Comparison of medium-weight vs heavy-weight mesh in minimally invasive extraperitoneal ventral hernia repairs.

BACKGROUND: Minimally invasive (MIS) extraperitoneal ventral hernia repair (VHR) has become increasingly common, however, limited data exist comparing medium-weight (MW) versus heavyweight (HW) mesh in this setting. We hypothesized that HW mesh in this context would be associated with decreased rates of hernia recurrence at 1 year and beyond compared to MW mesh. METHODS: We conducted a retrospective analysis of prospectively collected data from the Abdominal Core Health Quality Collaborative database, identifying adults who underwent MIS extraperitoneal (preperitoneal and/or retromuscular) VH

Subsampling strategies for compressive Hadamard single-pixel imaging.

Single-pixel imaging (SPI) based on Hadamard multiplexing enables efficient image acquisition with a reduced number of measurements, but its performance critically depends on the choice of subsampling strategy. In this work, we conduct a comprehensive evaluation of subsampling strategies for compressive Hadamard SPI, aiming to identify the method that selects the best-suited subset of Hadamard basis vectors. Natural images are known to contain most of their information in low spatial frequencies. Hence, the motivation for subsampling low-sequency patterns first, where sequency is the Hadamard

Influence of context on engagement with COVID-19 testing: a scoping review of barriers and facilitators to testing for healthcare workers, care homes and schools in the UK.

OBJECTIVE: The UK government's response to the COVID-19 pandemic included a 'test, trace and isolate' strategy. Testing services for healthcare workers, care homes and schools accounted for the greatest spend and volume of tests. We reviewed relevant literature to identify common and unique barriers and facilitators to engaging with each of these testing services. DESIGN: Scoping review. SEARCH STRATEGY: PubMed, Scopus and the WHO COVID-19 Research Database were searched for evidence published between 1 January 2020 and 7 November 2022. This was supplemented by evidence identified via free-t

Stereotypical love: a cluster analysis of self-presentation strategies in tinder profile pictures.

BACKGROUND: Although dating apps are the preferred means of meeting sexual and romantic partners, users frequently experience disappointment, highlighting the importance of understanding self-presentation strategies and selection processes to mitigate negative experiences. AIM: This study examines self-presentation strategies in Tinder profile pictures, aiming to identify typological patterns, characterize common profile-building strategies, and analyze differences by age, gender, and sexual orientation. METHODS: We employed a mixed dimensional approach-both categorical and numerical-to char

Enhancing information on stage at diagnosis of cancer in Africa.

BACKGROUND/PURPOSE: Stage at diagnosis is an important metric in treatment and prognosis of cancer, and also in planning and evaluation of cancer control. For the latter purposes, the data source is the population-based cancer registry (PBCR), but, although stage is usually among the variables collected by cancer registries, it is often missing, especially in low-income settings. Essential TNM has been introduced to facilitate abstraction of stage data by cancer registry personnel, but the accuracy with which they can do so is unknown. METHODS: 51 cancer registrars from 20 countries of sub-Sa

Disruption of c-MYC Binding and Chromosomal Looping Involving Genetic Variants Associated With Ankylosing Spondylitis Upstream of the RUNX3 Promoter.

Background: Ankylosing Spondylitis (AS) is a common form of inflammatory spinal arthritis with a complex aetiology and high heritability, involving more than 100 genetic associations. These include several AS-associated single nucleotide polymorphisms (SNPs) upstream of RUNX3, which encodes the multifunctional RUNT-related transcription factor (TF) 3. The lead associated SNP rs6600247 (p = 2.6 × 10-15) lies ∼13kb upstream of the RUNX3 promoter adjacent to a c-MYC TF binding-site. The effect of rs6600247 genotype on DNA binding and chromosome looping were investigated by electrophoretic mobilit

Whence Resistance?

BACKGROUND: Antimicrobial resistance results from a complex interaction between pathogenic and non-pathogenic bacteria, antimicrobial pressure, and genes, which together comprise the total body of potential resistance elements. The purpose of this study is to review and evaluate the importance of antimicrobial pressure on the development of resistance in a single surgical intensive care unit. METHODS: We reviewed a prospectively collected dataset of all intensive care unit (ICU)-acquired infections in surgical and trauma patients over a 6-y period at a single hospital. Resistant gram-negative

Brachial plexus injuries: outcome following neurotization with intercostal nerve.

OBJECT: Brachial plexus root avulsion injuries, which are devastating, usually result from high-speed accidents. Nerve transfer provides hope for successful treatment of this difficult set of injuries. Nevertheless, the controversies regarding indications, techniques, and outcome of the various available surgical procedures continue. METHODS: A retrospective analysis was performed in 51 patients (43 male and eight female patients) with brachial plexus injuries who underwent neurotization at the authors' institute between 1997 and 2003. Clinical, electrophysiological, and imaging data were use

Priorities, Approaches, and Performance for Outpatient Antithrombotic Stewardship: The ADAPT Project.

BACKGROUND: Antithrombotic (AT) agents are widely used and highly effective but are frequently implicated in preventable adverse events. Despite growing interest in AT stewardship, the outpatient care environment lacks a standardized measurement infrastructure analogous to what has been achieved in antimicrobial stewardship. The authors sought to characterize current outpatient practices across five care process priorities (CPPs) and examine gaps between clinical monitoring and system-level tracking to support future quality measurement and benchmarking. METHODS: The researchers conducted a s

Design principles of the common Gly-X6-Gly membrane protein building block.

UNLABELLED: Protein behavior in lipid is poorly understood and inadequately represented in current computational models. Design and prediction abilities for bilayer-embedded molecular structures may be improved by characterizing membrane proteins' most frequent, favored structural features to glean both context-specific and general principles. We used protein design to proactively interrogate the sequence-structure relationship and stabilizing atomic details of two highly prevalent antiparallel transmembrane (TM) motifs with Small-X 6 -Small consensus sequences. A fragment-based data-mining an

Trajectories of change in body mass index during inpatient treatment for severe anorexia nervosa during adolescence: predictive factors and hospitalization outcomes.

BACKGROUND: Weight restoration is one of the main goals of treatment for AN. Weight trajectories derive from various elements including baseline personal characteristics and factors linked to the course of treatment. The aim of our research was to identify different BMI trajectories during inpatient treatment, to examine whether patient characteristics were predictive of the nature of these trajectories, and to examine how they affect hospitalization outcomes. METHODS: The study population consisted of 310 female AN inpatients. To analyse the data, we examined trajectories of change in BMI, u

Explainable Automated TI-RADS Evaluation of Thyroid Nodules.

A thyroid nodule, a common abnormal growth within the thyroid gland, is often identified through ultrasound imaging of the neck. These growths may be solid- or fluid-filled, and their treatment is influenced by factors such as size and location. The Thyroid Imaging Reporting and Data System (TI-RADS) is a classification method that categorizes thyroid nodules into risk levels based on features such as size, echogenicity, margin, shape, and calcification. It guides clinicians in deciding whether a biopsy or other further evaluation is needed. Machine learning (ML) can complement TI-RADS classif

Systematic review of applied mathematical models for the control of Schistosoma japonicum.

BACKGROUND: Schistosoma japonicum remains endemic in China and the Philippines. Substantial progress has been made in the control of Japonicum in both China and the Philippines. China is reaching elimination thanks to a concerted effort of control strategies. Mathematical modelling has been a key tool in the design of control strategies, in place of expensive randomised-controlled trials. We conducted a systematic review to investigate mathematical models of Japonicum control strategies in China and the Philippines. METHODS: We conducted a systematic review on July 5, 2020, in four electronic

The lived experience of adults and parents: Transitioning from paediatric to adult health care with oesophageal atresia and tracheo-oesophageal fistula.

AIM AND OBJECTIVE: To explore the experience of healthcare transition from paediatric to adult health care for adults born with oesophageal atresia and tracheo-oesophageal fistula (OA/TOF) and parents. BACKGROUND: OA/TOF is a rare and chronic health condition that can require lifelong medical follow-up and management. There is evidence to suggest that transitioning from paediatric to adult health care can be problematic for people with rare and chronic conditions, including OA/TOF. The previous literature suggests that the experience of transitioning with a rare condition is more complex than

Getting airtime: Exploring how patients shape the stories they tell health practitioners.

INTRODUCTION: Effective communication during health encounters is known to decrease patient complaints, increase patient adherence and optimise health outcomes. While the aim of patient-centred care is to find common ground, health practitioners tend to drive the encounter, often interrupting patients within the first minute of the clinical conversation. Optimal care for people with chronic illnesses requires individuals to interact with health practitioners regarding their health concerns, but given these constraints, we know little about how patients strategise conversations with their care

The interRAI Suite of Mental Health Assessment Instruments: An Integrated System for the Continuum of Care.

The lives of persons living with mental illness are affected by psychological, biological, social, economic, and environmental factors over the life course. It is therefore unlikely that simple preventive strategies, clinical treatments, therapeutic interventions, or policy options will succeed as singular solutions for the challenges of mental illness. Persons living with mental illness receive services and supports in multiple settings across the health care continuum that are often fragmented, uncoordinated, and inadequately responsive. Appropriate assessment is an important tool that healt
