Fibrodysplasia ossificans progressiva presenting as osteomyelitis-like syndrome.

An 8-year-old boy presenting with fever, neck pain, and torticollis was eventually diagnosed as having fibrodysplasia ossificans progressiva (FOP). The initial bone scan was interpreted as being suggestive of vertebral osteomyelitis. Subsequent computerized tomographic studies (CT scan), however, demonstrated paravertebral soft-tissue calcification adjacent to, but not involving, intact cervical vertebrae. The acute process of FOP was thereby distinguished from that of osteomyelitis by use of CT scan.

[Neck injuries In children].

Cervical spine injuries are often overlooked in young children with multiple injuries while diagnostic accuracy is reduced by difficulties in clinical and radiological investigation. Misinterpretation of normal radiographic appearances of vertebrae at different stages of maturity may lead to diagnostic confusion and inappropriate treatment. Two case histories are presented to illustrate these difficulties and guidelines are suggested to help establish the nature and extent of such injuries in young children.

[Multiple erosive discopathies associated with ankylosing spondylitis (author's transl)].

The authors report a case of ankylosing spondylitis affecting a woman with low-back pains. Although initial X-rays did not show any lesions of the spine, one year later, radiographs prove the existence of multiple lesions mimicking spondylodiscitides and characterized by erosions diffusely involving end-plates of cervical, dorsal and lumbar vertebrae. Surgical biopsy at the lumbar site provides an histological evidence of non specific inflammatory lesions.

Neuroblastoma in the dog.

Two malignant canine tumours of peripheral neuronal tissue are described. A mediastinal neuroblastoma was found in a 15-mth-old Boston Terrier with dyspnoea. The tumour compressed adjacent lung and invaded the cervical spinal cord. A retroperitoneal ganglioneuroma was found in a 2-yr-old Labrador with posterior ataxia and atrophy of the muscles of the hind legs. This tumour surrounded adjacent adjacent soft tissues and invaded the overlying vertebrae.

Pyogenic vertebral osteomyelitis in heroin addicts.

The diagnosis of pyogenic vertebral osteomyelitis was made in seven narcotic addicts between 1967 and 1972. Vertebrae involved were either cervical or lumbar. Bacteriologic diagnosis was made in each case by percutaneous needle biopsy and aspiration. Staphylococcus aureus was cultured in two patients. Five patients had infections due to Gram-negative bacteria, including Klebsiella pneumoniae, Pseudomonas aeruginosa and Enterobacter. All patients were cured by treatment with antibiotics and immobilization.

Current insights and controversies in the pathogenesis and diagnosis of disc-associated cervical spondylomyelopathy in dogs.

Disc-associated cervical spondylomyelopathy (DA-CSM) is the most common cause of cervical spondylomyelopathy in dogs. In this condition, progressive caudal cervical spinal cord compression is typically caused by protrusion of one or more intervertebral discs. This disc-associated compression is sometimes seen in combination with mild vertebral abnormalities and dorsal compression resulting from ligamentum flavum hypertrophy. The intervertebral disc space between the sixth (C6) and seventh (C7) cervical vertebrae is most commonly affected. Although several large breed dogs can be affected, the …

Posttraumatic anterior cervical osteophyte and dysphagia: surgical report and literature review.

Degenerative changes in the cervical spine can produce osteophytes and other hypertrophic abnormalities. Asymptomatic osteophytes of the anterior margins of the cervical vertebrae may occur in 20-30% of the population. Occasionally, dysphagia or dysphonia may be caused by such cervical osteophytes pressing against the esophagus or trachea. Recently, the authors treated a patient with posttraumatic dysphagia and dysphonia secondary to osteophytic spurring of the anterior cervical spine. This 43-year-old man presented 2 years after sustaining a flexion/extension soft tissue injury to his cervica…

Anatomic study of the musculus longus capitis flap.

PURPOSE: To clearly delineate the anatomy of the musculus longus capitis, determine its clinical applications for reconstruction surgery, and provide a safer surgical method of developing the longus capitis muscle flap. METHODS: Anatomical investigations were performed in seven adult cadavers (five cadavers for gross anatomy and two for transparent specimen preparation) with respect to the location, morphology, arterial supply, and innervation of the musculus longus capitis, as well as its spatial relationship with the cervical sympathetic trunk, superior cervical ganglion, carotid sheath, an

Anatomical transformation in mammals: developmental origin of aberrant cervical anatomy in tree sloths.

Mammalian cervical count has been fixed at seven for more than 200 million years. The rare exceptions to this evolutionary constraint have intrigued anatomists since the time of Cuvier, but the developmental processes that generate them are unknown. Here we evaluate competing hypotheses for the evolutionary origin of cervical variants in Bradypus and Choloepus, tree sloths that have broken the seven cervical vertebrae barrier independently and in opposite directions. Transitional and mediolaterally disjunct anatomy characterizes the cervicothoracic vertebral boundary in each genus, although po…

[Cervically-induced symptoms of the Klippel-Feil syndrome].

Klippel-Feil syndrome is a congenital deformity disease caused by disturbances of the growing together of the mesenchymal preliminary vertebrae which are mostly located in the regions of the cervical spine and shoulder. Short neck and wryneck (torticollis), as well as limited motion of the neck and reduced mobility of the cervical spine, are the most striking findings on clinical examination. Vertigo, disturbed sound conduction and perception, as well as combined loss of hearing, tinnitus and paralysis of the facial nerve can occur although otoscopic findings may be normal. Differential diagno

Atlanto-occipital Assimilation with Basilar Invagination and Atlanto-occipital Dislocation Treated by Occipitocervical Fixation: A Rare Case Report.

INTRODUCTION: Atlanto-occipital assimilation is the most common osseous anomaly of the cervical-occipital junction. The incidence rate of occipitalization has been reported 1.42.5/1000 children. Most of the patients are asymptomatic initially. Clinical features usually appear after 3rd decade depending on the level and degree of spinal cord compression with surrounding vital structures such as vertebral artery involvement. Hence, early definitive diagnosis and subsequent successful treatment are necessary in such cases to prevent life-threatening complications. CASE REPORT: A 16-year-old male

The Effect of Posterior Fossa Decompression Surgery on Brainstem and Cervical Spinal Cord Dimensions in Adults with Chiari Malformation Type 1.

OBJECTIVE: Posterior fossa decompression (PFD) surgery creates more space at the skull base, reduces the resistance to the cerebrospinal fluid motion, and alters craniocervical biomechanics. In this paper, we retrospectively examined the changes in neural tissue dimensions following PFD surgery on Chiari malformation type 1 adults. METHODS: Measurements were performed on T2-weighted brain magnetic resonance images acquired before and 4 months after surgery. Measurements were conducted for neural tissue volume and spinal cord/brainstem width at 4 different locations; 2 width measurements were

Association between the development of the body axis and the craniofacial skeleton studied by immunohistochemical analyses using collagen II, Pax9, Pax1, and Noggin antibodies.

STUDY DESIGN: Immunohistochemical analyses on the axial skeleton from wild type mice. OBJECTIVE: In the clinic, we have previously observed cervical spine defects associated with deviations in the posterior part of the occipital bone and with morphologic and functional variations in the craniofacial skeleton. As examples, cervical spine fusions occurred frequently in patients with mandibular overjet and even more frequently and more caudally in the cervical spine in patients with sleep apnoea. The aims of the present study were to elucidate this association between the spine and the cranium b

A non-invasive technique for measurement of cervical vertebral angle: report of a preliminary study.

Non-invasive methods have traditionally been used to assess spine positioning and range of motion. Recently, the use of prediction models derived from external stick markers and videographic analysis has been shown to be effective at predicting lumbosacral and segmental lumbar vertebral angles. The objective of this study was to develop a similar non-invasive method to predict cervical vertebral inclination in forward head flexion. Fourteen subjects with no history of trauma or inflammatory or arthritic disorders (mean age: 25+/-1 years) participated in this study on a voluntary basis. Radiogr

Painful dysphagia due to fracture of the styloid process.

Pain in the neck region following trauma to the mandible can be caused by styloid process fracture. This is usually the result of pressure on the glossopharyngeal nerve as it crosses the gap between this process and the first cervical vertebra. The following paper discusses two case reports of patients who developed painful dysphagia as a result of this type of trauma.

[Lateral medullary stroke with extracranial course of the postero- inferior cerebellar artery].

We present two cases of lateral medullary stroke in subjects with extracranial trajectory of the postero-inferior cerebellar artery. Case 1: a 21-year-old male who presented ataxia and right dysmetria after cervical trauma in a rugby match. Case 2: 56-year-old woman, who started with vertigo and left hemiparesis after intense physical effort. In both cases, the angiographic studies showed an extracranial trajectory of the posterior inferior cerebellar artery. This vessel rarely originates below the foramen magnum, in close relationship with the first three cervical vertebrae and the atlanto-ax…

Compressive myelopathy associated with ectasia of the vertebral and spinal arteries in a dog.

A 4-year-old dog was presented for acute, progressive tetraparesis and cervical hyperesthesia. Symmetrical tubular structures coursing along the lateroventral aspects of the spinal cord at the fourth and fifth cervical vertebrae were identified in magnetic resonance images. At necropsy, vertebral arteries and their spinal branches were severely ectatic bilaterally, and the cervical spinal cord was compressed. Histologically, the ectatic branches of the vertebral and ventral spinal arteries were surrounded by fibrosis with scant mononuclear cell infiltrates and hemorrhage. Spinal branches of th…
