Posterior tibial tendon entrapment within an intact ankle mortise: a case report.

The present case report demonstrates a rare finding associated with irreducible ankle fracture dislocations. To our knowledge, posterior tibial tendon entrapment with an intact ankle mortise has not yet been documented in published studies. In the case of our patient, a high-energy, 12-ft fall resulted in a comminuted intra-articular fracture of the medial malleolus, confirmed by the initial radiographs. Preoperative magnetic resonance imaging showed the Achilles tendon to be ruptured and the posterior tibial tendon to be both displaced and entrapped between the medial malleolar fracture fragm

Biomechanical Comparison of a Closed-Loop Double Endobutton to a Lag Screw in Fixation of Posterior Malleolar Fractures.

Anteroposterior (AP) lag screw, posteroanterior (PA) lag screw, or posterior buttress plate are usually performed for posterior malleolar fixation, but the biomechanically strongest technique is unclear. The aim of our study was to biomechanically compare 3 different fixation methods for posterior malleolar fractures; AP lag screw, PA lag screw, and closed-loop double endobutton. Fracture models were created using a thin blade power saw after drawing the fracture line. The resultant fracture involved 30% of the joint on the distal tibial joint surface and extends with an angulation of approxim

Derotation of the mallet piece: A crucial point in mallet fracture surgery.

Mallet fracture is an avulsion of the extensor tendon and fracture of the dorsal rim of the articular surface of distal phalanx at the same time. If a part of the mallet fracture is angled or rotated to such a degree that prevents full anatomic reduction, malunion and deformities may occur as a result. The objective of this study was to describe a new surgical technique to provide derotation of the mallet fracture. A 22G or 21G needle is used like a joystick to reduce the mallet fracture with small, gentle movements. The extension block pinning technique described by Ishiguro was applied after

Revascularisation during fracture healing with soft tissue injury.

INTRODUCTION: The purpose of our study was to quantitatively assess changes in the revascularisation process in the fracture gap and in adjacent regions during the course of healing of diaphyseal fractures with and without closed soft tissue injury. METHODS: In a rat model (fracture n = 26; fracture with closed soft tissue crush n = 26) revascularisation was assessed in a long-term study with regional mapping by laser Doppler flowmetry, the healing outcome being mechanically tested after 4 weeks. Fracture and soft tissue crush were performed by modified controlled impact device. RESULTS: No

Perfecting the Outcome of Nasal Bone Fracture in the Pediatric Population With a Navigation System.

BACKGROUND: The trend of using navigation systems for treating facial bone fractures is increasing. Nevertheless, any detailed recommendation on using a navigation system to treat nasal bone fracture has never been discussed. Plastic surgeons are prone to do closed reduction of nasal bone fractures with remnant posttraumatic edema in the pediatric population. This study hypothesized that the navigation system benefits this population's treatment of nasal bone fractures. METHODS: The authors evaluated the medical records, plain x-ray, and facial computed tomography scans of 44 pediatric patien

Monteggia fractures in pediatric and adult populations.

A Monteggia fracture is a fracture of the proximal ulna coupled with a radial head dislocation. These fractures are an uncommon class of forearm fractures. Numerous classification systems have been developed to characterize these fractures, with the Bado classification being the most common. Elbow radiographs are the primary diagnostic modality, demonstrating dislocation when a line drawn extending through the radial head from the radial shaft does not penetrate the capitellum in all views. Notable differences exist in the prevalence, treatment, and outcomes of Monteggia fractures for pediatri

Percutaneous reduction and flexible intramedullary nailing for monteggia fracture in a skeletally mature patient.

INTRODUCTION: Monteggia lesions are defined as a fracture of the proximal ulna with an associated radial head dislocation. Management of these injuries varies depending upon the patient population, ranging from non-operative treatment with closed reduction and immobilization to surgical fixation. Percutaneous techniques of radial head reduction are often reserved for skeletally immature patients. PRESENTATION OF CASE: In this case report, a 14-year-old female presented with left elbow pain three days after a fall. Radiographs and CT images from an outside hospital revealed a displaced left ra

Management of displaced supracondylar fractures of the humerus using lateral versus crossed Kirschner wires: a prospective randomized controlled clinical trial.

AIMS: We sought to evaluate whether loss of reduction in lateral Kirschner (K)-wiring is noninferior to crossed K-wiring in closed reduction and percutaneous pinning of Type III supracondylar humerus fractures. METHODS: A prospective, randomized, non-inferiority trial was conducted. Emergency department patients aged three to seven years with Type III supracondylar humerus fractures were invited to participate with informed consent. Block randomization allocated patients into one of two pin configuration groups (lateral or crossed). Surgical technique and postoperative management were standar

Introducing the Surgical Therapeutic Index in trauma surgery: an assessment tool for the benefits and risks of operative fracture treatment strategies.

BACKGROUND: The Surgical Therapeutic Index (STI) has been described as an indicator of the benefits and risks of surgical treatment. The index is calculated by dividing the cure rate of an operative treatment by the complication rate. This study introduces the STI in trauma surgery by comparing the indices for surgical plate fixation (PF) and intramedullary fixation (IMF) of displaced midshaft clavicular fractures. METHODS: In a previously reported, randomized controlled fashion, 120 patients were assigned to PF or IMF. Cure was defined by a Disabilities of the Arm, Shoulder and Hand score of

Functional Outcomes of Distal Radius Fractures According to Surgical Technique: A Retrospective Study Using the DASH Score.

Distal radius fractures are common injuries requiring tailored treatment approaches. This study explores the relationship between the DASH (Disabilities of the Arm, Shoulder, and Hand) score and various surgical techniques in patients treated for wrist fractures. A retrospective cohort analysis was conducted involving 140 patients with distal radius fractures treated between 2017 and 2022 at CHU-NDS. Data were collected through phone interviews using the DASH questionnaire and clinical records. Four surgical methods were evaluated: closed reduction, DVR (dorsal volar radial), Kapandji techniqu

Evaluation of current treatment techniques for distal radius fractures amongst Belgian orthopaedic surgeons.

This online questionnaire was initiated to investigate if a consensus on the treatment of distal radius fractures amongst orthopaedic surgeons in Belgium exist. Two cases were presented: an extra-articular fracture, with dorsal displacement (Frykman type I) and a displaced intra-articular fracture (Frykman type VII). Treatment of choice and rationale of choice were investigated. 158 responses were collected. In case of a Frykman type I, the majority of surgeons would have performed a closed reduction and intrafocal K-wiring (37.3%) or volar plating (34.8%). In case of a Frykman type VII, volar

Clostridial gas gangrene complicating Colles' fracture.

We present a case of Clostridial gas gangrene following closed reduction of a Colles' fracture. A review of the laboratory and clinical literature on treatment of gas gangrene revealed wide differences of opinion regarding the effectiveness of surgery, antibiotics, antitoxin and especially hyperbaric oxygen. We found no previous report of clostridial gas gangrene in closed reduction of a Colles' fracture. The survival rates for 28 cases of Clostridial myonecrosis treated at Vancouver General Hospital with surgery, antibiotics and hyperbaric oxygen were 100% for extremity gangrene and 65% for t

Scoring system for assessing the treatment of supracondylar fractures of the humerus.

This study follows up supracondylar fractures of the humerus in 41 children. Twenty-one boys and 20 girls were observed over an average period of 4.6 yr. The ages at the time of the fracture varied between 1 and 15 yr, the highest incidence occurring between 5 and 8 yr. Twenty-four patients were treated by closed reduction and long arm cast fixation, and 17 were handled by closed reduction after overhead skeletal traction. The children who were treated with skeletal traction had comparatively more-severe fractures. Although the results have no statistical significance, a greater range of motio

Bilateral trans-radial trans-scaphoid perilunate fracture-dislocation with bilateral dorsal elbow dislocation: a case report.

BACKGROUND: Bilateral trans-radial trans-scaphoid perilunate fracture-dislocation is one of the less common injuries of the wrist, occurring as a consequence of high-energy trauma. Its coincidence with bilateral elbow dislocation is extremely rare. CASE PRESENTATION: The authors present a case of a 24-year old male of Czech origin who attained this unique injury by falling from a height of 4 m onto both upper extremities. Bilateral open reduction of the wrists using both palmar and dorsal approaches and closed reduction of both elbow joints was performed. The arms were immobilized in casts fo

Traditional radiography versus computed tomography to assess reduced distal radius fractures.

INTRODUCTION: This study compares computed tomography (CT) with plain radiography in its ability to assess distal radius fracture (DRF) malalignment after closed reduction and cast immobilization. METHODS: Malalignment is defined as radiographic fracture alignment beyond threshold values according to the Dutch guideline encompassing angulation, inclination, positive ulnar variance and intra-articular step-off or gap. After identifying 96 patients with correct alignment on initial post-reduction radiographs, we re-assessed alignment on post-reduction CT scans. RESULTS: Significant discrepanci
