Late-Onset Dilated Cardiomyopathy in Auto Immune-Mediated Complete Congenital Heart Block: A Case Report.

Complete congenital heart block (CHB), a rare and fatal bradyarrhythmia observed in children, carries significant mortality and morbidity. When congenital heart block occurs in isolation with a structurally normal heart, it prompts suspicion of an autoimmune etiology, wherein maternal antibodies are transmitted transplacentally, impacting the fetal conducting system. The manifestation of congenital complete atrioventricular block (CCAVB) can lead to complications such as dilated cardiomyopathies, arrhythmias, and fibroelastosis in certain cases. Notably, dilated cardiomyopathy is a significant

Dissemination of child abuse clinical decision support: Moving beyond a single electronic health record.

BACKGROUND: Child maltreatment is a leading cause of pediatric morbidity and mortality. We previously reported on development and implementation of a child abuse clinical decision support system (CA-CDSS) in the Cerner electronic health record (EHR). Our objective was to develop a CA-CDSS in two different EHRs. METHODS: Using the CA-CDSS in Cerner as a template, CA-CDSSs were developed for use in four hospitals in the Northwell Health system who use Allscripts and two hospitals in the University of Wisconsin health system who use Epic. Each system had a combination of triggers, alerts and chi

Antiretroviral drugs in the cupboard are not enough: the impact of health systems' performance on mother-to-child transmission of HIV.

OBJECTIVE: To model the effect of health systems performance on rates of mother-to-child HIV transmission. METHODS: We modeled the effect of variation in performance of the multiple steps of different prevention of mother-to-child transmission (PMTCT) protocols using hypothetical and reported data. SETTING: Data from a PMTCT program in a large province in South Africa was used to compare model predictions with reported outcomes for mother-to-child HIV transmission. MAIN OUTCOME MEASURE: Perinatal HIV transmission was predicted for infants of 6 weeks of age. RESULTS: HIV-infected pregnant w

Body wall thickness in adults and children--relevance to penetrating trauma.

AIM: Although uncommon in children, penetrating trauma may be associated with considerable risk of morbidity and mortality. In part this reflects the child's thinner body wall. No data exists on the difference in body wall thickness in children as compared to adults. METHOD: Computer tomography (CT) scans (23 adults and 17 children) selected at random were reviewed. Measurements of the thickness of the skin and subcutaneous tissue were taken from anatomically defined regions on the thoracic and abdominal wall. The differences between adults and children were compared and analysed statisticall

Thoracic trauma in children.

Thoracic trauma in children is an infrequent but potentially lethal injury often associated with coexisting significant injury to other systems. Most reports suggest that the incidence of chest trauma in the injured child is approximately 10%. The most frequently associated trauma is, unfortunately, to the central nervous system and is an unusually deadly combination of injuries. As such, the thoracic injuries serve as a reliable marker for injury severity in children, mandating meticulous evaluation of the child presenting with thoracic trauma. The diagnosis of the thoracic injury may be diff

The role of public health improvements in health advances: the twentieth-century United States.

Mortality rates in the United States fell more rapidly during the late nineteenth and early twentieth centuries than in any other period in … ion in major cities, three quarters of the infant mortality reduction, and nearly two thirds of the child mortality reduction. Rough calculations suggest that the social rate of return to these technologies was greater than 23 to 1, with a cost per person-year saved by clean water of about dollar 500 in 2003 dollars. Implications for developing countries are brie …

Effects of household concrete floors on maternal and child health - the CRADLE trial: a randomised controlled trial protocol.

INTRODUCTION: Early life soil-transmitted helminth infection and diarrhea are associated with growth faltering, anemia, impaired child development, and mortality. Exposure to fecally contaminated soil inside the home may be a key contributor to enteric infections, and a large fraction of rural homes in low-income countries have soil floors. The objective of this study is to measure the effect of installing concrete floors in homes with soil floors on child soil-transmitted helminth infection and other maternal and child health outcomes in rural Bangladesh. METHODS AND ANALYSIS: The Cement-bas

International Consensus Statement on the Radiological Screening of Contact Children in the Context of Suspected Child Physical Abuse.

IMPORTANCE: Physical abuse is a common but preventable cause of long-term childhood morbidity and mortality. Despite the strong association between abuse in an index child and abuse in contact children, there is no guidance outlining how to screen the latter, significantly more vulnerable group, for abusive injuries. Consequently, the radiological assessment of contact children is often omitted, or variably performed, allowing occult injuries to go undetected and increasing the risk of further abuse. OBJECTIVE: To report an evidence-based and consensus-derived set of best practices for the ra

Sexual homicides in South Africa: A national cross-sectional epidemiological study of adult women and children.

METHODS: We conducted a retrospective national mortuary based study to identify all adult female homicides (18 years and older) and all child homicides (boys and girls < 18 years) in 2009 in a randomly selected, proportionate sample of mortuaries. Victim, perpetrator and crime data were collected in three processes: from the mortuary register, the autopsy report and from police with the identification of sexual homicides validated across the data collection processes. FINDINGS: Among the 2670 (95% CI: 2311-2979) adult women killed in 2009, 494 (95% CI: 406-574) were identified as sexual homic

Caring for Children with Non-Accidental Head Injuries: A Case for a Child-Centered Approach.

Child abuse remains a significant issue. Non-accidental head injury (NAHI) is a major cause of mortality in young children with survivors often having to live a life with severe developmental and neurological dysfunction. The aim of this hermeneutic phenomenological research study was to examine the lived experiences of nurses who care for children and their families admitted to hospital with a non-accidental head injury. Semi-structured interviews were conducted with six nurses who had at least five years' experience of nursing children with NAHI. Following the interpretive approach described

Pediatric facial burns.

Despite major advances in the area of burn management, burn injury continues to be a leading cause of pediatric mortality and morbidity. Facial burns in particular are devastating to the affected child and result in numerous physical and psychosocial sequelae. Although many of the principles of adult burn management can be applied to a pediatric patient with facial burns, the surgeon must be cognizant of several important differences. Facial burns and subsequent scar formation can drastically affect the growth potential of a child's face. Structures such as the nose and teeth may become deform

Fifteen-minute consultation: Drowning in children.

Drowning is a significant cause of childhood morbidity and mortality globally. The underlying mechanisms vary with child development and most are modifiable to public health promotion strategies. This article serves to highlight some of the specific considerations for the clinical management of drowning in children, both prehospital and by the in-hospital paediatric resuscitation team. This includes changes to standard advanced paediatric life support in the presence of hypothermia.

[Congenital syphilis: need for adequate antenatal care].

Congenital syphilis is well-known and treatable with penicillin. Diagnosis in the neonate and young child may be difficult and consequently morbidity and mortality can be high. Prevention in children is of utmost importance and can be achieved by proper antenatal care and adequate follow-up of pregnant women. This includes identification of pregnant women at risk for contracting syphilis. The case presented demonstrates this need.

Prognostic significance of lymphocyte PD-1 expression in combination with clinical scoring systems in patients with liver cirrhosis complicated by sepsis.

BACKGROUND: Liver cirrhosis complicated by sepsis is associated with significant immune dysfunction and high mortality rates. Programmed cell death protein 1 (PD-1) expression on lymphocytes has been implicated in immune exhaustion, but its prognostic significance in this population remains unclear. OBJECTIVE: This study aimed to evaluate the prognostic value of lymphocyte PD-1 expression in combination with clinical scoring systems, such as the Child-Pugh score and chronic liver failure-sequential organ failure assessment (CLIF-SOFA), for predicting disease severity and 28-day mortality in p

Baseline characteristics associated with mortality among children living with HIV initiating ART at a rural district HIV clinic of Mozambique.

This study describes the incidence and factors associated with mortality in children living with HIV and initiating ART at rural settings. We analyzed routinely collected data from electronic medical records of children, aged under 15 years or younger, living with HIV (CLHIV) who were routinely managed at Carmelo Hospital of Chokwe, from 2002 to 2019, located in the Chokwe district, Gaza province, Mozambique. Kaplan-Meier survival curves and Cox regression analyzes were used to model the time to death and predictors of mortality respectively. Overall, 1341 HIV-infected children on ART contribu

Disturbances in sodium and chloride homeostasis predict outcome in stable and critically ill patients with cirrhosis.

BACKGROUND: Hyponatremia has prognostic implications in patients with cirrhosis, and thus, has been incorporated in the 2016 MELD-UNOS update. Changes in serum chloride are commonly perceived as 'just' parallel to changes in serum sodium. However, these are less well studied in the context of cirrhosis. AIMS: To investigate whether serum chloride independently predicts outcomes in patients with advanced chronic liver disease (ACLD) and stable clinical course or with critical illness. METHODS: 891 patients with ACLD (defined by hepatic venous pressure gradient [HVPG] ≥6 mm Hg) were followed a
