Ultrastructural localization of 5'nucleotidase in human normal and malignant lymphoid cells.

5'Nucleotidase (EC 3.1.3.5), a purine pathway enzyme, occurs as a cellular ectoenzyme. Widely differing 5'N activity has been reported in different types of lymphoid cells and appears to be related to lymphocyte function, differentiation and immunological subtype. This paper reports a study on the ultrastructural distribution of 5'N in in different types of lymphoid leukemias and non-Hodgkin's lymphomas. In lymphoid leukemias, only cALL cells showed strong 5'N staining. In CLL, PLL and HCL the intensity of staining was less than in normal cells. In the NHL group, the reaction pattern was mixed

Assessment of Gynecologic Emergency Triage and Appointment Wait Times: A Mystery Caller Study.

BACKGROUND: Front desk staff are often responsible for triaging patients, despite lacking formal medical training. Incorrect triage recommendations could result in patients delaying care for emergent conditions. No research exists to characterize triage accuracy in the gynecologic outpatient setting. OBJECTIVE: We investigated the ability of clinician office staff to identify emergent conditions and advise patients accordingly. Additionally, we sought to understand the influence of private versus public insurance on appointment wait time and triage recommendations. STUDY DESIGN: Obstetrician

Tricyclic antidepressant poisoning: an evidence-based consensus guideline for out-of-hospital management.

A review of U.S. poison center data for 2004 showed over 12,000 exposures to tricyclic antidepressants (TCAs). A guideline that determines the conditions for emergency department referral and prehospital care could potentially optimize patient outcome, avoid unnecessary emergency department visits, reduce healthcare costs, and reduce life disruption for patients and caregivers. An evidence-based expert consensus process was used to create the guideline. Relevant articles were abstracted by a trained physician researcher. The first draft of the guideline was created by the lead author. The enti

Water-soluble swab material for environmental sampling.

Biological threats represent a growing vulnerability to national security, affecting both military and civilian populations. Deliberate dissemination of chemical and biological (CB) agents can have immediate and significant impacts on public health and the economy, highlighting the need for faster and more efficient techniques to assess contamination and safely clear affected areas for public reentry following an accidental or intentional biological release. Current surface sampling procedures set forth by the Centers for Disease Control and Prevention (CDC) call for sampling swab materials th

Impact of abstinence on employment, education, and health outcomes in Egyptian SUD rehabilitation clients: A three-year prospective study.

BACKGROUND: SUD rehabilitation outcome studies have been carried out for decades. But no large study has been reported from Egypt or the Arab region. Freedom Drugs and HIV Programme is a large drug rehabilitation program operating since 1989. METHODOLOGY: A cohort sample of 2503 clients admitted to 28 SUD Rehabilitation centers in 2019 in Egypt was followed up in 2022. Staff member reports were validated through a randomized sample of family members feedback, and urine tests that were administered to 5% of participants (101 individuals). RESULTS: During the Year 3 follow-up, 82% of clients (

Overcoming access barriers in European CAR-T therapy: the role of decentralized manufacturing and the EASYGEN framework.

BACKGROUND: Chimeric antigen receptor T-cell (CAR-T) therapies have revolutionized the treatment of refractory B-cell malignancies, with seven approved therapies in major jurisdictions and rapid label expansion toward earlier lines of treatment. Nevertheless, patient access remains limited by multiple bottlenecks, including shortages of lentiviral vectors, manufacturing capacity constraints, and economic factors, such as prolonged production timelines, high treatment costs and associated reimbursement challenges, and long travel distances for patients to specialized CAR-T treatment centers. A review of the WHO strategy on traditional, complementary, and integrative medicine from the perspective of academic consortia for integrative medicine and health.

Despite important progress in modern medicine, widely regarded as an indispensable foundation of healthcare in all highly advanced nations and regions, not all patients respond well to available treatments in biomedicine alone. Additionally, there are concerns about side effects of many medications and interventions, the unsustainable cost of healthcare and the low resolution of chronic non-communicable diseases and mental disorders whose incidence has risen in the last decades. Besides, the chronic stress and burnout of many healthcare professionals impairs the therapeutic relationship. These

Is acidemia at birth a risk factor for functional gastrointestinal disorders?

UNLABELLED: Functional gastrointestinal disorders (FGIDs) are common in early childhood. It has been demonstrated that neonatal acidemia at delivery can lead to significant neonatal morbidity. The primary aim of this study was to evaluate the relationship between acidemia at birth and the development of FGIDs, as regurgitation, colic, and constipation, in term infants. Term newborns born at the Foggia University Hospital, Italy during the year 2020 were included in the study. As per routine clinical practice, a cord blood gas analysis on a blood sample drawn from the umbilical artery (UA) of e

Antepartum and intrapartum stillbirth rates across gestation: a cross-sectional study using the revised foetal death reporting system in the U.S.

BACKGROUND: There is a renewed call to address preventable foetal deaths in high-income countries, especially where progress has been slow. The Centers for Disease Control and Prevention released publicly, for the first time, the initiating cause and estimated timing of foetal deaths in 2014. The objective of this study is to describe risk and characteristics of antepartum versus intrapartum stillbirths in the U.S., and frequency of pathological examination to determine cause. METHODS: We conducted a cross-sectional study of singleton births (24-43 weeks) using 2014 U.S. Fetal Death and Natal

Multi-center validation of the Bowel Injury Predictive Score (BIPS) for the early identification of need to operate in blunt bowel and mesenteric injuries.

INTRODUCTION: The Bowel Injury Prediction Score (BIPS) is a tool for identifying patients at risk for blunt bowel and mesenteric injury (BBMI) requiring surgery. BIPS is calculated by assigning one point for each of the following: (1) WBC ≥ 17,000, (2) abdominal tenderness, and (3) injury grade ≥ 4 (mesenteric contusion or hematoma with bowel wall thickening or adjacent interloop fluid collection) on CT scan. A total score ≥ 2 is associated with BBMI requiring surgery. We aimed to validate the BIPS as a predictor for patients with BBMIs requiring operative intervention in a multi-center prospe

Gonorrhea, a current disease with ancient roots: from the remedies of the past to future perspectives.

Gonorrhea can be traced back to the earliest records of the human race even if Albert Neisser first described gonococcus in 1879. The Romans, Jews and Arabs all have documents referring to gonorrhea and each society had their own description of symptoms and treatment. The Roman physician Galen in 130 AD described the disease as an "involuntary escape of semen". The word itself derives from the Greek, meaning "the flow of seed". Gonorrhea is currently the second most commonly notifiable sexually transmitted infection (STI) reported to Centers for Disease Control and Prevention (CDC), second onl

Is it ethical to prevent secondary use of stored biological samples and data derived from consenting research participants? The case of Malawi.

BACKGROUND: This paper discusses the contentious issue of reuse of stored biological samples and data obtained from research participants in past clinical research to answer future ethical and scientifically valid research questions. Many countries have regulations and guidelines that guide the use and exportation of stored biological samples and data. However, there are variations in regulations and guidelines governing the reuse of stored biological samples and data in Sub-Saharan Africa including Malawi. DISCUSSION: The current research ethics regulations and guidelines in Malawi do not al

Minimizing the Risk of Disease Transmission in Emergency Settings: Novel In Situ Physico-Chemical Disinfection of Pathogen-Laden Hospital Wastewaters.

The operation of a health care facility, such as a cholera or Ebola treatment center in an emergency setting, results in the production of pathogen-laden wastewaters that may potentially lead to onward transmission of the disease. The research presented here evaluated the design and operation of a novel treatment system, successfully used by Médecins Sans Frontières in Haiti to disinfect CTC wastewaters in situ, eliminating the need for road haulage and disposal of the waste to a poorly-managed hazardous waste facility, thereby providing an effective barrier to disease transmission through a n

Trauma team activation criteria as predictors of patient disposition from the emergency department.

UNLABELLED: Many trauma centers use mainly physiologic, first-tier criteria and mechanism-related, second-tier criteria to determine whether and at what level to activate a multidisciplinary trauma team in response to an out-of-hospital call. Some of these criteria result in a large number of unnecessary team activations while identifying only a few additional patients who require immediate operative intervention. OBJECTIVES: To separately evaluate the incremental predictive value of individual first-tier and second-tier trauma team activation criteria for severe injury as reflected by patien

Medical Management and Revascularization for Asymptomatic Carotid Stenosis.

BACKGROUND: Improvements in medical therapy, carotid-artery stenting, and carotid endarterectomy call into question the preferred management of asymptomatic carotid stenosis. Whether adding revascularization to intensive medical management would provide greater benefit than intensive medical management alone is unclear. METHODS: We conducted two parallel, observer-blinded clinical trials that enrolled patients with high-grade (≥70%) asymptomatic carotid stenosis across 155 centers in five countries. The stenting trial compared intensive medical management alone (medical-therapy group) with ca

The Identification and Work-Up of Nonaccidental Trauma: Practice Variation Across US Children's Hospitals.

BACKGROUND: Nonaccidental trauma (NAT) in children is associated with several warning signs. Since fractures are the second-most common injury caused by NAT, pediatric orthopaedic surgeons are positioned to help identify and treat at-risk children. However, no standardized tools for identifying NAT exist, resulting in national practice variation. The purpose of this study was to compare NAT protocols from Children's Hospitals across the US to understand how institutions vary in screening and treatment. METHODS: We used treatment and identification recommendations for suspected child abuse fro

Seasonal variations in child mortality during the 2014-2015 Ebola epidemic in Sierra Leone.

PURPOSE: Under-five mortality is a key indicator of child health, and a measure of the quality of healthcare in a country. Seasonality affecting under-five deaths is not well understood. We reviewed seasonal variations of mortality among infants under one year, children under five years, and stillbirths in Sierra Leone using data collected continuously during the 2014-2015 Ebola outbreak in Sierra Leone. METHODS: We used Ebola and non-Ebola death data from the 117 Call Centers, laboratory testing records, burial team records, and the Viral Hemorrhagic Fever module divided into dry and rainy s…

Interobserver variability in preclinical assessment of collision variables following traffic accidents.

PURPOSE: Traffic accidents persist as a leading cause of death. European law mandates the integration of automatic emergency call systems (eCall). Our project focuses on an automated injury prediction device for car accidents, correlating technical and epidemiological input data, such as age, gender, seating position, impact on the passenger compartment, seatbelt usage, impact direction, EES, vehicle class, and airbag deployment. This study aims to explore interobserver variability in data collection quality in real accident scenarios. The assessment will evaluate the impact of user training a
