Multiple Tracheobronchial Polyposis Caused by Tracheobronchopathia Osteochondroplastica.

Tracheobronchopathia osteochondroplastica (TO) is a rare benign airway disease that is characterized by the presence of multiple rock-garden-like nodules on bronchoscopy. TO is a slowly progressive disease of the trachea and major bronchi, which is typically characterized by such symptoms as a persistent nonproductive cough, dyspnea and wheezing. The clinical features of TO are variable, and asymptomatic patients may incidentally be diagnosed during the work-up for other diseases. We herein report a rare case of TO accompanying multiple tracheobronchial polyposis in which bronchoscopic resecti…

Relapsing Polychondritis Presented with Encephalitis Followed by Brain Atrophy.

Relapsing polychondritis (RP) is a rare autoimmune disease that is characterized by inflammatory reaction of unknown etiology and destruction of cartilaginous structures. Characteristic symptoms of this disease include cartilage inflammation of the ear, nose, larynx, trachea, bronchi, joints, eyes, heart and skin. Concomitance with neurologic symptom is very rare in RP, and the detailed underlying mechanism of neurological involvement associated with RP is not fully understood. We herein described an unusual recurrent case of inflammatory brain lesions associated with RP, with attention to cli…

A rare cause of pleural effusion: ruptured primary pleural hydatid cyst.

Hydatidosis is an endemic parasitic disease in Mediterranean countries, often caused by the dog tapeworm Echinococcus granulosus. The disease predominantly affects the liver (60-70%) and lungs (30%), and the surgical management is considered as the gold standard for treatment. Besides anaphylactic reactions, the most frequent complication of the hydatid disease is rupture into neighbouring structures, often affecting the bronchi, gastrointestinal tract and peritoneal/pleural cavities, according to its location. Primary pleural hydatidosis is an extremely rare entity and we present a ruptured p…

A Kansas wheat harvest: near-fatal asphyxiation with wheat grains.

We present a patient who had a near-fatal asphyxiation with wheat particles. A previously healthy 11-year-old boy slipped into wheat being unloaded from a transport wagon and was immediately submersed. On arrival at the hospital, he was found to have grain impacted in both mainstem bronchi. A combination of flexible and rigid bronchoscopy with optical forceps and Fogarty balloon catheters was successful in clearing the airways. The removal of the particles through the use of small bronchoscopic instruments is a tedious and delicate procedure, but in this case, it resulted in a life saved.

Bronchial wall schwannoma removed by sleeve resection of the right stem bronchus without lung resection.

A 28-year-old man presented with a bronchial wall schwannoma occurring in the right main to the intermediate bronchus. The tumor was resected en-bloc with the stem bronchi without pulmonary resection. Airway reconstruction consisted of an end-to-end anastomosis of the intermedius bronchus to the right main bronchus, and an end-to-side anastomosis of the right upper lobe bronchus to the lateral wall of the trachea. Postoperative course was uneventful. Bronchoplastic procedure with preservation of the lung parenchyma seems to be the technique of choice for benign tumors occurring intramurally in…

Activity of bromhexine and ambroxol, semi-synthetic derivatives of vasicine from the Indian shrub Adhatoda vasica, against Mycobacterium tuberculosis in vitro.

The benzylamines, bromhexine and ambroxol, widely used as mucolytics, have a pH-dependent growth-inhibitory effect on Mycobacterium tuberculosis. As these compounds are concentrated in macrophages, they might exert a clinically useful effect on intracellular tubercle bacilli. This, combined with indirect effects including enhancement of lysozyme levels in bronchial secretions and levels of rifampicin in lung tissue and sputum, and possibly clearance of bacilli-laden mucus from cavities and bronchi, suggests a potentially useful adjunctive function for these agents in the therapy of tuberculosi…

[Inflammatory role of eosinophil in allergic reactions].

A role of eosinophil is thought to be an inflammatory effect, although it had been believed that the cell has a protective role by blocking mediators from the mast cell. Eosinophils release several kinds of basic and cationic proteins following receptor-mediated activation. These mediators gradually lead to tissue damage of the bronchi, skin, nerve, heart and other general organs. Bronchial asthma and eosinophilic pneumonia as well as hyper-eosinophilic syndrome is based on this pathogenesis. Potent therapy, including steroids, will be needed for long terms in these diseases with tissue eosino…

Familial congenital bronchiectasis: Williams-Campbell syndrome.

Williams-Campbell syndrome was first described in 1960 as a rare form of bronchiectasis. Its pathogenesis is characterized by the absence or markedly diminished cartilage around the bronchi. Although the familial nature was postulated early on, only one possible familial occurrence has been reported in the literature. We present two family members in whom respiratory symptoms developed within the first year of life and were found to have histopathologic changes consistent with Williams-Campbell syndrome. This world lend further support of a developmental origin for this type of bronchiectasis.

[Diffuse lung leiomyomatosis].

Eight observations of diffuse pulmonary leiomyomatosis in women were studied. This disease in a number of cases was combined with leiomyoma of the uterus. The clinical picture depends to a certain extent on which structural part of the pulmonary tissue is the predominant source of tumor growth. Disorders in the lymphatic system causes chylous pneumothorax. Hyperplasia of smooth muscle fibers occurs multicentrically: in interalveolar septae, along the small bronchi and bronchioli, in the walls of small blood and lymph vessels, intrathoracic lymph nodes. Leiomyomatosis of the lungs is of neoplas…

[Bronchial asthma in children].

Asthma is characterized by increased responsiveness of the trachea and bronchi to various stimuli. A widespread narrowing of the airways is manifested as recurrent epidoses of wheezing. Asthma in infancy can be confused with different airway diseases which produce similar symptoms and changes. The patient's history, chest x-ray, measurement of specific IgE and repeated pulmonary function testing help to establish the right diagnosis. Nebulized sympathomimetic agents, theophylline and oxygen are used in the management of acute dyspnea; environmental control, dinatrium cromoglycicum, ketotifen, …

Complications of Respiratory Surgery in the Horse.

Respiratory surgery and procedures typically focus on conditions of the upper airway that restrict airflow at rest or exercise. This article focuses on respiratory anatomy and common surgeries of the trachea and larynx in horses. The trachea serves as the airway conduit from the larynx to the bronchi. A variety of upper airway disorders can lead to obstruction and in these instances, tracheotomy is typically utilized to bypass these obstructions and maintain airway patency. Emergency and permanent tracheotomy procedures are discussed. Other common upper airway operations, such as laryngotomy, …

Use of an Endobronchial Blocker in a Patient with Tracheobronchial Anomaly for Minimally Invasive Cardiac Surgery: A Case Report.

Tracheal bronchi (TB) is a rare anomaly and is usually asymptomatic. Although it is generally not a problem when a single lumen tube is used, it may cause ventilation difficulties in the intraoperative period in procedures requiring one lung ventilation, such as minimally invasive cardiac surgery. Therefore, these difficulties may cause intraoperative and postoperative complications. While a double-lumen tube is recommended as the primary choice for one-lung ventilation in patients with TB, bronchial blockers can be used to avoid the need for tube exchange in patients who will remain intubated…

A Case of Spontaneous Pneumomediastinum in Covid-19 Pneumonia.

Spontaneous pneumomediastinum (SPM) is defined as free air in the mediastinum thought to be due to non-traumatic rupture of marginal alveoli without evidence of underlying lung disease. Secondary causes must be excluded, including perforations of the esophagus, trachea, and bronchi, due to their propensity for unfavorable outcomes. Infectious etiologies have been documented to cause pneumomediastinum, but this is most frequently observed in the setting of mechanical and noninvasive positive-pressure ventilation (NIPPV). Here we present a case of asymptomatic spontaneous pneumomediastinum in a …

Reconstruction of the Mediastinum and Tracheopexy for Tracheomalacia in Straight Back Syndrome.

Tracheomalacia in straight back syndrome results from chronic compression of the trachea and the mainstem bronchi mainly because of decreased mediastinal diameter. The mainstay of correction is the increase of mediastinal space and the restoration of the tracheal lumen and stability. Owing to the great variability of the manifestation of this disease, individualized approaches are required. We describe our approach in a 36-year-old woman with straight back syndrome associated severe tracheobronchomalacia with reconstruction of the proximal aorta, brachiocephalic artery, sternoplasty, and anter…

Mounier-Kuhn syndrome: A variable course disease.

Mounier-Kuhn syndrome or tracheobronchomegaly is a rare disease characterized by marked dilation of the trachea and proximal bronchi with recurrent lower tract respiratory infections. Computed tomography and bronchoscopy are the key tools to accomplish the diagnosis. This is a condition with a clinical polymorphism, symptoms vary from minor with preserved respiratory function, to very severe with life threatening exacerbations leading to respiratory failure and premature death. The treatment is mainly symptomatic, stenting or surgery are reserved to extreme cases.Herein, we report two cases of…

Anesthesia in Diagnostic and Therapeutic Pediatric Bronchoscopy.

Airway endoscopy (rigid and flexible bronchoscopy) is an important procedure that allows for visualization of the trachea and bronchi as well as treatment of a variety of airway disorders for diagnostic and therapeutic interventions. Excellent communication between the anesthesiologist and the endoscopist is required to ensure that adequate oxygenation and ventilation is maintained via the shared airway. Various anesthetic and airway management techniques can be used for airway management in pediatric foreign-body aspiration. This article highlights indications, techniques, and complications e…

A case of congenital orbital fibroepithelial polyp.

Fibroepithelial polyp (FEP) is a benign tumor of mesodermal origin, composed of varying amounts of stroma covered by epithelium. The skin and the genitourinary tract are the most common sites of FEP. Uncommon locations of FEP include the inferior turbinate of the nasal cavity, bronchi, and the pharynx. To date, there has been no report of orbital FEP. The authors describe a patient with congenital orbital FEP arising from the lateral canthal area. This congenital FEP was completely excised successfully through an anterior orbitotomy without any complications, with no evidence of recurrence aft…

Pulmonary lipoma in a dog.

An eight-year-old, neutered, male German short-haired pointer was presented for a chronic cough and an intrathoracic mass. Computed tomography revealed a mass with low attenuation in the right caudal lung lobe that invaded the principal bronchi. The mass was removed by right caudal and accessory lung lobectomy. The histopathological diagnosis was pulmonary lipoma. The clinical signs resolved following surgery. There was no evidence of recurrence or de novo lesions on computed tomography performed 12 months post-surgery. To the authors' knowledge, this is the first report of a pulmonary lipoma …

[Bronchial foreign bodies].

Coughing, wheezing, dyspnea and recurrent pneumonia can be signs of foreign body aspiration. About 80% of all foreign body aspirations occur in children, especially in infants between 1 and 3 years of age. Although most foreign bodies are found in the bronchi they are especially dangerous in the larynx or trachea. Foreign body aspiration is less common in adults, being confirmed in only about 1-2 of every 1,000 bronchoscopies. The most common foreign bodies are foods particles. Bronchoscopy is the method of choice for foreign body removal.

Endobronchial echinococcosis presenting as non-resolving pneumonia.

Hydatid disease of the lungs is caused by larval cysts of the Echinococcus tapeworm. Pulmonary cysts may occasionally invade bronchi or pleura as a result of coughing, trauma, or elevated intra-abdominal pressure. We present the case of a patient evaluated for non-resolving pneumonia whose radiographic and bronchoscopic findings were strikingly similar to those seen in pulmonary tuberculosis with endobronchial invasion; he was ultimately diagnosed with pulmonary echinococcosis. This case underscores the importance of considering unusual diagnoses even when typical features of more common condi…
