Successful radiofrequency catheter ablation of "clockwise" and "counterclockwise" bundle branch re-entrant ventricular tachycardia in the absence of myocardial or valvar dysfunction without detecting bundle branch potentials.

A case is reported of a patient with only isolated conduction abnormalities of the His-Purkinje system with no identifiable myocardial or valvar dysfunction, leading to "clockwise" and "counterclockwise" bundle branch re-entrant ventricular tachycardias (BBRVTs). The electrophysiological study showed infra-Hisian conduction system disease and two different inducible wide QRS complex tachycardias. Neither right bundle branch nor left bundle branch potentials were recorded despite extensive catheter manipulation. However, these tachycardias were diagnosed as BBRVTs by using entrainment manoeuvre

Tetralogy of Fallot: postoperative electrophysiologic studies.

Electrophysiologic studies performed during postoperative cardiac catheterization in 51 patients following repair of tetralogy of Fallot (TF) were compared with studies performed in a control group of 30 patients. These studies include His bundle electrograms, measurement of atrioventricular (A-V) conduction at progressively increasing atrial pacing rates, and evaluation of sinus node recovery time. More postoperative TF patients (20%) had prolonged H-V intervals compared to the controls (7%). With atrial pacing, no patient in the control group developed second degree A-V block below 160 beats

Overdrive suppression of conduction in the canine His-Purkinje system after occlusion of the anterior septal artery.

The purpose of these experiments was to determine whether overdrive suppression of conduction (OSC), i.e., transient worsening of conduction or development of atrioventricular block after cessation of rapid pacing, could be produced in the canine His-Purkinje system damaged by ligation of the anterior septal coronary artery and to investigate the responsible mechanism. We found that OSC occurred in vivo after rapid ventricular and His bundle pacing but not after atrial pacing, and that it occurred in vitro after rapid pacing from the left bundle branch but not after pacing from the proximal Hi

Fulguration for AV nodal tachycardia: results in 42 patients with a mean follow-up of 23 months.

This report describes a catheter ablation technique to treat atrioventricular nodal reentrant tachycardia while preserving anterograde conduction, and its application in 42 patients with drug-refractory repetitive episodes of tachycardia. One of these patients had common and reverse forms of tachycardia. Using atrial activation in the His-bundle lead as a reference, the optimal ablation site was selected by positioning an electrode catheter to obtain a synchronous or earlier atrial activation than the reference during tachycardia. At this site, His-bundle deflection was completely absent, or w

Physiologic Pacing in Heart Failure.

Cardiac physiologic pacing, also known as cardiac resynchronization therapy, is indicated in patients with heart failure, reduced left ventricular ejection fraction (LVEF) of 50% or less, and either a high (or anticipated high) ventricular pacing burden or a wide QRS complex. Traditionally, physiologic pacing has been achieved with biventricular pacing with a right ventricular lead and a coronary sinus branch lead. Randomized trials involving more than 10,000 patients with heart failure have shown clinical, exercise, and quality-of-life benefits associated with biventricular pacing, as well as

Clinical Outcomes and Electrophysiological Characteristics of Partial Perforation After Left Bundle Branch Area Pacing.

BACKGROUND: Septal perforation, defined as partial or complete protrusion of a lead helix, is a potential complication of left bundle branch area pacing, theoretically increasing risks of pacing failure and thromboembolism. However, no studies have examined the long-term prognosis of patients with partial perforations (PPs). OBJECTIVES: This study aimed to elucidate the incidence, outcomes, and electrophysiological characteristics of PP in clinical and experimental swine studies. METHODS: Patients requiring pacing who underwent successful left bundle branch area pacing were retrospectively i

The Postoperative S Wave in Lead V5 and/or V6 Predicts Better Clinical Outcomes in Heart Failure Patients With Left Bundle Branch Area Pacing.

BACKGROUND: S wave in lead V5/6 has been reported as a marker indicated for successful left bundle branch (LBB) capture. This study aimed to evaluate the value of the S wave in lead V5/6 in predicting the long-term clinical outcomes in heart failure (HF) patients treated with left bundle area pacing (LBBAP). METHODS: Consecutive HF patients receiving LBBAP were prospectively enrolled and followed at least 2 years. ECG were analyzed to identify the S wave in lead V5/6. The composite end point was all-cause mortality, HF hospitalization, and malignant ventricular arrhythmias. Non-response to LB

Septal Branch Occlusion Leading to Complete Heart Block Post-Percutaneous Intervention: A Cautionary Tale in Left Bundle Branch Block Patients.

A 66-year-old man with a baseline electrocardiogram (ECG) showing left bundle branch block developed persistent complete atrioventricular block following stent placement in the proximal left anterior descending artery (LAD), necessitating permanent pacemaker implantation. The complication is believed to have resulted from occlusion of the second septal branch, which likely supplied the right bundle of the His system. During elective interventions targeting the LAD in patients with pre-existing conduction abnormalities, it is crucial to maintain adequate blood flow to the septal perforators.

Mid-term clinical outcomes of left bundle branch area pacing compared to accurate right ventricular septal pacing.

BACKGROUND: Although left bundle branch area pacing (LBBAP) reportedly results in fewer adverse outcomes after implantation than conventional stylet-guided right ventricular septal pacing (RVSP), previous studies have not compared LBBAP with accurate RVSP using a delivery catheter. The aim of this study was to compare clinical outcomes between LBBAP and accurate RVSP among patients with atrioventricular block (AVB). METHODS: This single-center observational study enrolled 160 patients requiring RV pacing due to symptomatic AVB between September 2018 and December 2021. Primary composite outcom

Use of extendable helix leads for conduction system pacing: Differences in lead handling and performance lead design impacts conduction system pacing.

INTRODUCTION: Pacing leads with extendable-retractable helix (EHL) are alternatives to fixed-helix leads (FHL) for conduction system pacing (CSP), but data on handling characteristics are limited. This study evaluated a dual-center experience of lead handling and performance during CSP. METHODS AND RESULTS: Consecutive patients with His-bundle pacing (HBP) or left bundle branch pacing (LBBP) were evaluated for the primary outcome of lead failure, defined as structural damage to the lead necessitating lead replacement. Differences in pacing characteristics were compared. Among 280 patients (me

Dual-chamber ICD for left bundle branch area pacing: the cardiac resynchronization and arrhythmia sensing via the left bundle (cross-left) pilot study.

BACKGROUND: Left bundle branch area pacing (LBBAP) has emerged as a promising technique to deliver cardiac resynchronization therapy (CRT). However, safety and efficacy of ventricular arrhythmia sensing via the left bundle in implantable cardioverter-defibrillator (ICD) recipients remain unclear. We sought to evaluate the feasibility of a single LBBAP lead connected to a dual-chamber ICD in patients indicated with a CRT-D implantation. METHODS: The CROSS-LEFT pilot study prospectively included 10 consecutive patients with a reduced ejection fraction and a complete left bundle branch block, in

A novel 9-partition method using fluoroscopic images for guiding left bundle branch pacing.

BACKGROUND: Left bundle branch (LBB) pacing is a novel pacing modality, but there is no standard fluoroscopic methodology. OBJECTIVES: This study aimed to analyze the characteristics of His bundle (HB) and LBB pacing lead locations and establish a method to guide LBB pacing using fluoroscopic images. METHODS: Seventy patients who underwent HB or LBB pacing were enrolled. The fluoroscopic image was recorded, and ventricular contraction ring in the right anterior oblique 30° projection was determined. The region between the apex and the ventricular contraction ring was divided into 9 partition

[Indications for electrophysiological investigation of tachycardia].

Cardiologists are often confronted by the situation of discussing the indications for invasive electrophysiological procedures. In this field, the recommendations of American scientific societies have been published but they are relatively dated and require updating. These procedures require the introduction of one or more catheters by a femoral venous or arterial approach, or by the endoesophageal route for positioning at different sites in the heart. The first stage of the procedure is usually to evaluate the electrophysiological properties of the cardiac chamber under investigation; followe

Disorders of impulse conduction and impulse formation caused by hyperkalemia in man.

In eight patients we have demonstrated manifold types of impairment of impulse conduction produced by hyperkalemia. These abnormalities of impulse conduction occurred either simultaneously or in sequence, and were located in the atria, in the A-V junction, in the fascicular distribution of the ventricular conduction system, or in the free ventricular walls. In association with the abnormalities of conduction, abnormal impulse formation was also frequently observed as manifested by acceleration of normal pacemakers or the emergence of ectopic pacemakers. In one patient hyperkalemia produced alt

A Curious Change in QRS Width: Timing Is Everything.

CASE BACKGROUND: A 61-year-old man with a mechanical aortic valve developed Staphylococcus aureus bacteremia. His electrocardiogram showed sinus rhythm with atrioventricular Wenckebach and phase 4 left bundle branch block. He was subsequently found to have a peri-aortic root abscess that was likely eroding not only into the atrioventricular node, but into the left bundle fibers as well. TAKE-HOME MESSAGES: Phase 4 block is a pause-dependent conduction block that reflects underlying His-Purkinje system disease. During a critical pause, spontaneous phase 4 depolarization in a diseased His-Purki

[Auricular fibrillation and flutter with slow ventricular frequency. Clinical and electrophysiological study].

We have studied atrio-ventricular conduction in 9 patients with atrial fibrillation and flutter and ventricular response below 90 per minute, by means of His bundle electrograms. Despite the finding of intraventricular conduction defects in 6 cases, the block of the atrial impulses occurred at the high atrio-ventricular junction in every case. In four patients the arrhythmia was an incidental finding and there were no other signs of cardiac disease. Several patients tolerated low ventricular rates for years without symptoms, but in two cases it was necessary to insert a pacemaker to treat cere

Diagnostic electrocardiographic trap: A case of conduction system pacing.

His-bundle pacing (HBP) is a relatively new method of cardiac pacing, with recent studies showing an association between HBP and a lower risk of developing right ventricular heart failure compared to classical pacing methods. However, HBP is also associated with a higher risk of lead dislodgement, undersensing, and loss of capture. As such, a detailed assessment of pacing effectiveness in pacemaker patients is vital. In the presented case, an electrocardiogram (ECG) recording seems to present successful selective His-bundle pacing, while pacemaker follow-up demonstrated the loss of ventricular

[A histologic study on prevention of surgical conduction disturbance, with special reference to the relationship between the central portion of the A-V bundle and adjacent structures].

UNLABELLED: The present study was aimed to examine the course of the proximal portion of the His bundle (HB). Twenty-six hearts was histologically investigated, 5 normal, 13 with isolated perimembranous ventricular septal defect (VSD) and with tetralogy of Fallot (TOF). Following items were analyzed by serial sectioning: 1. Distance from axis of the nonbranching bundle (NBB) to the lower irm of VSD. 2. Deviation of the penetrating bundle (PB) and the NBB to the right ventricular septal endocardium. 3. Distance from the HB to the attachment of the tricuspid septal leaflet (ALS). 4. Level of the
