Direct and indirect effects of contextual factors, caregiver depression, and parenting on attachment security in preschoolers.

The aim of this study was to develop a multiple-level-of-analysis model of preschool attachment security and to determine the processes (direct and indirect) whereby factors from different domains (e.g., stress and parenting) are related to attachment during this period. This study examined the direct and indirect effects of stress, family conflict, caregiver depression symptoms, and parenting on attachment security in a large (N = 796) and diverse sample of 4-year-olds. This study used the 3-Boxes Task to assess aspects of parenting critical to sensitivity in the preschool period, labeling th

Do underrepresented minority medical students differ from non-minority students in problem-solving ability?

BACKGROUND: In medical education, examinations must assess a logical progression toward problem-solving skills. Differences in cognitive development between underrepresented minority students (URMs) and non-URMs may affect examination performance and subsequent attrition rates. PURPOSE: The authors investigated URM and non-URM performances by retrospectively analyzing success rates on exam items of differing cognitive demand. METHOD: Mean correct responses to exam items classified as Recall, Interpretation, or Problem-Solving questions were calculated. Both URM and non-URM groups were strati

The outreach-assisted model of partner notification with IDUs.

OBJECTIVE: This analysis describes the Outreach-Assisted Model of Partner Notification, an innovative strategy for encouraging seropositive injecting drug users (IDUs) to inform their partners of shared human immunodeficiency virus (HIV) exposure. The analysis focuses on two core components of the notification process: the identification of at-risk partners and preferences for self-tell vs. outreach assistance in informing partners of possible exposure to the virus. METHODS: Using community outreach techniques, 386 IDUs were recruited for HIV pretest counseling, testing, and partner notificat

Toxicology screening in adolescent trauma.

We sought to define the prevalence of positive drug screens in adolescent victims of major trauma. The records of 125 consecutive adolescent patients presenting with major trauma to an inner-city trauma center during the last nine months of 1990 were reviewed. Eighty-five (68%) received urine toxicology screens for alcohol and illicit drugs. Twenty-one (25%) of screened patients had a positive urine drug screen. The most commonly detected drugs were alcohol, cocaine, and opiates. Gender, race, mechanism of injury, mental status at presentation, injury severity score, and revised trauma score w

A model for the prospective analysis of perinatal deaths in a perinatal network.

This prospective study assesses factors that contribute to perinatal mortality. The study population includes the 1362 perinatal deaths that occurred among 85,402 live births between 1983 and 1987 at hospitals of the University of Chicago Perinatal Network. After peer review of demographic, clinical, and pathologic data, each perinatal death was classified in one of the following categories: (1) the result of congenital malformation incompatible with life, (2) unavoidable, (3) potentially avoidable by patient, by health provider, or by both, or (4) of undetermined responsibility. Of 1362 death…

Artists Can Design Memory, and AI Can Predict It.

Can artists deliberately design artworks to be remembered-or forgotten? Across three experiments, we tested whether visual-memory outcomes can be shaped through artistic design and how they are predicted by artists, laypeople, and artificial intelligence. We launched a nationwide art contest in which artists submitted works intended to be memorable or forgettable. Using recognition tasks with adults based in the United States (119 from Amazon Mechanical Turk, 892 from Prolific, and 62 visitors from a Chicago gallery), we found that artists achieved their intended memory outcomes. However, thei…

The role of polygenic risk in Alzheimer's disease prediction for African Americans.

INTRODUCTION: African Americans (AAs) face a higher risk of Alzheimer's disease and related dementias (ADRD) than European Americans (EUs), yet the utility of polygenic risk scores (PRS) in AAs remains underexplored. METHODS: A standardized dementia PRS was evaluated in the Chicago Health and Aging Project (n = 4336; 61% AA) for associations with ADRD and cognitive trajectories over 8.4 years. RESULTS: PRS predicted ADRD more strongly in EUs (c-index 0.86) than AAs (0.77); however, it conferred higher risk in AAs (hazard ratio [HR] = 1.36, 95% confidence interval [CI]: 1.04-1.78) compared to…

Fate of isoprene peroxy radical constrains the urban photochemical regime.

Declining nitrogen oxide (NOx = NO + NO2) emissions have transformed oxidation pathways in urban atmospheres, with implications for air qual … mes (τbi) as a proxy for isomerization potential, finding longer τbi (17 ± 11 seconds) in New York, Chicago, and Toronto compared to Los Angeles (7 ± 6 seconds). Satellite measurements reveal that long τbi is widespread across urban North America, suggesting that declining NOx is likely to lead to greater HOM formation in urban regions. These findings ind …

Vanishing Aortic Valve Cusp: A Rare Case of Aortic Valve Insufficiency in a Durable Left Ventricular Assist Device Recipient After Micro-Axial Flow Pump Support.

De novo aortic insufficiency (AI) is a recognized complication in patients supported with continuous-flow durable left ventricular assist devices (LVADs), carrying important clinical consequences including heart failure progression and increased device-related morbidity. We report the case of a HeartMate 3 (Abbott Inc., Chicago, Illinois, USA) patient, previously supported with a micro-axial flow pump (mAFP), who developed severe AI within the first year of LVAD support and ultimately underwent urgent heart transplantation. Intraoperative inspection and histopathological analysis revealed mech…

Future perspectives in esophageal manometry.

Esophageal manometry has undergone significant advancements, transitioning from conventional line tracings to high-resolution manometry with topographic analysis. This evolution has improved the classification and diagnosis of esophageal motility disorders, as defined by the Chicago Classification. However, challenges remain in interpreting borderline cases, assessing esophagogastric junction outflow obstruction, and correlating manometric findings with clinical symptoms. Artificial intelligence (AI) has emerged as a promising tool for enhancing esophageal manometry by enabling automated data …

CardioMEMS Heart Failure System: An Up-to-Date Review.

Heart failure (HF) continues to represent a significant public health concern. While patients with HF with reduced ejection fraction (HFrEF) face a high risk of arrhythmias, both patients with HFrEF and HF with preserved ejection fraction (HFpEF) experience fluid overload, leading to repeated hospitalizations. Traditional monitoring methods have limited ability to manage HF exacerbations preemptively. However, the CardioMEMS™ (Abbott Laboratories, Chicago, IL) HF system, an implantable microelectromechanical sensor, has emerged as an innovative solution, enabling real-time remote monitoring of…

Lessons learned from physician-performed high-resolution esophageal manometries.

High-resolution esophageal manometry (HRM) is the gold standard test for evaluation and diagnosis of esophageal motility disorders. While the Chicago Classification offers a standardized protocol for performing and interpreting HRM studies, it does not provide guidance on catheter placement techniques, nor the specific skillset required to conduct the test. At most centers nationally and globally, HRMs are performed by a trained nurse or medical technician. However, in selected centers, physicians perform HRMs alongside a clinical care assistant. Direct physician involvement in performing HRM …
