Melorheostosis of the hand with complicating bony spur formation and bursal inflammation: diagnosis and treatment.

A 37-year old woman presented with intolerable pain associated with a slowly growing mass on the dorsum of the right hand. The radiographs were characteristic of melorheostosis involving the third and fourth metacarpals, with a cortical bony projection extending from the base of the third metacarpal eroding slightly into the fourth metacarpal, associated with bursal formation. Magnetic resonance imaging (MRI) demonstrated an inflamed bursal collection adjacent to the spur. This region corresponded to the patient's severe clinical, focal pain. To our knowledge, this is the first case report in

[A case of neuroleptic malignant syndrome caused by discontinuation of psychotropic drugs].

A 71-year-old woman with schizophrenia and diabetes discontinued psychotropic drugs due to right purulent knee bursitis. Five days after discontinuation, she presented with a fever of >40°C, severe disturbance of consciousness, lead-pipe muscle rigidity, tachypnea, and hypertension. She was diagnosed with neuroleptic malignant syndrome (NMS) induced by the discontinuation of psychotropic drugs. The patient's symptoms improved after dantrolene administration and systemic management. We investigated the clinical characteristics of cases with NMS induced by the discontinuation of psychotropic dru…

Concomitant Polymyalgia Rheumatica and Large-Vessel Vasculitis Visualized on 18F-FDG PET/CT.

Polymyalgia rheumatica (PMR) and large-vessel vasculitis (LVV) are related rheumatic diseases which are occasionally present concomitantly. PMR is characterized by synovitis and bursitis. In LVV, inflammation of the blood vessel wall is seen. Both disorders can be difficult to diagnose since patients often present non-specific symptoms and results of blood tests. The non-specific symptoms cannot always be distinguished from symptoms indicating an occult malignancy. We present a case of PMR and LVV in a Scandinavian man visualized on [18F]-2-deoxy-D-glucose positron emission tomography/computed…

Staphylococcus aureus triggered reactive arthritis.

OBJECTIVES: To report two patients who developed reactive arthritis in association with Staphylococcus aureus infection. METHODS: A review of the case notes of two patients. RESULTS: Two adult female patients have developed sterile arthritis in association with Staph aureus infection. The first patient has had two episodes of arthritis; the first followed olecranon bursitis, the second followed infection of a central venous catheter used for dialysis. The second patient developed sterile arthritis while being treated for pyomyositis. Both patients had a self limited arthritis and were HLA-B2…

Treatment of soft tissue calcifications with magnesium.

80 patients with soft tissue calcifications were treated. 24 patients suffered from myositis ossificans traumatica, 23 from calcific bursitis (Duplay's disease), 6 from osteoarthropathy of elbow joint after severe craniocerebral trauma, 9 from calcifications round elbow joint after local trauma, 13 from calcifications round hip joint, 5 from calcifications in ligaments and tendons. About 75 per cent patients were cured. Calcifications disappeared or substantially diminished. Very good functional improvement followed in affected joints. Patients were treated with new method: local application o…

Light-microscopic lesions associated with naturally occurring bursal cryptosporidiosis in chickens.

Information concerning histologic significance of naturally occurring bursal Cryptosporidium sp. infection in chickens was obtained by retrospective examination of histopathology reports and clinical data from histology accessions received during 1986. Cryptosporidiosis was diagnosed in 197 bursas. In two-thirds of the accessions, more than 50% of the examined bursas were infected with Cryptosporidium sp. The histologic morphologic lesion diagnosis for Cryptosporidium sp.-infected bursas most often was marked diffuse chronic-active superficial purulent protozoal bursitis with mucosal epithelia…

Carpal boss in chronic wrist pain and its association with partial osseous coalition and osteoarthritis - A case report with focus on MRI findings.

The carpal boss is a bony prominence at the dorsal aspect of the 2(nd) and/or 3(rd) carpometacarpal joint, which has been linked to various etiologies, including trauma, os styloideum, osteophyte formation, and partial osseous coalition. It may result in symptoms through secondary degeneration, ganglion formation, bursitis, or extensor tendon abnormalities by altered biomechanics of wrist motion. We present a case of symptomatic carpal boss with the finding of a partial osseous coalition at the 2(nd) carpometacarpal (metacarpal-trapezoid) joint and highlight the magnetic resonance imaging (MRI…

Aseptic arthritis in a man with toxic shock syndrome.

Synovitis in toxic shock syndrome (TSS) is an unusual finding. A 31-year-old man presented with pain and swelling in both knees and was found to have TSS, secondary to a septic bursitis caused by Staphylococcus aureus. Immune complexes were not detected in serum or synovial fluid, and the S aureus was not recovered from the inflamed joints. Antibodies against the TSS toxin-1 were detected in serum and synovial fluid, but in lower levels than would be seen in a normal control serum. Complement studies implicated alternative pathway activation by a marked diminution in C3 levels compared with C4…

Pathomechanics, gait deviations, and treatment of the rheumatoid foot: a clinical report.

This article describes the five major foot deformities or problems often seen in patients with rheumatoid arthritis: hallux valgus, pronation of the foot, depression of the metatarsal heads, hammer or claw toes, and tendocalcaneal bursitis or subplantar spur formation. These deformities contribute to the development of common rheumatoid gait deviations such as decreased velocity, cadence, and stride length; poor heel-toe pattern; and abnormal patterns of weight bearing. Nonsurgical treatment for these problems includes joint protection methods, assistive gait devices, orthotic intervention, an…

MRI to Arthroscopy Correlations in SLAP Lesions and Long Head Biceps Pathology.

Superior labral anterior and posterior lesions and long head biceps injury are 2 common sources of shoulder pain. The presenting complaints and physical examination findings for these etiologies are often nonspecific, making diagnosis challenging. Further complicating their diagnoses, these 2 pathologies often cooccur together or with other shoulder pathologies such as subacromial bursitis, rotator cuff tear, and shoulder instability. However, with advancement in magnetic resonance imaging and shoulder arthroscopy techniques the ability to diagnose and treat these pathologies has vastly improv…

Hamstring injuries.

There is a continuum of hamstring injuries that can range from musculotendinous strains to avulsion injuries. Although the proximal hamstrin … n this article allows for the endoscopic management of proximal hamstring tears and chronic ischial bursitis, which until now has been managed exclusively with much larger open approaches. The procedure allows for complete exposure of the posterior aspect of the hip in a safe, minimally invasive fashion.

The place of musculoskeletal ultrasonography in gout diagnosis.

Gout represents a common inflammatory arthritis conditions in which the monosodium urate (MSU) crystals are deposited in joints and other tissues producing attacks of acute arthritis/ chronic arthropathy with important destructive lesions in articular or juxta-articular spaces. High resolution ultrasonography (US) is able to detect these deposits and their consequences: hyaline cartilage pathology, joint effusions, synovitis, bone erosions, tendons, bursitis, and other soft tissues involvement. Moreover, there are characteristic findings of this aggregates that can be detected by US, helping t…

Patella tendon rupture after arthroscopic resection of the prepatellar bursa--a case report.

The use of arthroscopic techniques for excision of the pre-patellar bursa has become more common in recent years for the treatment of prepatellar bursitis. The current literature includes several case series that report few complications with this technique. We report the case of a 73-year-old male who sustained a low-energy patella tendon rupture 2 months after arthroscopic resection of the prepatellar bursa. We hypothesize that during arthroscopic excision of the prepatellar bursa there was an iatrogenic injury to the patellar tendon, which contributed to the subsequent rupture. Surgical rep…

MR imaging assessment of clinical problems in rheumatoid arthritis.

Although MR imaging has been increasingly recognized as a useful tool in the diagnosis of early rheumatoid arthritis (RA) and in the assessm … nical problems derived from local extra-articular involvement, such as tenosynovitis, "rice-bodies" bursitis, and Baker's cyst rupture, are also described. Finally, we also review the value of MRI in evaluation of some complications of RA such as tendinous rupture, osteonecrosis, stress fracture, and septic arthritis/osteomyelitis.

Disorders of the hallux sesamoid complex: MR features.

Numerous painful conditions can affect the first metatarsophalangeal-sesamoid joint complex. Symptoms can be of sudden or insidious onset, and be of acute or chronic duration. Although conventional radiography is recognized as the initial diagnostic procedure for these symptoms, there is often a need to proceed to MR imaging. MR imaging is sensitive and can be utilized in the investigation of the hallux sesamoid complex to differentiate soft tissue from osseous pathology. Synovitis, tendonitis, and bursitis can be distinguished from bony abnormalities such as sesamoid fracture, avascular necro…

Streptomycin-loaded bone cement in the treatment of tuberculous osteomyelitis: an adjunct to conventional therapy.

Streptomycin-loaded bone-cement (7 g of streptomycin in 40 g of bone cement) beads were used in the treatment of tuberculous bursitis and osteomyelitis of the greater trochanter in a 76-year-old woman. Wound drainage, serum and urine concentrations of streptomycin were measured. For the first 96 hours, the streptomycin levels in the wound drainage ranged from 2932 mg/L to 414.4 mg/L, and in the serum, streptomycin levels ranged from 1.7 mg/L to 13.7 mg/L. The patient recovered without complication and at follow-up 2 years later was free of infection and walking without pain. The authors conclu…
