MDCT detection of mitral valve calcification: prevalence and clinical relevance compared with echocardiography.

OBJECTIVE: The purpose of this study was to analyze the prevalence and clinical significance of mitral valve calcification incidentally detected on chest CT scans in comparison with echocardiography. MATERIALS AND METHODS: The data of 390 patients (227 men and 163 women; mean age, 62.4 +/- 12.2 years) who underwent MDCT of the chest and echocardiography were retrospectively evaluated. On MDCT, mitral valve leaflet and annulus calcification were visually graded on a scale of 0-3, with grade 0 denoting no calcification and grade 3 indicating severe calcification. CT findings were correlated wit

Clinical relevance of musical murmurs in color-coded carotid and transcranial duplex sonographies.

BACKGROUND AND PURPOSE: Musical murmurs (MMs), sometimes called seagull cry, goose cry, honks, or cooing murmur, are murmurs with a single frequency that sounds like a musical tone. Doppler detections usually show mirror-image parallel strings or bands of low to moderate frequency. Musical murmurs are mostly described in cardiac murmurs and have seldom been mentioned in cerebrovascular disease. METHODS: A retrospective review of 12,000 patients from our neurosonographic data base of the past 7 years was conducted to find patients who had MMs during color-coded carotid and transcranial duplex

Clinical relevance of Aspergillus isolation from respiratory tract samples in critically ill patients.

INTRODUCTION: The diagnosis of invasive pulmonary aspergillosis, according to the criteria as defined by the European Organisation for the Research and Treatment of Cancer/Mycoses Study Group (EORTC/MSG), is difficult to establish in critically ill patients. The aim of this study is to address the clinical significance of isolation of Aspergillus spp. from lower respiratory tract samples in critically ill patients on the basis of medical and radiological files using an adapted diagnostic algorithm to discriminate proven and probable invasive pulmonary aspergillosis from Aspergillus colonisatio

Clinical relevance of homocysteine levels in patients receiving coronary stenting for unstable angina.

BACKGROUND: We prospectively investigated whether plasma homocysteine (HCY) concentrations are related to target lesion revascularization (TLR) rates in patients with unstable angina undergoing stenting. METHODS: We enrolled 196 consecutive patients with at least one successful coronary stent implantation for unstable angina. RESULTS: The mean vessel diameter was 3.1 +/- 0.5 mm. At follow-up (17.8 +/- 7.5 months), patients with higher HCY levels (> 17 micromol/l, 4th quartile) had similar TLR rates to the rest of the sample (11.1 vs 13.2%, p = 0.90). On the other hand, high HCY levels did se

Clinical relevance of three subtypes of primary sinonasal lymphoma characterized by immunophenotypic analysis.

BACKGROUND: The purpose of this study was to investigate the clinical relevance of subtypes categorized by immunophenotypic analysis in primary sinonasal lymphomas. METHODS: Eighty patients with localized non-Hodgkin's lymphoma involving the nasal cavity and/or paranasal sinuses were divided into three subtypes on the basis of their immunohistochemical findings: (A) B-cell lymphoma (n = 19), (B) T-cell lymphoma (n = 27), and (C) natural killer (NK)/T-cell lymphoma (n = 34). The clinicopathologic profiles, immunophenotypic data, patterns of treatment failure, and survival data among the three …

Imatinib targets other than bcr/abl and their clinical relevance in myeloid disorders.

Imatinib mesylate is a small molecule drug that in vitro inhibits the Abelson (Abl), Arg (abl-related gene), stem cell factor receptor (Kit), and platelet-derived growth factor receptor A and B (PDGFRA and PDGFRB) tyrosine kinases. The drug has acquired therapeutic relevance because of similar inhibitory activity against certain activating mutations of these molecular targets. The archetypical disease in this regard is chronic myeloid leukemia, where abl is constitutively activated by fusion with the bcr gene (bcr/abl). Similarly, the drug has now been shown to display equally impressive thera

[Clinical relevance and treatment possibilities of rapid cycling in patients with bipolar disorder].

In the actual version of the WHO diagnostic guidelines, the ICD-10, subtypes of bipolar disorder are not specified, in contrast to the American DSM-IV, where bipolar disorder has already been differentiated in bipolar I (severe manic and depressive episodes) and bipolar II disorder (depressive and hypomanic episodes). Furthermore, aspects of the longitudinal course of the illness, like rapid cycling (RC), are reflected as well. Rapid cycling is defined as four or more affective episodes within one year of the illness. It has been postulated that rapid cycling is related with a poor response to

Relevance of CagA positivity to clinical course of Helicobacter pylori infection in children.

A potential virulence determinant of Helicobacter pylori is the cagA gene product. To determine the relevance of the expression of CagA to the clinical picture and outcome of H. pylori infection in children, we examined 104 consecutive children diagnosed with H. pylori infection. Serum samples were collected to test for the presence of immunoglobulin G (IgG) anti-CagA antibodies. Forty-five patients (43%) had antibodies to the CagA protein (group I), and 59 did not (group II). Seropositive patients had a longer prediagnostic history of abdominal pain (P = 0.02), more severe abdominal pain (def

[Sonography of the infant hip. Sources of error, progress and current clinical relevance].

Effective prevention of late dysplastic dislocation of the hip (DDH) has become a reality since reliable diagnosis has been possible at an early stage by means of the original sonographic method according to Graf. A correct sonographic diagnosis must be instantly followed by adequate, biomechanically rational, and consistent therapeutic management and long-term follow-up. The results of a strictly comparative follow-up study emphasize the enormous progress in the quality of therapeutic results gained when routine sonographic newborn hip "screening" is performed directly in the maternity wards:

Clinical relevance of PM-1 antibody and physiochemical characterization of PM-1 antigen.

Using an improved immunodiffusion test with partially purified antigen, PM-1 antibody was identified in the serum of 18 patients. In 67% this system was associated with a polymyositis-scleroderma overlap, it occurred less frequently in polymyositis, dermatomyositis and scleroderma, and was not detected in other rheumatic diseases. The predominant clinical features of PM-1 positive patients were muscle weakness, sclerodactyly, Raynaud's phenomenon and pulmonary disease; widespread sclerodermatous features with infrequent. Characterization of the PM-1 antigen showed it to be a heat sensitive, tr

Colloids versus crystalloids as volume substitutes: clinical relevance of the serum oncotic pressure.

With some amplifications, Starling's concept of the serum colloid osmotic or oncotic pressure as the determinant of fluid partition between the intravascular and the interstitial compartment has been confirmed by modern physiological research. The relationship between serum oncotic pressure and interstitial edema is non-linear, i.e. edema becomes progressively greater per mm decrease of the oncotic pressure. The intravascular volume effect of crystalloids is inseparable from interstitial edema, because it depends on an expansion of the interstitium which increases the hydrostatic pressure in t

The role of USP19 in human diseases: from molecular function to clinical relevance.

USP19 is an important member of the ubiquitin-specific protease (USP) subfamily within the deubiquitinase superfamily. It primarily regulates protein stability, subcellular localization, and signaling pathway activity by specifically removing ubiquitin modifications from substrate proteins, and it is widely involved in the regulation of cellular physiological homeostasis and various pathological processes. USP19 shows aberrant expression and functional dysregulation in multiple malignancies, participating in the regulation of tumor proliferation, metastasis, apoptosis, immune evasion, and chem

Artificial Intelligence-Advanced Imaging for Solid-Type Lung Adenocarcinoma: Towards Greater Clinical Relevance.

OBJECTIVES: Non-small-cell lung cancer (NSCLC) lesions appearing solid on imaging are highly likely to be malignant. However, "solid" has a subjective definition. This study aimed to determine whether artificial intelligence (AI)-based imaging analysis can offer a more objective and clinically meaningful definition of solid tumours, specifically in regards to lymph node metastasis and prognosis. METHODS: This study included 216 patients with cN0 lung adenocarcinoma (pathological invasive diameter ≤ 30 mm) who underwent lobectomy with lymph node dissection.
