BACKGROUND: To investigate the association between BMI and the incidence of ischemic stroke in patients with symptomatic artery occlusion, and further to evaluate the utility of BMI as a screening tool for identifying candidates for extracranial-intracranial bypass surgery.
MATERIALS AND METHODS: The authors analyzed the relationship between BMI and the occurrence of ipsilateral ischemic stroke (IIS) among patients receiving only medical management in the Carotid or Middle cerebral artery Occlusion Surgery Study (CMOSS). Additionally, the authors compared the primary endpoint of CMOSS-stroke

BACKGROUND: The superficial temporal artery-middle cerebral artery (STA-MCA) bypass surgery requires an anastomosis of the STA to an MCA with diminished blood flow. However, identifying the precise location of the MCA with reduced flow preoperatively is challenging as it often remains nonvisualized. To address this issue, we developed a novel technique, the area target bypass (ATB) method, to infer the location of the responsible vessel for low-flow areas.
OBJECTIVE: The cornerstone of the ATB method lies in the utilization of the vascular fusion map (VFM). The VFM integrates 3D perfusion and

Fusiform aneurysms of the middle cerebral artery (MCA) are both relatively uncommon and challenging to treat given their pathophysiology, morphology, and anatomy (e.g., perforating arteries involvement).1,2 Endovascular treatment of fusiform MCA aneurysms can achieve good outcomes in well-selected cases.3,4 Open microsurgical strategies are effective in a case of fusiform MCA aneurysms with complex anatomy or perforator involvement.2,5,6 We demonstrate the bypass strategy for resection of a fusiform M1 MCA aneurysm (Video 1). BACKGROUND AND OBJECTIVE: Re-vascularization is an effective treatment for moyamoya disease (MMD) patients, including direct re-vascularization, indirect re-vascularization and combined re-vascularization, in which combined re-vascularization is particularly widely used. At present, there are few reports on the analysis of epilepsy after combined re-vascularization surgery. To analysis the risk factors of epilepsy in adult MMD patients after combined re-vascularization.

BACKGROUND: Patients with severe steno-occlusive disease of a main cerebral artery without causative lesions on magnetic resonance imaging (MRI) often develop cognitive impairment. However, the effects of revascularization surgery and the source of the cognitive impairment remain unclear. Therefore, we investigated the early postoperative course of cognitive function and its association with cerebral blood flow (CBF), cerebrovascular reserve (CVR), white matter disease (WMD), lacunar infarction, and cerebrovascular risk factors.
METHODS: Cognitive function was examined using neurobehavioral c

Management of unruptured intracranial aneurysms concomitant with proximal stenosis remains challenging. Video 1 demonstrates simultaneous clipping and superficial temporal artery (STA)-middle cerebral artery (MCA) bypass for unruptured MCA aneurysm concomitant with proximal stenosis. A 56-year-old man presented with paroxysmal left limb weakness for 2 years. Magnetic resonance angiography and digital subtraction angiography showed a right MCA bifurcation aneurysm concomitant with severity of proximal M1 stenosis. Arterial spin labeling imaging revealed decreased perfusion in the right frontal

BACKGROUND: Giant aneurysms are very high-risk lesions both in terms of natural history and treatment. Bypass with distal occlusion is thought to be a safe treatment option for these aneurysms. Here, we report 2 cases of aneurysm rupture after bypass and distal occlusion, review the literature, and discuss the possible underlying mechanisms, in the hopes of influencing treatment planning and averting such complications in the future.
CASE DESCRIPTION: Two patients successfully underwent surgical treatment of a giant anterior circulation aneurysm via bypass and distal vessel occlusion. In each

Moyamoya disease (MMD) involves progressive occlusion of the intracranial internal carotid artery resulting in formation of moyamoya-like vessels at the base of the brain. It can be characterized by hemorrhage or ischemia. Direct vascular bypass is the main and most effective treatment of MMD. However, patients with MMD differ from those with normal cerebral vessels. MMD patients have unstable intracranial artery hemodynamics and a poor blood flow reserve; therefore, during the direct bypass of superficial temporal artery (STA)-middle cerebral artery (MCA) anastomosis, perioperative risk facto

Acute ischemic stroke affects 3.3 of 100,000 children per year. The causes of AIS in children can be broadly divided into the following 6 categories: cardiac disese, sickle cell disease, moyamoya, arterial dissection, other arteriopathies, and other causes. Approximately 24% of the cases are classified as idiopathic. Magnetic resonance imaging (MRI) and cerebral angiography play an important role in the determining the causes of an AIS in children. Medical approaches, including anticoagulation, anti-inflammatories, and antiplatelet therapies, surgical revascularization and endovascular approac

In this report, we present the fatal spontaneous delayed rupture of a previously unruptured large PICA aneurysm following treatment with the PED. Pathology at postmortem examination has supported the theory that intra-aneurysmal thrombus may acutely destabilize the aneurysm wall. Aneurysms with an anatomic arrangement that promote continued flow into the neck may not be optimal candidates for the flow-diversion treatment strategy.

Cervical internal carotid artery (ICA) occlusions causing acute ischemic stroke have extremely poor outcomes without rapid re-establishment of cerebral perfusion. Given the high risk for early recurrent stroke, carotid stenting at the time of intra-arterial therapy may be crucial to improving outcomes. To our knowledge, there are only two prior case series that address intra-arterial reperfusion therapy and stent placement within six hours of stroke onset (the hyperacute period). We present two cases to further support the feasibility of this intervention in the hyperacute period, and report t

Moyamoya disease (MMD) is a progressive, occlusive disease of the distal internal carotid arteries associated with secondary stenosis of the circle of Willis. Symptoms include ischemic infarcts in children and hemorrhages in adults. Bypass of the stenotic vessel(s) is the primary surgical treatment modality for MMD. Superficial temporal artery-to-middle cerebral artery bypass is the most common direct bypass method. Indirect techniques rely on the approximation of vascularized tissue to the cerebral cortex to promote neoangiogenesis. This tissue may be in the form of muscle, pericranium, dura,

A total of 13 cases of Moyamoya disease were verified by internal and external carotid artery angiography. The biopsy samples were taken from the superficial temporal arteries and middle meningeal arteries in 12 and six cases, respectively. Angiographical and histological studies revealed that both of the internal and external carotid system could be involved in Moyamoya disease. Our studies demonstrated that Moyamoya disease was a type of vascular disease not only confined to the cerebral vessels. We recommend angiography of external carotid system and extracranial segment of internal carotid

Moyamoya disease is an uncommon cerebrovascular disease characterized angiographically by progressive stenosis of the distal internal carotid arteries and proximal anterior and middle cerebral arteries coupled with the development of a fine network of vessels at the base of the brain. In children, the disease causes recurrent hemisphere ischemia. A newly described operative procedure (encephalo-duro-arterio-synangiosis) was recently recommended by Matsushima and Inaba [Child's Brain 11: 155-170, 1984] and was used in a recent patient treated at the UCLA Medical Center. The procedure did not su

The champagne bottle neck (CBN) sign refers to a reduction in the diameter of the proximal portion of the internal carotid artery that resembles a CBN, and is a characteristic feature of Moyamoya disease. A 43-year-old woman with an infarction of the posterior limb of the left internal capsule was diagnosed with Moyamoya syndrome associated with Graves' disease. The CBN sign was observed bilaterally. Cerebral revascularization surgery was performed, including left-sided superficial temporal artery to middle cerebral artery anastomosis. During four years of follow-up, she maintained a euthyroid…

BACKGROUND: Cerebral hyperperfusion syndrome (CHS) is a serious complication following revascularization in moyamoya disease (MMD) patients, yet reliable predictors remain scarce. This study aims to evaluate subcortical hemodynamics intraoperatively using a novel ultrasound imaging technique, synchronous arbitrary gate spectral Doppler (SAGSD), and to identify indicators associated with CHS.
METHODS: A total of thirty adult MMD patients undergoing revascularization were included. Intraoperative SAGSD imaging, conventional Doppler ultrasound, and indocyanine green videoangiography (ICG-VA) wer

BACKGROUND: Changes in levels of hemoglobin would result in alterations of cerebral blood flow (CBF). However, the impact of hemoglobin on CBF in moyamoya disease (MMD) remains largely unknown. This study sought to determine whether CBF would be influenced by hemoglobin before surgical revascularization and to analyze the relationships between hemoglobin and CBF with clinical outcome after surgery in patients with MMD.
METHODS AND RESULTS: We prospectively enrolled adult patients with MMD undergoing surgical revascularization between June 2020 and December 2022. Preoperative CBF was measured
