Neurotoxic Effects of Ammonia in a Patient With Ornithine Transcarbamylase Deficiency and Bilateral Brain Abscesses: Case Report.

Brain abscesses are a rare complication of dental procedures. High concentrations of ammonia in brain abscesses may increase vasogenic edema and other brain abscess symptoms. Ornithine transcarbamylase deficiency (OTCD) is an x-linked genetic disorder of the urea cycle associated with an increased risk of brain damage due to hyperammonia. During acute metabolic decompensations, due to stresses such as infection in OTCD patients, blood ammonia levels become moderately high. This, in turn, causes cerebral glutamine levels to increase and exacerbate cerebral edema and neurological symptoms. In th…

A Case of Streptococcus anginosus Brain Abscess Caused by Contiguous Spread from Sinusitis in an Immunocompetent Patient.

Brain abscesses are infections of the brain parenchyma that can arise from either contiguous spread from local infection or by hematogenous spread from a distant site. Streptococcus anginosus of the Streptococcus anginosus group (SAG) is a commensal microbe of the mucosae of the oral cavity, gastrointestinal tract, and urogenital tract. We present a case of mono-microbial brain abscess caused by contiguous spread from relatively asymptomatic sinusitis that initially presented as a subdural hemorrhage on computed tomography. A 70-year-old male presented, obtunded, with a Glasgow Coma Score of e…

Spontaneous intraventricular rupture of pyogenic brain abscess: A short series of three cases and review of literature.

BACKGROUND: Spontaneous intraventricular rupture of brain abscess (IVROBA) is a dreaded complication of pyogenic brain abscess (PBA) and is associated with very high mortality. We discuss the clinical, radiological, and therapeutic aspects associated with this potentially fatal complication of PBAs. CASE DESCRIPTIONS: Three cases of spontaneous IVROBA presenting to our institute over a period of 6 months were reviewed with respect to their clinical and radiological presentation, their therapeutic plan, and neurological outcome. Individualized approach to our patients with IVROBA with abscess …

Bacillus cereus bacteremia and multiple brain abscesses during acute lymphoblastic leukemia induction therapy.

Bacillus cereus can cause serious infections in immunosuppressed patients. This population may be susceptible to B. cereus pneumonia, bacteremia, cellulitis, and rarely cerebral abscess. Here we report an 8-year-old boy undergoing induction therapy for acute lymphoblastic leukemia who developed multifocal B. cereus cerebral abscesses, highlighting the propensity for B. cereus to develop cerebral abscesses. A review of the literature over the past 25 years identified another 11 cases (3 children and 8 adults) of B. cereus cerebral abscess in patients undergoing cancer therapy. B. cereus cerebra

Effect of rehabilitation on a patient suffering from a tuberculous brain abscess with Gerstmann's syndrome: case report.

There are few reports in the literature of tuberculous brain abscess. Tuberculous brain abscess usually occurs in an immunocompromised host. Almost all previously documented cases have involved acquired immune deficiency syndrome. We encountered a 53-year-old right-handed immunocompetent male who was initially suspected of having a cerebrovascular accident due to acute-onset right hemiparesis and paresthesia. A tentative diagnosis of brain tumor versus brain abscess was made on imaging studies. The patient was finally diagnosed with a tuberculous brain abscess based upon deterioration on imagi…

Multiple brain abscesses: an initial presentation in a child with an undiagnosed sinus venosus atrial septal defect.

Cerebral abscess is a serious condition and in children the commonest cause is cyanotic congenital heart disease. We report a 12-year old male who presented with multiple brain abscesses. After an extensive evaluation including a transesphageal echocardiogram (TEE), we found a previously undiagnosed sinus venous atrial septal defect (ASD). He was treated medically with antibiotics. This is an unusual initial presentation of asymptomatic acynotic congenital heart disease in children. We recommended a complete cardiac evaluation including TEE with saline contrast study in the work up of an idiop…

Undiagnosed patent foramen ovale presenting as a cryptogenic brain abscess: case report and review of the literature.

A 43-year-old man with no significant medical history but poor oral hygiene presented with fever and new-onset tonic-clonic seizures secondary to a left parieto-occipital brain abscess defined by computed tomography, magnetic resonance imaging, and surgical evacuation. A comprehensive workup looking for a source of infection was unremarkable including computed tomography of the chest, abdomen, and pelvis; blood cultures; and a tagged white blood cell scan. A transesophageal echocardiogram bubble study revealed the presence of a patent foramen ovale (PFO) but no other abnormalities. Culture of …

[Role of non-spore-forming anaerobic microflora in the onset and development of otogenic abscesses of the brain and cerebellum].

The role of non-spore-forming anaerobic microflora in development of otogenic cerebral and cerebellar abscesses was studied in 49 patients with brain abscesses. Microfloral study of the middle ear discharge and content of cerebral abscess demonstrates that at present the role of pathogenic pyogenic staphylococci, streptococci and other kinds of purulent infection is lower than that of previously unreported non-spore-forming anaerobs in the pus of otogenic brain abscesses. It means that urgent sanation of the primary focus and cerebral abscesses should be supplemented with targeted combined ant…

Successful treatment of brain abscess caused by Nocardia in an immunocompromised patient after failure of co-trimoxazole.

Disseminated infection caused by Nocardia asteroides is a fairly rare entity occurring mostly in immunocompromised states. Metastatic brain abscesses are a frequent and ominous complication. We report on a patient whose underlying disease was stage II pulmonary sarcoidosis. He acquired disseminated N. asteroides infection while on immunosuppressive therapy with prednisolone. After the generally recommended therapy with co-trimoxazole (trimethoprim/sulfamethoxazole) proved ineffective in controlling his brain abscesses, the lesions of the central nervous system completely resolved under a combi…

Aspergillus fumigatus brain abscess in an immunocompromised patient receiving a second-generation Bruton Tyrosine Kinase inhibitor (BTKi).

Chronic lymphocytic leukemia (CLL) is a frequent type of adult leukemia. Bruton Tyrosine Kinase inhibitors (BTKi) are first-line treatments … patient with CLL, with an in-range absolute neutrophil count and an isolated Aspergillus fumigatus brain abscess mimicking a brain tumor. The lesion was excised "en bloc", and lesion cultures revealed Aspergillus fumigatus sensitive to amphotericin and voriconazole. The patient received postoperative treatment with voriconazole. While invasive fungal infection …

Odontogenic Brain Abscess in a Hereditary Haemorrhagic Telangiectasia (HHT) Patient: Case Report with a Comprehensive Literature Review.

BACKGROUND: Recent diagnostic methods have enabled the detection of often culture-negative pathogens, including anaerobic bacteria from the oral cavity. Characterising the microbial diversity and co-occurrence of bacteria in such infections is important for understanding the molecular pathophysiology in odontogenic brain abscesses. CASE PRESENTATION: We describe a case of polymicrobial odontogenic brain abscess in a 59-year-old man of Vietnamese ethnicity with a documented increased risk of brain abscess due to Hereditary Haemorrhagic Telangiectasia (HHT). The microbiological diagnostic work-…

Early secondary brain abscess formation caused by Bacteroides fragilis at the intracerebral hemorrhage site: A case report.

BACKGROUND: Brain abscess formation at the site of intracerebral hemorrhage (ICH) is extremely rare, particularly in association with anaerobic pathogens, such as Bacteroides fragilis, and its pathogenesis remains incompletely understood. Here, we report an ultimately fatal case of a brain abscess caused by this organism that developed at an ICH site. CASE DESCRIPTION: An 86-year-old woman presented with right parieto-occipital ICH accompanied by systemic inflammatory signs. Abdominal computed tomography performed at admission revealed findings consistent with sigmoid diverticulitis. Despite …

Long-term insights into brain abscess management: An 8-year study from a single center in India.

BACKGROUND: Both industrialized and developing nations continue to face difficulties in diagnosing and treating brain abscesses. Their etiology and management are still complicated and uncertain, which make treatment and result challenging. METHODS: To ascertain management, demographics, and factors influencing the outcome of subjects with brain abscesses, we performed a retrospective analysis of data from 48 individuals who underwent surgical treatment for brain abscesses at Mahatma Gandhi Hospital in Jaipur between January 2015 and December 2023 using image-guided aspiration, craniotomy, an…
