Severe congenital nemaline myopathy with primary pulmonary lymphangiectasia: unusual clinical presentation and review of the literature.

INTRODUCTION: Nemaline myopathy is a rare genetic muscle disorder defined by the presence of nemaline rods in the muscle fibre sarcoplasm. Congenital nemaline myopathy is the most serious form of the disease's spectrum. CASE PRESENTATION: The affected newborn has no spontaneous movement, fractures at birth and respiratory insufficiency. The present case was a Thai male, floppy at birth with fractures of both humeri and femurs and ventilator-dependent respiration. The patient developed bilateral chylothorax two weeks later and died at the age of 6 weeks. Whole-body postmortem examination with …

Magnetic resonance imaging of the fetus in congenital intrathoracic disorders: preliminary observations.

BACKGROUND AND OBJECTIVE: Advances in magnetic resonance imaging (MRI) provide high-quality images of the intrathoracic organs. We studied the ability of MRI to define spatial relationships of the fetal lungs and measured lung volume in two cases of congenital diaphragmatic hernia (CDH), one of severe oligohydramnios secondary to bilateral cystic renal dysplasia and one case of prenatal chylothorax. PATIENTS AND METHODS: We performed pelvic MRI using single-shot fast spin echo (SSFSE) pulse sequence in four pregnant women referred because of abnormal prenatal ultrasound (US) findings associat…

[Feto-placental non-immunological anasarca].

Two clinical cases of fetal hydrops are discussed: one of them was caused by diffuse lymphangiectasis and the other was idiopathic. A review of recent literature regarding the management of non-immunological hydrops is presented and gives hope for a new therapeutic approach to fetal hydrops especially in cases of chylothorax. However the mortality remains extremely high because of lung hypoplasia.

VERDEDT 1.0: five years single-center experience with real-time indocyanine green fluorescence imaging of the thoracic duct during minimally invasive esophagectomy.

One method to improve visualization of the thoracic duct (TD) during surgery is to improve its identification through the application of ind … agectomy was feasible. No adverse reactions to the ICG injection occurred. We had one postoperative chylothorax. NIR-ICG imaging during minimally invasive esophagectomy is highly effective in identifying the thoracic duct. Given the number of cases and the visualization rate, we conclude that the procedure could be used in esophageal surgery and could signifi …

Intrathoracic lymphangioma.

Of 14 cases of intrathoracic lymphangioma, 4 occurred in the anterior, 5 in the superior, and 4 in the posterior mediastinum. One lesion was … ncluded bone lesions and subdiaphragmatic extension. Both of these latter cases were complicated by chylothorax after attempted removal of the mediastinal lymphangioma. Computed tomography and lymphangiography were helpful in determining the extent of the disease and the cystic and lymphatic nature of the mass. The patient with Gorham's disease had extensive …

Gorham-Stout disease with thoracic involvement: pathogenic mechanisms, respiratory complications, and multimodal therapies.

BACKGROUND: Gorham-Stout disease (GSD), also known as vanishing bone disease, is a rare osteolytic disorder characterized by progressive bone resorption and proliferation of lymphatic and vascular channels. Thoracic involvement often leads to life-threatening respiratory complications, yet clinical recognition remains delayed due to its rarity and heterogeneity. OBJECTIVE: This review aims to synthesize recent advances in the understanding of GSD pathogenesis, highlight the disproportionate burden of respiratory complications-particularly chylothorax-and propose an integrated diagnostic and th…

A case of sarcoidosis with pleural involvement.

Sarcoidosis is a chronic, multisystem inflammatory disorder of unknown etiology that is characterized by noncaseating granulomas. Although lung involvement is common in sarcoidosis, pleural involvement is rare. Pleural involvement may manifest as a pleural effusion, pneumothorax, pleural thickening and nodules, hydropneumothorax, hemothorax, or chylothorax. Here, we describe a case of sarcoidosis with pleural nodular thickening.

Concurrent cranial mediastinal Blastomyces granuloma and carcinoma with cranial vena caval syndrome in a dog.

This report describes an unusual progression of blastomycosis in a dog with concurrent mediastinal carcinoma. The dog was evaluated for respiratory distress. Diagnostic results revealed chylothorax and a cranial vena caval thrombus. Histopathology of the cranial mediastinal mass diagnosed mediastinal carcinoma and fungal granuloma. Intercurrent disease may complicate the clinical presentation and clinical course of blastomycosis.

Thoraco-abdominal birth injury--presentation, diagnosis and management in an unusual case.

As a result of a traumatic delivery a newborn male child developed thoracic and intra-abdominal problems. Haemoperitoneum and chylous ascites were diagnosed; from a scrotal enlargement secondary to bilateral patent processus vaginalis and a chylothorax became apparent during investigation of the scrotal swellings. We discuss the presentation, diagnosis and management of the case and review the literature.

Complications of retroperitoneal lymphadenectomy for nonseminomatous tumors of testis.

We present 4 patients with serious complications after retroperitoneal lymphadenectomy. Chylothorax developed in the first patient; hypertension, blindness, and paralysis of the lower extremities in the second; volvulus and necrosis around an adhesion in the right upper quadrant in the third; and small-bowel obstruction and radiation myelitis in the fourth patient. These complications and modes of treatment and prevention are discussed.

A rare case report of chylopericardium after esophageal cancer surgery: Diagnostic challenges and successful surgical management.

INTRODUCTION: Chylothorax is a rare but serious complication following thoracic surgery, characterized by the abnormal accumulation of chylous fluid within the pleural cavity or other intrathoracic spaces. Owing to its low incidence, chylothorax is often underrecognized by clinicians. Among its various forms, chylopericardium is even rarer and poses greater clinical challenges. PRESENTATION OF CASE: The patient was a 55-year-old woman who developed acute pericardial tamponade on postoperative day 7, presenting with sudden loss of consciousness and cardiorespiratory arrest. Pericardiocentesis …

[Emergency thoraco amniotic shunting in cases with compressive pleural effusion with hydrops: A retrospective study of 60 cases].

OBJECTIVES: To study perinatal outcome following thoraco-amniotic shunting for fetal pleural effusions with hydrops. MATERIALS AND METHODS: Retrospective study (1984-2004) to evaluate a policy of emergency thracoamniotic shunting in hydropic fetuses with suspected chylothorax, on the basis of the rationale that mediastinal compression could lead to acute fetal distress. RESULTS: Shunting was performed immediately following diagnosis, and was successful in all 60 cases attempted. There were 7 pregnancy terminations, 10 in utero deaths, and 43 live births, of which 7 children died in the neona…

Aberrent Thoracic Duct Cyst in Postrior Mediastinum.

Thoracic duct cysts in the upper portion of the diaphragm are mostly found in the neck and are rarely found in the mediastinum. Thoracic duct cysts should be differentiated from other mediastinal tumors or cysts, and surgical treatment is required to avoid the development of chylothorax if the cyst ruptures. Herein, we report the case of a patient with a thoracic cyst located just above the diaphragm that was treated with surgical resection.

Bilateral agenesis of the superior vena cava associated with congenital hydrothorax.

Agenesis of the superior vena cava is a rare anomaly that is generally asymptomatic in the neonate. We report a male neonate with bilateral (total) agenesis of the superior vena cava with obstructed thoracic duct and subsequent congenital hydrothorax, anomalies that were detected by prenatal ultrasound at 25 weeks' gestation. The cardiac anomaly was confirmed by postnatal magnetic resonance angiography. The chylothorax disappeared with conservative therapy.

Off-pump Kawashima operation.

We describe the technique of Kawashima repair without using cardiopulmonary bypass in 6 consecutive patients of single ventricle morphology with interrupted inferior vena cava and pulmonary stenosis. No patient had central nervous system disorder or chylothorax. The off-pump technique is feasible in Kawashima repair. This technique avoids use of cardiopulmonary bypass, thereby preventing its deleterious effects, which is also economical.
