathway was limited to the midbrain tectum. The BAEPs were normal in neuronal ceroid lipofuscinosis, but abnormal bilaterally in inheritable leukoencephalopathies. All patients with Leigh's encephalopathy had BAEP abnormalities; in two, abnormalities occurred before the appearance of lesions on computed tomographic scan. Patients with Friedreich…

[Concentration of antibiotics (cefoperazone) in human brain, brain tumor tissue and cerebrospinal fluid].

The transfer of cefoperazone (CPZ) into cerebrospinal fluid (CSF), brain or brain tumor tissue was studied in 13 cases with brain tumor, chronic subdural hematoma and benign intracranial hypertension in 1982. The peak values of CPZ in serum came up immediately after its rapid intravenous administration and then decreased exponentially. The concentration of CPZ in CSF started to increase with a long delay of about 60 min. The average peak level in CSF remained 21.6 micrograms/ml and corresponded to 10.3% of the peak level in serum. The best transfer of chloramphenicol into CSF has been reported…

Electroshock effects on brain protein synthesis: relation to brain seizures and retrograde amnesia.

The effects of electroshock on brain seizure activity and brain protein synthesis were studied in male mice. A significant but short-lasting inhibition of brain protein synthesis and an increase in the amount of free leucine were produced by electroshock at intensities above the brain seizure threshold. Electroshock at intensities below the brain seizure threshold did not affect brain protein synthesis.

Ultrasonic head-mounted device spatiotemporal opening blood-brain barrier enhances the brain permeation of drugs for treatment of radiation-induced brain injury.

The field of physical therapy is advancing and using focused ultrasound to deliver drugs into the brain gains growing interest. However, the blood-brain barrier makes it difficult for drugs to enter. Finding safe and efficient physical therapy strategies to complement drug treatments is essential. Here, the rule and molecular mechanisms of spatiotemporal opening blood-brain barrier of ultrasound were explored using a Bluetooth-controlled ultrasonic head-mounted device which was used to enhance the brain permeation of drugs for the treatment of radiation-induced brain injury. The falling-off of…

BRAIN-DISC: a novel analytical approach integrating data-driven brain pattern discovery with cohort studies for evaluating findings in brain modulation interventions.

Brain modulation interventions (BMIs) targeting cognitive and neuropsychiatric symptoms have shown substantial heterogeneity in response, limiting their clinical utility. Resting-state functional connectivity (rsFC) may capture BMI-induced neuroplasticity and support patient stratification. However, examining these biomarkers within small, heterogeneous intervention samples remains challenging. The objective of this study is to present BRAIN-DISC, an analytic framework that links large-scale cohort-derived rsFC patterns with evaluation in targeted BMI trials. Three demonstrations were conducte…

Nanoparticles Coated with Brain Microvascular Endothelial Cell Membranes can Target and Cross the Blood-Brain Barrier to Deliver Drugs to Brain Tumors.

The blood-brain barrier (BBB) contains tightly connected brain microvascular endothelial cells (BMECs) that hinder drug delivery to the brain, which makes brain tumors difficult to treat. Previous studies have shown that nanoparticles coated with tumor cell membranes selectively target their homologous tumors. Therefore, this study investigated whether bEnd.3-line BMEC membrane-coated nanoparticles with poly(lactide-co-glycolide)-poly(ethylene glycol)-based doxorubicin-loaded cores (BM-PDs) can be used to target BMECs and cross the BBB. In vitro, the BM-PDs effectively target BMECs and cross a…
