Quantitative neuroimaging in mucopolysaccharidoses clinical trials.

The mucopolysaccharidosis (MPS) disorders are rare lysosomal storage disorders caused by mutations in lysosomal enzymes involved in glycosaminoglycan (GAG) degradation. The resulting intracellular accumulation of GAGs leads to widespread tissue and organ dysfunction. In addition to somatic signs and symptoms, patients with MPS can present with neurological manifestations such as cognitive decline, behavioral problems (e.g. hyperactivity and aggressiveness), sleep disturbances, and/or epilepsy. These are associated with significant abnormalities of the central nervous system (CNS), including wh

[The German Surgery Congress from the perspective of young surgeons; expectations and barriers : Results of a nationwide online survey].

BACKGROUND: The German Surgery Congress (DCK) represents a central platform for scientific exchange and continuing medical education in the surgical disciplines in Germany; however, participation rates among young surgeons appear disproportionately low relative to the target population. OBJECTIVE: The study assessed the expectations, barriers to participation and potential areas for improvement of the DCK from the perspective of young surgeons. MATERIAL AND METHODS: An anonymous web-based cross-sectional survey was conducted among young surgeons in Germany. The questionnaire included sociode

Faculty Development Needs in Southeastern Conference Nursing Schools.

BACKGROUND: This study examined nurse faculty members' teaching, scholarship, and professional development interests across Southeastern Conference (SEC) nursing schools, and evaluated differences in these needs associated with years in academia, tenure status, faculty rank, and Accountable Health Communities (AHC) versus non-AHC settings. METHOD: Rosenthal and Stanberry's framework for faculty development provided the theoretical foundation for this cross-sectional study to explore topics of interests of faculty at 12 SEC nursing schools. RESULTS: A total of 470 faculty responded to the onl

Convening on the influenza human viral challenge model for universal influenza vaccines, Part 2: Methodologic considerations.

In response to global interest in the development of a universal influenza vaccine, the Bill & Melinda Gates Foundation, PATH, and the Global Funders Consortium for Universal Influenza Vaccine Development convened a meeting of experts (London, UK, May 2018) to assess the role of a standardized controlled human influenza virus infection model (CHIVIM) towards the development of novel influenza vaccine candidates. This report (in two parts) summarizes those discussions and offers consensus recommendations. Part 1 covers challenge virus selection, regulatory and ethical considerations, and issues

Citing conduct, individualizing symptoms: Accomplishing autism diagnosis in clinical case conferences.

In this paper, I examine how clinicians at a clinic for developmental disabilities in the United States determine whether children being evaluated for autism spectrum disorder (ASD) showed symptoms of that condition. Drawing on a convenience sample of 61 audio and video recorded case conferences from two time periods (1985 and 2011-15), and combining Conversation Analysis with insights from Actor Network Theory, I find that clinicians describe (via a representational practice called "citation") children's conduct in ways that advance diagnostic claims. More specifically, they portray key actan

[Hans Selye, the grandmaster of creativity and originality].

Hans Selye, the father of the stress concept, was a giant of science of the twentieth century. Beyond his best-known work on stress, he also made several discoveries on various other fields of experimental medicine. He described and characterized various pluricausal diseases. In addition, he made pivotal contributions to the broad field of endocrinology, especially to the classification of steroids and to our better understanding of their mode of action. He developed surgical technics and experimental animal models suitable for studying the pathogenesis and prevention of human diseases. Selye

Knowledge of participants of the 5th Polish Foot and Ankle Society (PFAS) Congress about the diagnosis and treatment of plano-valgus feet secondary to posterior tibial tendon dysfunction (PTTD).

BACKGROUND: Posterior tibial tendon dysfunction (PTTD) ranks among the most common causes of adult acquired flatfoot deformity. The deformity develops gradually through characteristic stages and its early manifestations are often ignored or mis-diagnosed. The aim of the study was to gain an insight into what the participants of the 5th Polish Foot and Ankle Society Congress knew about the diagnosis and treatment of flatfoot. MATERIAL AND METHODS: An anonymous survey described the clinical presentation of a hypothetical patient with fixed stage III (according to the Johnson and Strom classific

γδ T-cell conference 2012: close encounters for the fifth time.

The fifth international γδ T-cell conference was held in Freiburg, Germany, from May 31 to June 2, 2012, bringing together approximately 170 investigators from all over the world. The scientific program covered topics such as thymic development and the mechanisms of ligand recognition and activation, the interaction of γδ T cells with other immune and non-immune cells and its implications for homeostasis, infection, tissue repair and autoimmunity, and the role of γδ T cells in malignancy and their potential for novel immunotherapies. Here we discuss a selection of the oral communications at th

The precarious state of the liver after a Fontan operation: summary of a multidisciplinary symposium.

As the cohort of survivors with the single-ventricle type of congenital heart disease grows, it becomes increasingly evident that the state of chronically elevated venous pressure and decreased cardiac output inherent in the Fontan circulation provides the substrate for a progressive decline in functional status. One organ at great risk is the liver. Wedged between two capillary beds, with the pulmonary venous bed downstream, which typically has no pulsatile energy added in the absence of a functional right ventricle, and the splanchnic bed upstream, which may have compromised inflow due to in

Will performance measurement lead to better patient outcomes? What are the roles of the National Quality Forum and Medical Specialty Societies?

Performance measures (PMs) are specified metrics by which a health-care provider's care can be compared with national benchmarks. The use of PMs is a key component of efforts to improve the quality and value of health care. The National Quality Forum (NQF) is the federally recognized endorser of PMs. From 2006 to 2009, the Quality Improvement Committee (QIC) of the American College of Chest Physicians engaged in the review of proposed PMs as a member of the NQF. This article provides a review of the QIC's experience with PMs and NQF membership and the lessons learned, an overview of the enhanc

Hypothalamic obesity in children.

Hypothalamic obesity is an intractable form of obesity syndrome that was initially described in patients with hypothalamic tumours and surgical damage. However, this definition is now expanded to include obesity developing after a variety of insults, including intracranial infections, infiltrations, trauma, vascular problems and hydrocephalus, in addition to acquired or congenital functional defects in central energy homeostasis in children with the so-called common obesity. The pathogenetic mechanisms underlying hypothalamic obesity are complex and multifactorial. Weight gain results from dam

[Stendhal syndrome: origin, characteristics and presentation in a group of neurologists].

INTRODUCTION: Travelling, when searching for knowledge and emotion, can cause psychic discomfort that occasionally leads the traveller to seek medical attention. The psychiatrist Graziella Magherini described, in tourists visiting Florence, acute attacks including disorders of thought and affects, and even including, anxiety attack. She named it the Stendhal syndrome (SS) remembering the experience of the writer when visiting the Basilica of Santa Croce in Florence. METHODS: We attempt to investigate the incidence of SS or isolated symptoms related to it, in a homogeneous group of travellers.

[History of neurology and education on neurology in Japan].

The first medical society of Japanese neurologists and psychiatrists was founded in 1902, but psychiatrists gradually dominated in number. New "Japanese Society of Neurology" (JSN) was founded in 1960. The number of members was only 643 in 1960, while it rose up to 8,555 in 2009, including regular, junior, senior and associate members. JSN contributed much to solve the causes and treatment of the medicosocial and iatrogenic diseases such as Minamata disease and SMON (subacute myelopticoneuropathy) at its early period. In undergraduate education at medical school neurology is one of the core su

Genetic engineering for xenotransplantation.

Xenotransplantation is being pursued vigorously to solve the shortage of allogeneic donor organs. Experimental studies of the major xenoantigen (Gal) and of complement regulation enable model xenografts to survive hyperacute rejection. When the Gal antigen is removed or reduced and complement activation is controlled, the major barriers to xenograft survival include unregulated coagulation within the graft and cellular reactions involving macrophages, neutrophils, natural killer (NK) cells, and T lymphocytes. Unlike allografts, where specific immune responses are the sole barrier to graft surv

The efforts of WHO and Pugwash to eliminate chemical and biological weapons--a memoir.

The World Health Organization and the Pugwash Conferences on Science and World Affairs (Nobel Peace Prize 1995) have been involved in questions concerning chemical and biological arms since the early 1950s. This memoir reviews a number of milestones in the efforts of these organizations to achieve the elimination of these weapons through international treaties effectively monitored and enforced for adherence to their provisions. It also highlights a number of outstanding personalities who were involved in the efforts to establish and implement the two major treaties now in effect, the Biologic
